Gut microbiota and stroke
Benjamin Y. Q. Tan, Prakash R Paliwal, Vijay K SharmaAnnals of Indian Academy of Neurology 2020 23(2):155-158 Ischemic stroke remains a significant health problem, which is expected to increase owing to an aging population. A considerable proportion of stroke patients suffer from gastrointestinal complications, including dysphagia, gastrointestinal hemorrhage, and constipation. Often, these complications adversely affect stroke outcomes. Recent research postulates the role of “brain-gut axis” in causing gut microbiota dysbiosis and various complications and outcomes. In this review, we present ou...

Effect of age on arterial recanalization and clinical outcome in thrombolyzed acute ischemic stroke in CLOTBUST cohort
Conclusion: Elderly AIS patients achieve relatively poor functional outcome after IV-tPA despite similar rates of arterial recanalization. However, there is no increase in the hemorrhagic risk. Perhaps, decision for IV thrombolysis in elderly patients should be made cautiously.

Endovascular Treatment for Acute Tandem Occlusion Stroke: Results from Case Series of 17 Patients
Conclusion: Our study determined a promising outcome with reasonable good recanalization and clinical recovery for endovascular intervention in tandem ischemic. In the subgroup of treatment, “extracranial stent first” had more complex disease with completed ICA occlusion which required longer procedure time may lead to worse outcome.

A Singular Manifestation of Contrast-induced Encephalopathy Following Coronary Angiography
We report a 71-year-old woman with a past medical history of hypertension, type two diabetes and no history of renal disease. She was admitted to our cardiology department with symptoms of recurrent angina. She had a history of unstable angina two years ago and had undergone a percutaneous coronary intervention without incident. Three hours after un-elective coronary angiography, she experienced a sudden, transitory deterioration in her consciousness's level with neurovegetative symptoms (high blood pressure, high temperature). The cerebral Computer Tomography scan ruled out any acute hemorrhagic or ischemic str...

The Benefits and Risks of Statin Therapy in Ischemic Stroke: A Review of the Literature
Wang Zhao, Zhi-Jie Xiao, Shui-Ping ZhaoNeurology India 2019 67(4):983-992 Statins are effective cholesterol-lowering drugs for reducing the risks of mortality and morbidity of cardiovascular diseases. Increasing evidence has shown that statin use is associated with a significant beneficial effect in patients with ischemic stroke. Both pre-stroke and post-stroke statin use has been found to be beneficial in ischemic stroke. Furthermore, good adherence is associated with a better clinical outcome, and statin withdrawal is associated with a poor functional outcome in patients with ischemic stroke. High-intensity statin thera...

Antiplatelets versus Anticoagulants in the Treatment of Extracranial Carotid and Vertebral Artery Dissection
Conclusions: The risk of recurrent ischemic events in carotid and vertebral artery dissection is low and is irrespective of the type of antithrombotic treatment. Stroke as the presenting event and severity of stroke at onset were significant predictors of a poor three-month outcome.

Hemorrhagic Stroke in Chronic Kidney Disease.
Authors: Jha VK, Sharda V, Mirza SA, Shashibhushan, Bhol KK Abstract Hemorrhagic stroke is leading cause of death in Chronic Kidney Disease (CKD) population. Uremic patients are susceptible to hemorrhagic complications due to multiple reasons i.e platelet dysfunction, low platelet number, use of heparin during hemodialysis, use of anticoagulants for thromboembolic risk etc. Prevention and treatment of hemorrhagic stroke is complicated in CKD setting and if not managed properly can lead to several fold increased mortality and morbidity rate. In this brief review we will discuss about the magnitude of hemorrhagic str...

The risk of symptomatic intracranial hemorrhage after thrombolysis for acute stroke: Current concepts and perspectives
Conclusions: In addition to the already known risk factors considered in the standard protocols, an individualized evaluation of risks is needed to minimize the risk of brain hemorrhage after thrombolysis for ischemic stroke.

Validation of ICH and ICH-GS Scores in an Indian Cohort: Impact of Medical and Surgical Management
Prognostic scores help in predicting mortality and functional outcome post intracerebral hemorrhage (ICH). We aimed to validate the ICH and ICH-GS scores in a cohort of Indian patients with ICH and observe the impact of any surgical intervention on prognostication.

Acute cholecystitis in patients with stroke
Conclusions: Acute cholecystitis and stroke are closely associated, and anti-thromboembolic drugs may cause hemorrhagic cholecystitis. Stroke patients tend to have atherosclerotic risk factors resulting in ischemic injury of the gallbladder. Furthermore, severe hemiparesis, a fasting state, dehydration, or bacteremia, which are occasionally exhibited by stroke patients, are known risk factors for acalculous cholecystitis. Stroke patients, especially patients with aphasia and consciousness disturbance, require immediate abdominal examination, if acute cholecystitis is suspected.

Age- and Sex-Associated Impacts of Body Mass Index on Stroke Type Risk: A 27-Year Prospective Cohort Study in a Low-Income Population in China
Conclusions Being overweight increased the risk of both ischemic and hemorrhagic strokes; obesity was only associated with an increased risk of IS. Additionally, the positive association between BMI and stroke risk was only observed in participants aged <65 years and the associations differed between men and women. Being overweight increased the risk of both ischemic and hemorrhagic strokes in men and being underweight increased their risk of hemorrhagic stroke. In women, being overweight increased the hemorrhagic stroke risk, whereas obesity increased their IS risks. The high prevalence of hypertension and elevat...
