Oral antidiabetic drugs and dementia risk: Does treatment matter?
As the population ages, dementia grows as a public health problem. The rising life expectancy and the aging of the so-called baby boomer cohort translate to a substantial number of people reaching ages of high risk for age-related conditions like dementia. As a major cause of disability and dependency in elderly people, dementia puts social and economic burden on patients and their families and affects health care systems worldwide. Thrombolysis in acute ischemic stroke in patients with dementia: A Swedish registry study
Conclusions: Younger patients with dementia and AIS are less likely to receive IVT. Among patients receiving thrombolysis, there are no differences in sICH or death, although patients with dementia have worse accommodation and functional outcomes at 3 months.

Change in multimodal MRI markers predicts dementia risk in cerebral small vessel disease
Conclusions: This longitudinal prospective study provides evidence that change in MRI measures including DTI, over time durations during which cognitive change is not detectable, predicts cognitive decline and progression to dementia. It supports the use of MRI measures, including DTI, as useful surrogate biomarkers to monitor disease and assess therapeutic interventions.

Waxing and waning of white matter hyperintensities
Small vessel disease (SVD) manifests in myriad ways, most prominently as white matter hyperintensities (WMH).1,2 By the age of 60 years, virtually every healthy individual has evidence of WMH,3 and those with ischemic stroke have even more extensive changes.4 Cerebral ischemic injury from occlusion or stenosis of deep penetrating arteries due to (uncontrolled) hypertension ranks highest among the potential causes of WMH.5 These WMH are among the most important vascular contributors to cognitive decline, dementia, and parkinsonism.6

White matter hyperintensity reduction and outcomes after minor stroke
Conclusions: Some WMH may regress after minor stroke, with potentially better clinical and brain tissue outcomes. The role of risk factor control requires verification. Interstitial fluid alterations may account for some WMH reversibility, offering potential intervention targets.

Aggressiveness of care following intracerebral hemorrhage in women and men
Conclusions: After ICH, women do not receive less aggressive care than men after controlling for the substantial comorbidity differences. Future studies on sex bias should include the presence of comorbidities, prestroke disability, and other factors that may influence management.

Insulin resistance among obese middle-aged is associated with decreased cerebrovascular reactivity
Conclusions: IR is associated with impaired CVR; the relationship appears to be driven by the degree of IR and not by obesity. These rarely reported results suggest that early forms of cerebrovascular dysfunction exist among obese middle-aged individuals with significant IR but without type 2 diabetes mellitus. These functional vascular abnormalities may help explain the associations among IR, diabetes, and dementia, and suggest that interventions aiming to improve IR or CVR may help prevent cognitive decline later in life.

Hospital safety among neurologic patients: A population-based cohort study of adverse events
Conclusions: This study demonstrates that neurologic patients have a high proportion of AEs in hospital. The findings provide information on the quality and safety of care for people with neurologic conditions in hospital, which can help inform future quality improvement initiatives.

Cerebrovascular disease affects brain structural integrity long before clinically overt strokes
In the current issue of Neurology®, Werden et al.1 examined a group of patients experiencing first-ever or recurrent stroke to understand the relationships between stroke and cortical structural integrity. Relationship between carotid arterial properties and cerebral white matter hyperintensities
Conclusions: Large carotid artery diameters are associated with greater burden of white matter hyperintensity (WMH) in this multiethnic population. The association between increased diameters, decreased STRAIN, and greater WMH burden is more pronounced among Hispanics. These associations suggest a potential important pathophysiologic role of extracranial large artery remodeling in the burden of WMH.

Probable REM sleep behavior disorder and risk of stroke: A prospective study
Conclusions: Presence of pRBD was associated with a higher risk of developing stroke, including both ischemic and hemorrhagic types. Future studies with clinically confirmed RBD and a longer follow-up would be appropriate to further investigate this association.
