Effects of preoperative statin use on perioperative outcomes of carotid endarterectomy
ConclusionsTaking statins prior to CEA did not seem to affect the risk of major perioperative ischemic events and death, most likely due to the extremely low overall incidence of perioperative complications. Recent reports have shown that the preoperative use of statins has a protective role after noncardiac vascular and cardiac surgery, attenuating the perioperative incidence of stroke, myocardial infarction, and death. Only few studies, however, have investigated on the benefit of statin use in patients undergoing carotid endarterectomy for severe carotid disease, reporting variable results. Findings from our study show...

Moyamoya Vasculopathy in a Patient with Scleroderma: A Case Report and Review of the Literature (P3.273)
Conclusions:MMS in the context of scleroderma is a very rare entity. The angiographic findings are result of a systemic process at the cellular level, independent of atheromatous changes. They may represent progressive vasculopathy different from the classically described Suzuki stages for moyamoya disease.Disclosure: Dr. Ahmad has nothing to disclose. High Rate of Stenosis Progression Identified with Routine Carotid Doppler Surveillance (P5.284)
Conclusions:Among this prospective cohort of asymptomatic ≥50% carotid stenosis subjects followed with carotid Doppler imaging, high rates of carotid stenosis progression occurred despite contemporary intensive medical management, and significant TIA/stroke rates were also noted, especially among those with ≥80% stenosis. These results support further study of routine carotid Doppler surveillance in management of asymptomatic carotid stenosis.Study Supported by:Grants from the American Heart Association (15GRNT25270010), NIH (1 U54GM104938) and the VA Clinical Science Research & Development Service (CX000340)Disc...

The beneficial effects of HMG-CoA reductase inhibitors in the processes of neurodegeneration
AbstractStatins, cholesterol lowering drugs, have been demonstrated to exert beneficial effects in other conditions such as primary and progressing neurodegenerative diseases beyond their original role. Observation that statins ameliorate the neurodegenerative diseases such as Parkinson ’s disease (PD), Alzheimer’s disease (AD), multiple sclerosis (MS) and cerebral ischemic stroke, the neuroprotective effects of these drugs are thought to be linked to their anti-inflammatory, anti-oxidative, and anti-excitotoxic properties. Despite the voluminous literature on the clinical adva ntages of 3-hydroxy-3-methylglutaryl Co-e...

Undertreatment of Vascular Risk Factors in Patients with Monocular Ischaemic Visual Loss
Conclusions: Approximately one-third of patients with ocular ischaemia had ≥3 vascular risk factors with recurrences higher in these patients. Yet only half of those with previous ischaemic ocular events were on antiplatelets or statins. These patients should be investigated and treated as aggressively as other forms of TIA or stroke.Cerebrovasc Dis 2018;45:228 –235

Thrombocytopenia and declines in platelet counts: predictors of mortality and outcome after mechanical thrombectomy
ConclusionIn AIS patients treated with MT an initial TP was independently associated with higher mortality rates and a marked DPC with higher mortality rates as well as poorer clinical outcomes.

Use of the Cardiovascular Polypill in Secondary Prevention of Cerebrovascular Disease: A Real-Life Tertiary Hospital Cohort Study of 104 Patients
Conclusion: In our experience, the cardiovascular polypill achieved a higher reduction in SBP levels and was well tolerated. Adherence was similar to that found in the previous literature, which is remarkable given the real-life setting of our study.Cerebrovasc Dis Extra 2020;10:166 –173

Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL) – Still to be Considered in the Presence of Vascular Risk Factors
We report a 46-year-old male with a 9 and 3-month history of progressive unilateral lower limb weakness and dysarthria, respectively. He had a history of diabetes mellitus but no hypertension, hyperlipidemia, or smoking history. Both parents had a stroke at the age of 65 years. Neurological examination was significant for moderate dysarthria and reduced right upper limb dexterity. Magnetic resonance imaging (MRI) of the brain revealed extensive white matter disease, lacunar infarcts, and a few microhemorrhages. Electron microscopy of his skin biopsy showed electron-dense deposits of extracellular osmiophilic granular mater...

Impact of statin pretreatment on the complications of carotid stenting in asymptomatic patients: observational study
Carotid stenosis is a known risk factor for ischemic stroke, and carotid artery stenting is an effective preventive procedure. However, the stroke risk reduction for asymptomatic patients is small. Therefore, ...

Association Between 18-FDG Positron Emission Tomography and MRI Biomarkers of Plaque Vulnerability in Patients With Symptomatic Carotid Stenosis
Conclusion: This study demonstrated novel correlations of non-invasive imaging biomarkers of inflammation-related plaque metabolism with morphological MRI markers of plaque instability. If replicated, our findings may support the application of combined MRI and PET to detect vulnerable plaque in future clinical practise and randomised trials.

The effect of intensive statin therapy in non-symptomatic intracranial arteries: The STAMINA-MRI sub-study
ConclusionIntensive statin therapy produced a favorable morphological change in cerebral arteries of not only the target arterial segment but also non-symptomatic arterial segments. The change in cerebral arterial luminal diameter was influenced by the baseline SBP and was dependent on the topographic distribution of the cerebral arteries.Clinical Trial Registration: ClinicalTrials.gov, identifier NCT02458755.

Cerebral hemorrhages in CADASIL: Report of four cases and a brief review
Conclusions: Hemorrhagic stroke can occur in CADASIL similarly to sporadic cerebral small vessel diseases; this finding expands the phenotype of the disease. A diagnosis of CADASIL should probably be considered also in patients with ICH. These data bear potential implications in terms of need of better control of risk factors, particularly hypertension, and raise relevant questions about the use of antiplatelets as prevention measures in CADASIL patients.
