Helping Haiti: Addressing the Burden of Neurologic Disease (I10-2.004)

Incidence of cerebral microbleeds in preclinical Alzheimer disease
Conclusions: Older age, baseline LMBs, higher β-amyloid burden, and concomitant cerebrovascular disease may all confer higher risk of incident LMBs. This should be considered when designing protocols for amyloid-modifying clinical trials.

Cortical thickness mediates the effect of {beta}-amyloid on episodic memory
Conclusion: These results suggest strong links among Aβ, cortical thickness, and memory. They highlight that, in individuals without dementia, vascular risk also contributes to cortical thickness and influences the relationships among Aβ, cortical thickness, and memory.

Randomized Evaluation of Carotid Occlusion and Neurocognition (RECON) trial: Main results
Conclusion: Cognitive improvement following bypass surgery was not superior to medical therapy among patients with recently symptomatic carotid occlusion and increased OEF. Among those with no recurrent stroke, less hemodynamic impairment at baseline was associated with greater cognitive gain in both groups. Reversing cognitive impairment in hemodynamic failure remains an open challenge. Classification of evidence: This study provides Class II evidence that for patients with symptomatic ICA occlusion and increased OEF on PET, EC-IC bypass compared to no bypass does not improve cognitive function after 2 years.

How experienced community neurologists make diagnoses during clinical encounters
We describe 3 core domains of diagnosis: 1) clinical (C), 2) laboratory and electrodiagnostics (L), and 3) neuroimaging (N). Neurologists were uniform in their practices across these domains except within the clinical domain, where the physical examination varied considerably among clinicians. All neurologists coordinated findings from the 3 domains to arrive at a final diagnosis. This practice of coordination varied across common disease categories (e.g., meningitis vs dementia). Relations of arterial stiffness and endothelial function to brain aging in the community
Conclusions: Greater arterial stiffness and pressure pulsatility are associated with brain aging, MRI vascular insults, and memory deficits typically seen in Alzheimer dementia. Future investigations are warranted to evaluate the potential impact of prevention and treatment of unfavorable arterial hemodynamics on neurocognitive outcomes.

Risk of incident stroke in patients with Alzheimer disease or vascular dementia
Conclusions: Patients with VD, but not AD, have a markedly higher risk of developing an ischemic stroke than those without dementia. In patients with AD, but not VD, use of atypical antipsychotic drugs was associated with an increased risk of TIA.

Risk factors for dementia with Lewy bodies: A case-control study
Conclusion: DLB risk factors are an amalgam of those for AD and PD. Smoking and education, which have opposing risk effects on AD and PD, are not risk factors for DLB; however, depression and low caffeine intake, both risk factors for AD and PD, increase risk of DLB more strongly than in either.
