Fatal Staphylococcus Aureus Endocarditis Misdiagnosed as Multisystem Inflammatory Syndrome in Children
We present a case of a 10-year-old male with Staphylococcus aureus mitral endocarditis who was initially misdiagnosed with multisystem inflammatory syndrome associated with coronavirus disease 2019, with eventual fatal outcome due to brain hemorrhage after cardiac intervention. Our case differs from recent studies, in which microbleeds did not increase the risk of hemorrhagic stroke or global mortality risk.

Lactobacillus Endocarditis Complicated by Presumed Vertebral Osteomyelitis and Embolic Stroke

Silent but Deadly: Legionella maceachernii: Causing Culture-Negative Bioprosthetic Mitral Valve Endocarditis Complicated by Stroke
We report the first case of bioprosthetic valve endocarditis due to Legionella maceachernii. This case highlights the importance of the use of DNA target sequencing to identify the cause of culture-negative endocarditis, as standard empiric therapy would not have covered the identified organism.

A Complicated Case of Haemophilus influenzae Endocarditis in an Adult
Bacterial causes for infective endocarditis have changed over the years, with Staphylococcus aureus now being the most common pathogen isolated. With the advent of routine childhood vaccinations, invasive infections caused by Haemophilus influenzae have become rare. We wish to report a case of infective endocarditis caused by H. influenzae that was complicated by meningitis, septic arthritis, and stroke.

ConclusionsStroke or stroke-like syndrome of infectious origin can be observed in an important proportion of case presenting with sensory-motor deficit of unknown origin; their accurate diagnosis has a considerable impact in terms of treatment choices and outcome.

ConclusionsLSIE in PWID is not uncommon. Compared to non-LSIE in PWID, valve surgery is more common and mortality is higher. For reasons that are unclear, stroke is more frequent in LSIE in PWID than in non-PWID with LSIE but mortality is no different.

A classic and fatal case of Streptococcus mutans subacute bacterial endocarditis; A now potentially underappreciated disease
We describe a patient found to have Streptococcus mutans subacute bacterial endocarditis, who presented with classic risk factors and findings, and who died shortly after presentation due to hemorrhagic conversion of an embolic stroke in the setting of anticoagulation. It is critical that all cases of streptococcal bacteremia be appropriately evaluated and treated, and that Streptococcus spp. not ever be routinely considered a blood culture ‘contaminant’.

Fatal Neisseria macacae infective endocarditis: first report
We present here the first case ofN. macacae infective endocarditis in a 65-year-old man with a native aortic valve infection complicated by a peri-aortic abscess. N. macacae was isolated from blood culture and was found on the cardiac valve using 16S rDNA detection. Despite an appropriate antibiotic therapy, and aortic homograft replacement, and mitral repair, the patient died 4  days after surgery from a massive hemorrhagic stroke.

Native Valve Emphysematous Endocarditis Caused by Finegoldia magna in a Novel Pathogenic Role
We describe a case of native valve emphysematous endocarditis presenting with stroke and ultimately leading to left ventricular rupture and the patient's untimely death. Premortem blood cultures and postmortem pathologic and histologic examination yielded Finegoldia magna (formerly Peptostreptococcus magnus) in a novel pathogenic role. As detection methods continue to improve for Finegoldia magna, it is important to increase awareness of the pathogenic role of this organism.

Profile of infective endocarditis observed from 2003 - 2010 in a single center in Italy
Conclusion: S. aureus emerged as the leading causative organism of IE in a University hospital in northern Italy. Our study confirmed the high in-hospital mortality of IE, particularly if health-care associated, and the protective role of surgery.
