Challenges in implementation of lung cancer screening —radiology requirements
SummaryImplementation of lung cancer screening is a  challenge for radiology. The present short review summarizes major requirements which are needed to ensure high rates of diagnostic accuracy and appropriate protocols for follow-up and invasive work-up. Standardization of procedures, structured reporting, assessment of quality metrics, close multi disciplinary collaboration, and continuous education are essential. Additional quantification of coronary artery calcium and emphysema may provide synergistic medical benefits for a lung cancer screening programme.

How Much Breast Milk Does a Lactating Woman Make?
Discussion Each person needs to eat the right about of healthy foods to maintain a healthy body. This varies based on gender, age, activity, healthy problems, or health status. Healthy women who are moderately active and not pregnant or lactating need about 1800-2000 calories/day. Pregnant women are advised to keep the calories the same in the first trimester (about 1800 calories/day), increase ~200-300 (about 2200 calories/day)in the second trimester, and then increase ~300-400 in third trimester (about 2400 calories/day). The increase in calories should be met by a normally balanced and varied diet. When pregnant or br...

What Is In A Ketogenic Diet?
Discussion Epilepsy is a common problem for the general pediatrician. Its incidence is estimated at 41-87/100,000 children. While many children are controlled with medication, it is also estimated that up to 1/3 will develop drug-resistant epilepsy. Some children may have an identifiable seizure focus that may be amenable to surgery, but many others do not. One option for potential control is a ketogenic diet (KD). Indications for KD usually are for drug-resistant epilepsy including partial and complex seizure patterns as well as some metabolic disorders such as glucose transport 1 deficiency syndrome. It has also found ...

Metformin for diabetes prevention: Update of the evidence base
Curr Med Res Opin. 2021 Jul 20:1. doi: 10.1080/03007995.2021.1955667. Online ahead of print.ABSTRACTWe have conducted a narrative review based on a structures search strategy, focussing on the effects of metformin on the progression of non-diabetic hyperglycaemia to clinical type 2 diabetes mellitus. The principal trials that demonstrated a significantly lower incidence of diabetes in at-risk populations randomised to metformin (mostly with impaired glucose tolerance [IGT]) were published mainly from 1999 to 2012. Metformin reduced the 3-year risk of diabetes by -31% in the randomised phase of the Diabetes Prevention Progr...

Pathophysiology and management of Akathisia 70 years after the introduction of the chlorpromazine, the first antipsychotic
CONCLUSIONS: Pharmacological management may pose a challenge in chronic akathisia. Rotation between different pharmacological management strategies may be optimal in resistant cases. Discontinuation of the causative drug and use of b-blockers, mirtazapine, benzodiazepines or gabapentinoids for symptomatic relief is the basis of management.PMID:34337722 | DOI:10.26355/eurrev_202101_26386

A neonate with late-onset hypocalcemia due to unrecognized maternal hyperparathyroidism and a systematic overview of similar cases
Conclusion: This is one of the few cases in the literature where neonatal symptomatology led to the diagnosis of undiagnosed maternal hyperparathyroidism. Early detection and appropriate management of neonatal hypocalcemia could eliminate serious maternal and fetal morbidity.PMID:34539299 | PMC:PMC8422796 | DOI:10.3205/000296
