Natural History and Sequelae of Penetrating Aortic Ulcer
A man in his 70s with history of tobacco use and hypertension presented for transesophageal echocardiography after a recent diagnosis of ischemic stroke. This demonstrated a moderate right-to-left shunt on Valsalva release and no left atrial appendage thrombus. Assessment of the thoracic aorta revealed a complex penetrating aortic ulcer and saccular aneurysm with thrombus in the distal arch and proximal descending thoracic aorta (Figure). Computed tomography angiography confirmed the presence of multiple penetrating ulcerations with large saccular aneurysms.

Effect of mechanical thrombectomy with and without intravenous thrombolysis on the functional outcome of patients with different degrees of thrombus perviousness
ConclusionPatients with thrombi of lower perviousness were recommended to undergo intravenous alteplase followed by endovascular thrombectomy, and those with thrombi of higher perviousness could undergo thrombectomy alone.

P135/313 Predictive value of CT head and CT angiogram in identifying ICAD associated large vessel occlusion
ConclusionOur findings suggest a significant association between the absence of the hyperdense artery sign and the presence of tapering at the occlusion site with ICAD in patients with LVO. ConclusionsIn this case series performed over a 10-year period at a high-volume stroke center we show that CAS is safe and efficient in patients with SyNC. DiscussionThis case illustrates the important clinical aspects and physical findings needed to diagnose DRS. The presence of focal allodynia and hyperalgesia with recent cerebrovascular incidents provides significant evidence for DRS. E-150 Mechanical thrombectomy in distal middle cerebral artery occlusions using a novel blind exchange mini-pinning technique: experience in 7 consecutive cases
ConclusionBlind mini-pinning allows for combined use of distal stentriever with a 035 thrombectomy catheter, affording a larger ID than traditional microcatheter engagement of the thrombus with enhanced stability for removal, and improved reperfusion rates in distal occlusions. O-012 Relationship between clot composition and procedural results for ischemic stroke aspiration thrombectomy: subset analysis from the INSIGHT registry
ConclusionsClot removed during neuroendovascular procedures can differ in consistency and removability, leading to an increased focus on thrombus composition as a crucial variable for improving outcomes of ischemic stroke. In this study, good final revascularization (mTICI 2b-3 at the final angiogram) occurred after aspiration thrombectomy regardless of clot composition. ConclusionsThe first-year experience demonstrates the effectiveness and safety of the transitioning from TPA to TNK. The higher early recanalization rates with TNK are striking.
