Quiz case: a clinical reasoning challenge in the emergency stroke setting
AbstractA right-handed woman in her 80s was admitted to the emergency department 1 h after sudden-onset global aphasia and right-sided hemiparesis. Medical history included arterial hypertension, dyslipidemia, hyperuricemia, aortic stenosis, osteoporosis, and recent pulmonary embolism. Medication consisted of apixaban, bisoprolol, hydrochlorothiazide, allopurinol, fenofibrate, and vitamin D. Vital parameters (blood pressure, heart rate, body temperature, oxygen saturation) and glycemia were all within normal range. Electrocardiogram showed a first-degree atrioventricular block. The patient was promptly transported to the e...

Zero-contrast imaging for the assessment of transcatheter aortic valve implantation in candidates with renal dysfunction
CONCLUSION: The proposed contrast-free imaging protocol appears to be a promising clinical tool for pre-TAVI evaluation in patients with severe renal dysfunction.PMID:37350345 | DOI:10.1080/0886022X.2023.2224888

Clinical outcomes of the ACURATE  neo2 transcatheter heart valve: a prospective, multicenter, observational, post-market surveillance study
CONCLUSIONS: The study results support the safety and efficacy of TAVI with ACURATE neo2 in patients in routine clinical practice.PMID:36440588 | DOI:10.4244/EIJ-D-22-00914

Investigation on Imaging Features and Clinical Significance of Cardiac CT in Comprehensive Evaluation of Aortic Valve and Root before Percutaneous Aortic Valve Replacement
This study retrospectively includes patients with severe aortic stenosis who underwent TAVR with routine computed tomography. Based on CT images, the determination and grouping of bicuspid aortic valve and tricuspid aortic valve were completed. Thirteen cross-sectional levels of the aorta-iliac-femoral vascular access were completed. The results showed that 3 people had stroke (17.6%) and 5 people had myocardial infarction (29.4%) during the follow-up period. Atrial fibrillation occurred in 5 patients (29.4%), permanent pacemaker implantation was performed in 1 patient (5.9%), and acute kidney injury occurred in 7 patients...

Thoracic Aortic < sup > 18 < /sup > F-Sodium Fluoride Activity and Ischemic Stroke in Patients With Established Cardiovascular Disease
CONCLUSIONS: In patients with established cardiovascular disease, thoracic aortic 18F-sodium fluoride activity is associated with the progression of atherosclerosis and future ischemic stroke. Arterial 18F-sodium fluoride activity identifies localized areas of atherosclerotic disease activity that are directly linked to disease progression and downstream regional clinical atherothrombotic events. Association Of Stroke With Pattern Of Aortic Leaflet And Left Ventricular Outflow Tract Calcifications
Introduction: Stroke is a feared complication of transcatheter aortic valve replacement (TAVR) used for treatment of severe aortic stenosis. The risk of stroke has been associated with leaflet calcification burden. We aimed to investigate whether a specific pattern of aortic leaflet and left ventricular outflow tract (LVOT) calcification is associated with stroke risk.

Comparison of Transcatheter Aortic Valve Replacement between Self-Expanding versus Balloon-Expandable Valves in Patients with Small Aortic Annulus
CONCLUSIONS: TAVR with self-expanding valves was associated with superior hemodynamic outcomes compared with balloon-expandable valves in patients with small aortic annulus.PMID:33655721 | DOI:10.4070/kcj.2020.0409
