A Multicenter Comparison of Open Versus Arthroscopic Fixation for Pediatric Tibial Spine Fractures
Conclusion: This study demonstrated no difference in outcomes or nonunion following ARIF or ORIF, with a significantly higher rate of concomitant injuries identified in patients treated with ARIF. The majority of identified concomitant injuries were treated with surgical intervention. Extensive surgical evaluation or pretreatment magnetic resonance imaging should be considered in the workup of tibial spine fractures to increase concomitant injury identification. Level of Evidence: Level III.

Philips debuts Intellispace intensive care support system
Royal Philips (NYSE:PHG) said today it launched its IntelliSpace Console Critical Care cloud-based clinical decision support dashboard designed to support enhanced workflows intensive care units. The product will be released at the 2016 HIMSS annual conference in Las Vegas. The newly-released dashboard is designed to allow clinicians to access patient data across multiple systems and EMRs with aims of helping streamline processes by prioritizing and analyzing patient data. “As health systems continue to move toward value-based care while at the same time facing an explosion of patient data, solutions such as Intell...

Point-of-Care Ultrasound to Diagnose Pyomyositis in a Child
We report a case of a patient presenting with fever, right lower leg swelling, and pain who was found to have a fluid collection between muscle planes noted on point-of-care ultrasound. Point-of-care ultrasound raised the clinician's concern for deep musculoskeletal infection, leading to prompt initiation of antibiotics and magnetic resonance imaging.

Decreased renal perfusion during acute kidney injury in critical COVID-19 assessed by magnetic resonance imaging: a prospective case control study
Renal hypoperfusion has been suggested to contribute to the development of acute kidney injury (AKI) in critical COVID-19. However, limited data exist to support this. We aim to investigate the differences in ...

Point-of-Care Ultrasound Diagnosis of Retinoblastoma in the Emergency Department
An 18-month-old girl presented to the emergency department with the chief complaint of squinting. The right eye demonstrated esotropia, heterochromia, and anisocoria. Ocular point-of-care ultrasound facilitated the rapid diagnosis of retinoblastoma, which was confirmed by computed tomography scan and orbital magnetic resonance imaging.

Clinically integrated ultrasound for decreasing the number of chest x-rays in the intensive care unit: It is high time to move forward a “global” use of ultrasound
This article shows that US is emerging as an effective tool allowing intensivists to correctly schedule advanced imaging being advantageous especially in terms of radiation exposure, and it gives us a unique opportunity to contribute to the ongoing discussion about different chest x-rays (CXRs) strategies in ICU [1].

Measuring Stroke Volume: Impedance Cardiography vs Phase-Contrast Magnetic Resonance Imaging.
CONCLUSIONS: Impedance cardiography had low accuracy and precision in measuring absolute stroke volume and was a poor detector of stroke-volume trends. PMID: 28864438 [PubMed - in process]

Sonographic diagnostics in operative intensive care medicine : Pericardial hematoma after cardiac surgery.
Abstract These two case reports describe the use of transthoracic echocardiography in cardiac surgery patients during postoperative intensive care, when a pericardial hematoma developed. A focused echocardiographic examination was performed, which in both cases led to the correct diagnosis and revealed the cause for hemodynamic instability. Following additional computed tomography (CT) scans, cardiac surgery was performed on one patient, while in the other, bedside sonography was used for controlled pleural puncture and drainage of the pericardial hematoma. The case reports demonstrate that intrathoracic bleeding...

Prognostication in comatose survivors of cardiac arrest: An advisory statement from the European Resuscitation Council and the European Society of Intensive Care Medicine
Abstract Objectives To review and update the evidence on predictors of poor outcome (death, persistent vegetative state or severe neurological disability) in adult comatose survivors of cardiac arrest, either treated or not treated with controlled temperature, to identify knowledge gaps and to suggest a reliable prognostication strategy. Methods GRADE-based systematic review followed by expert consensus achieved using Web-based Delphi methodology, conference calls and face-to-face meetings. Predictors based on clinical examination, electrophysiology, bio...

Point-Of-Care Ultrasound Diagnosis of Medial Plantar Artery Pseudoaneurysm Secondary to Penetrating Injury
Pseudoaneurysms of the foot are rare and can occur from a range of etiologies, including laceration from a foreign body. The majority of reported cases have been diagnosed by computed tomography, magnetic resonance imaging, or angiography. These tests require intravenous access and contrast, confer radiation, take time to perform and interpret, are expensive, and are not always readily available in the acute setting. No prior reported pseudoaneurysms of the foot have been diagnosed by point-of-care ultrasound (POCUS).

Ketogenic Diet as a Treatment for Super-Refractory Status Epilepticus in Febrile Infection-Related Epilepsy Syndrome
Conclusions: The characteristic EEG pattern in the acute phase promoted timely diagnosis of FIRES. Our data suggest that KD may be a safe and promising therapy for FIRES with SRSE, and that early initiation of KD produces a favorable prognosis. Therefore, KD should be applied earlier in the course of FIRES. Intravenous KD can be an effective alternative route of administration for patients who may not take KD enterally. Introduction Febrile infection-related epilepsy syndrome (FIRES) is a rare epileptic encephalopathy of unknown etiology which occurs in patients without active epilepsy or underlying neurological diso...

Applications of pet/mri in pediatric patients
Conclusion: Simultaneous PET/MRI is an emerging hybrid modality that offers an excellent combination of anatomic, functional, and metabolic imaging. It has been increasingly used in the pediatric population and has a significant advantage by reducing ionizing radiation. The simultaneous acquisition of both PET and MRI in one session reduces the number of sedation and misregistration between modalities. The high cost and low availability of this combined examination remain the major obstacles that prevent its more widespread use.
