Erythropoiesis-stimulating agents for anaemia in adults with chronic kidney disease: a network meta-analysis
CONCLUSIONS: The comparative effects of different ESAs on blood transfusions, death (any cause and cardiovascular), major cardiovascular events, myocardial infarction, stroke, vascular access thrombosis, kidney failure, fatigue and breathlessness were uncertain.PMID:36791280 | PMC:PMC9924302 | DOI:10.1002/14651858.CD010590.pub3

Independent risk factors for an increased incidence of thromboembolism after lung transplantation
ConclusionsThe incidence of TE after LTX is high, especially in lung transplant recipients with a BMI  >  25 and an age >  55 years as well as cardiovascular risk factors closely associated with the metabolic syndrome. As these patients comprise a growing recipient fraction, intensified research should focus on the risks and benefits of regular screening or a prolonged TE prophylaxis in these patients.Trial registration number DKRS: 00021501.

Contemporary Mechanical Circulatory Support With Continuous-Flow Biventricular Assist Devices: A Systematic Review
As the incidence of heart failure increases, so too has that of biventricular failure. While transplantation remains the gold standard therapy for end-stage heart failure, the limited organ supply has increased the need for durable mechanical circulatory support. We therefore sought to conduct a systematic review of continuous-flow ventricular assist devices in a biventricular configuration (CF-BiVAD). An electronic search of PubMed and Cumulative Index to Nursing and Allied Health Literature (CINAHL) databases was performed using the keyword “BIVAD.” Studies were reviewed to identify discrete variables, including impl...

103 High rates of venous and arterial thrombotic events in POEMS patients
Conclusion Arterial and venous thromboses are more common in POEMS syndrome than previously reported and appear associated with disease pathology and activity. We propose a management strategy for appropriate thromboprophylaxis in POEMS patients throughout the course of disease. stephen.keddie@nhs.net

Late-onset cerebral embolism after transcatheter aortic valve implantation under direct oral anticoagulant therapy
We describe the case of a 92-year-old woman with delayed valve thrombosis causing stroke after TAVI, who developed recurrent strokes eight months after TAVI for severe aortic valve stenosis. Transesophageal echocardiography and cardiac computed tomography revealed a string-like thrombus attached to the implanted valve.

Physiological reno-portal bypass in liver transplantation with non-tumorous portal vein thrombosis
AbstractReno-portal anastomosis (RPA) in presence of spleno-renal shunts (SRS) is a physiological option to restore blood flow in liver transplantation with portal vein thrombosis (PVT). Diffuse splanchnic venous system thrombosis (complex PVT) is its main indication but RPA proved to be useful in selected cases of less extensive thrombosis (non-complex PVT). Up until now only two monocentric and one multicentric case series has been published on this topic in addition to few anecdotal reports. After 2014, we introduced RPA in our institution to manage some cases of complex PVT in presence of SRS. Here, we present the evol...

Six ‐month outcomes in postapproval HeartMate3 patients: A single‐center US experience
ConclusionIn this single-center study, the real-world 6-month survival in the postapproval group was comparable to the trial results. Further studies are needed to monitor long-term outcomes.

Computational Investigation of Outflow Graft Variation Impact on Hemocompatibility Profile in LVADs
This study, used a simulation-based approach to investigate the influence that the cannula orientation has in modulating flow conditions leading to HRAEs.

Stroke and Pump Thrombosis Following LVAD Implantation: The Impact of the Implantation Approach
The aim of this study is to investigate the impact of LVAD (left ventricular assist device) implantation technique on long-term mortality and morbidity, with special focus on stroke and pump thrombosis.

Antiplatelet agents for chronic kidney disease
CONCLUSIONS: Antiplatelet agents probably reduced myocardial infarction and increased major bleeding, but do not appear to reduce all-cause and cardiovascular death among people with CKD and those treated with dialysis. The treatment effects of antiplatelet agents compared with each other are uncertain.PMID:35224730 | DOI:10.1002/14651858.CD008834.pub4

Bacteriophage Therapy and the Biofilm: An Old Solution for a Persistent and Desperate Problem?
With the adoption of use of continuous-flow pumps with centrifugal design and complete magnetic levitation (i.e., HeartMate 3, Abbott, Chicago, IL), outcomes following durable left ventricular assist device (LVAD) have continuously improved over the last five years (1). Improved hemocompatibility associated with this device design has translated into a significant reduction in major adverse events, primarily pump thrombosis, stroke, and to a lesser extent, reduction in non-surgical bleeding (1).

Stroke epidemiology and outcomes in the modern era of left ventricular assist devices
AbstractThe care for the patients with end-stage heart failure has been revolutionized by the introduction of durable left ventricular assist devices, providing a substantial improvement in patient survival and quality of life and an alternative to heart transplantation. The newest devices have lower instances of mechanical dysfunction and associated pump thrombosis. Despite these improvements in complications, the use of continuous flow assist devices is still associated with high rates of thrombotic and hemorrhagic complications, most notably stroke in approximately 10% of continuous flow assist devices patients per year...

Twelfth Interagency Registry for Mechanically Assisted Circulatory Support Report: Readmissions after LVAD
Ann Thorac Surg. 2022 Jan 7:S0003-4975(22)00007-8. doi: 10.1016/j.athoracsur.2021.12.011. Online ahead of print.ABSTRACTThe twelfth annual report from the Society of Thoracic Surgeons (STS) Interagency Registry for Mechanically Assisted Circulatory Support (Intermacs) highlights outcomes for 26,688 continuous-flow LVAD patients over the past decade (2011-2020). In 2020, we observed the largest drop in yearly LVAD implant volumes since the registry's inception, which reflects the effects of the COVID-19 pandemic on cardiac surgical volumes in the United States. The 2018 heart transplant allocation policy change in the U.S. ...

Liver Transplant in Hemoglobin SC Disease and Autoimmune Hepatitis: A Case Report
We report a case of a 50-year-old man with sickle cell disease genotype SC (HbSC) and cirrhosis secondary to autoimmune hepatitis who underwent liver transplant. His postoperative course included upper extremity deep vein thrombosis, pulmonary embolus, stroke via a patent foramen ovale after a line removal, and posterior reversible encephalopathy syndrome. Fortunately, he is alive with a functioning graft at 10 months after liver transplant. This case highlights the feasibility of liver transplant in sickle cell disease given the support of meticulous multidisciplinary care and the unique aspects of autoimmune hepatitis an...
