Current Evidence-based Approaches to Multimodal Pain Control and Opioid Minimization After Arthroscopic and Knee Preservation Surgery
J Am Acad Orthop Surg. 2023 Aug 21. doi: 10.5435/JAAOS-D-23-00342. Online ahead of print.ABSTRACTNonarthroplasty knee procedures are common and may cause a wide spectrum of postoperative pain, ranging from minimal to severe, depending on the patient, pathology, and procedure. Procedures include ligament repair and reconstruction, especially anterior cruciate ligament reconstruction, meniscal débridement, repair and transplant, periarticular osteotomy, and cartilage restoration. Multimodal analgesia regimens have been implemented successfully, but notable variation characterizes current protocols. Increased public and phys...

The influence of pain-related comorbidities on pain intensity and pain-related psychological distress in patients presenting with musculoskeletal pain
CONCLUSIONS: Depression was associated with greater pain intensity. No comorbidities were able to account for the extent of pain-related psychological distress.PMID:37611373 | DOI:10.1016/j.bjpt.2023.100532

Virtual reality for the treatment of postoperative pain after laparoscopic hysterectomy for benign Indications: A randomized controlled trial
CONCLUSION: Although virtual reality use following hysterectomy did not improve pain scores or decrease narcotic usage, it was well received by patients.PMID:37611537 | DOI:10.1016/j.ejogrb.2023.08.012

Patient Experience of Lichen Planus: A Qualitative Exploration of Signs, Symptoms, and Health-Related Quality of Life Impacts
ConclusionThe findings contribute to the literature by providing qualitative insights into signs/symptoms and HRQoL impacts of LP, from the adult patient perspective. The findings also highlight the importance of considering assessment of HRQoL impacts in future clinical LP research, particularly impacts on emotional wellbeing when selecting instruments for assessment of HRQoL in the CLP population.Trial RegistrationNCT04300296.

Variation in Methamphetamine-Associated Heart Failure Hospitalizations Across the United States
Methamphetamine is a potent, highly addictive psychostimulant. Methamphetamine use (MU) is associated with cardiovascular complications including heart failure (HF).1 Increasing MU associated HF-hospitalizations (MU-HFH) in the United-States (US) have recently been reported with large geographic variation .2 This variation may be related to differences in actual population MU rates, but it could also reflect toxicology testing patterns. Urine drug screening (UDS) varies greatly for patients presenting with chest pain, with higher testing rates for Black persons3 though these data are sparce and selective.

The role of the gut microbiome and microbial metabolism in mediating opioid-induced changes in the epigenome
The current opioid pandemic is a major public health crisis in the United States, affecting millions of people and imposing significant health and socioeconomic burdens. Preclinical and clinical research over the past few decades has delineated certain molecular mechanisms and identified various genetic, epigenetic, and environmental factors responsible for the pathophysiology and comorbidities associated with opioid use. Opioid use-induced epigenetic modifications have been identified as one of the important factors that mediate genetic changes in brain regions that control reward and drug-seeking behavior and are also im...
