Impact of bridging with perioperative low-molecular-weight heparin on cardiac and bleeding outcomes of stented patients undergoing non-cardiac surgery.
In conclusion, LMWH bridging in patients with coronary stents undergoing surgery is a common and possibly harmful practice, resulting in worse ischaemic outcomes at 30 days, and a significant risk of bleeding. PMID: 26017898 [PubMed - as supplied by publisher]

Management of Bleeding Complications in Patients Taking Direct Oral Anticoagulants at a Large Tertiary Academic Medical Center
Abstract Direct oral anticoagulants (DOACs) offer a fixed-dose alternative to warfarin for stroke prevention in patients with atrial fibrillation. However, there is minimal evidence to guide the management of bleeding in patients taking a DOAC. We retrospectively evaluated the acute management of major bleeding in this patient population at a large, tertiary academic medical center. We evaluated various strategies of management including dialysis, transfusions, pharmacologic agents, surgical intervention, and reinitiation of anticoagulation. We also assessed length of intensive care unit and hospital stay, mor...

Vicky's Story
My older sister, Vicky, never listened to authority. One of my earliest childhood memories was around age four, seeing my Mom whip Vicky with a long tree twig to the point she was bleeding. My mother, a frustrated widow raising five girls on meager Social Security benefits in rural southeast Missouri used the only form of discipline she knew -- corporal punishment. As Vicky's 10-year-old old legs, back, and buttocks became covered in red dotted lines, she never cried or moaned. Instead she looked straight ahead, stone-faced. Mom perceived her look as defiance and gave her more lashings than she should have that day. Event...

Radial versus Femoral Access in Patients Presenting with Acute Coronary Syndrome in the Post-Matrix Era: Still One Size Does Not Fit All
Recently, the MATRIX trial(1) results were published. The authors randomized>8000 patients presenting with acute coronary syndromes (ACS) to either transfemoral or transradial approach. Althoughtouted as an overwhelming "win" for the radial approach, to the point of practice and guideline modification in North America,thetrial(1) missed the pre-specified α level of significance of 0.025 for theco-primary major adverse cardiovascular events (MACE) endpoint of the composite of all-cause mortality, myocardial infarction (MI),or strokewhile meeting a second co-primary net adverse cardiovascular events (NACE) endpoint of the c...

Harmonizing Optimal Strategy for Treatment of coronary artery diseases – comparison of REDUCtion of prasugrEl dose or POLYmer TECHnology in ACS patients (HOST-REDUCE-POLYTECH-ACS RCT): study protocol for a randomized controlled trial
DiscussionThe HOST-REDUCE-POLYTECH-ACS RCT is the first study exploring the optimal maintenance dose of prasugrel beyond 1 month after PCI for ACS in Asian all-comers. In addition, this is the largest study dedicated only to ACS patients to evaluate the polymer issue in the situation of ACS by directly comparing biostable-polymer PtCr-EES versus BP-BES.Trial registrationClinicalTrials.gov (ID: NCT02193971, 13 July 2014).

Antithrombotic Strategy Variability In ATrial Fibrillation and Obstructive Coronary Disease Revascularized with PCI - Rationale and study design of the prospective observational multicenter AVIATOR 2 Registry
Publication date: Available online 21 September 2015 Source:American Heart Journal Author(s): Jaya Chandrasekhar, Ioannis Mastoris, Usman Baber, Samantha Sartori, Mikkel Schoos, Sameer Bansilal, George Dangas, Roxana Mehran Background In the era of novel anti-thrombotic therapy, the optimal treatment for patients with non-valvular atrial fibrillation (AF) or flutter undergoing percutaneous coronary intervention (PCI) is undetermined. Study Design The AVIATOR 2 study is a multicenter prospective observational registry that will enroll approximately 2500 patients with non-valvular AF or flutter undergoing PCI star...

Prescribing patterns of target-specific oral anticoagulants: an academic hospital perspective
Target-specific oral anticoagulants have been rapidly adopted into clinical practice for stroke prophylaxis and venous thromboembolism treatment, raising concerns about off-label prescribing practices. We conducted a retrospective review of consecutive patients prescribed dabigatran, rivaroxaban or apixaban prior to inpatient hospitalization over an 18-month period to examine the off-label prescribing frequency, contraindications and related complications. Chart review included baseline demographics, hospital admitting service, outpatient prescribing service, renal function, therapeutic indication, echocardiographic findin...

Safety of prasugrel in real-world patients with ST-segment elevation myocardial infarction: 1-year results from a prospective observational study (Bleeding and Myocardial Infarction Study).
CONCLUSION: The rate of major bleeding, particularly out-of-hospital bleeding, in patients treated with prasugrel is low within 1 year after a STEMI. Accurate selection of patient candidates for prasugrel is likely to have reduced the risk of bleeding. PMID: 26514325 [PubMed - as supplied by publisher]

Brachiocephalic artery access in transcatheter aortic valve implantation: a valuable alternative: 3-year institutional experience
CONCLUSIONS TAVI implantation through the brachiocephalic artery is safe and feasible. The distance between the point of access and the aortic valve annulus is short, improving catheter stability and implant site accuracy. We consider it to be a valuable alternative in patients without femoral access.

Ticagrelor in Triple Antithrombotic Therapy: Predictors of Ischemic and Bleeding Complications
ConclusionsThe risk of bleeding and ischemic complications among patients on TT is similar between those on ticagrelor and clopidogrel. However, caution with use of bridging anticoagulation should be taken when using ticagrelor.

Comparison of Four Bleeding Risk Scores to Identify Rivaroxaban‐treated Patients With Venous Thromboembolism at Low Risk for Major Bleeding
ConclusionsFour scoring systems that use criteria obtained in routine clinical practice, derived to predict low bleeding risk with VKA treatment for VTE, identified patients with less than a 1% risk of major bleeding during full‐course treatment with rivaroxaban.

Pancreatogastrostomy Versus Pancreatojejunostomy for RECOnstruction After PANCreatoduodenectomy (RECOPANC, DRKS 00000767): Perioperative and Long-term Results of a Multicenter Randomized Controlled Trial
Conclusions: The rate of grade B/C fistula after PG versus PJ was not different. There were more postoperative bleeding events with PG. Perioperative morbidity and mortality of pancreatoduodenectomy seem to be underestimated, even in the high-volume center setting.

Contemporary antiplatelet treatment in acute coronary syndrome patients undergoing percutaneous coronary intervention: One‐year outcomes from the GReek AntiPlatelet Registry (GRAPE)
ConclusionsIn real‐word ACS/PCI patients, prasugrel offered better antiischemic protection than clopidogrel, while use of both novel agents is accompanied by more frequent bleeding events. This article is protected by copyright. All rights reserved.
