The BDNF polymorphism Val66Met may be predictive of swallowing improvement post pharyngeal electrical stimulation in dysphagic stroke patients
Conclusions & InferencesOur findings suggest an association between BDNF and stimulation induced swallowing recovery. Further work will be required to validate these observations and demonstrate clinical utility in patients. The BDNF polymorphism Val66Met may be predictive of swallowing improvement post pharyngeal electrical stimulation in dysphagic stroke patients.

Biomechanical changes in the pharynx and upper esophageal sphincter after modified balloon dilatation in brainstem stroke patients with dysphagia
Conclusions & InferencesDysphagia therapy with modified dilatation improved UES relaxation, strengthened pharyngeal propulsion, restored UES resting pressure and improved functional oral intake to a greater extent than regular therapy alone. Using high‐resolution manometry (HRM), we examined the biomechanical properties of swallowing in brainstem stroke patients with dysphagia following modified balloon dilation therapy. Dysphagia therapy with modified dilatation improved UES relaxation, strengthened pharyngeal propulsion, restored UES resting pressure and improved functional oral intake to a greater extent than regular therapy alone.

The prognostic significance of troponin I elevation in acute ischemic stroke
Conclusion Positive TnI greater than or equal to 0.5 μg/L in patients with acute ischemic stroke was independently associated with worse outcomes. Patients with diagnosis of MI represent a particularly high-risk subgroup.

Troponin elevation predicts critical care needs and in-hospital mortality after thrombolysis in white but not black stroke patients
Conclusion Troponin is a useful predictor of poor outcome in white but not black post-IVT stroke patients.

Failed Deglutitive Upper Esophageal Sphincter Relaxation Is a Risk Factor for Aspiration in Stroke Patients with Oropharyngeal Dysphagia.
Conclusions: Manometric measurements of the pharynx and UES were well correlated with abnormal findings in the VFSS and a shorter relaxation time interval of the UES during deglutition is an important parameter for the development of aspiration. PMID: 27510474 [PubMed - as supplied by publisher]

Association Between Proton Pump Inhibitor Use and Risk of  Stroke
We read with interest the article by Nguyen et  al1 evaluating the association between proton pump inhibitor (PPI) use and ischemic stroke. The authors found that PPI use was associated with a significant increase in the risk of ischemic stroke after adjustment for established risk factors for stroke including smoking status, body mass index, p hysical activity, dietary quality, alcohol use, menopausal hormone use in women, multivitamin use, regular aspirin use, regular nonaspirin nonsteroidal anti-inflammatory drug, use and personal history of hypertension, hyperlipidemia, coronary artery disease, or diabetes.

Stroke Volume Variation for the Evaluation of Circulating Blood Volume after Living Donor Liver Transplantation.
CONCLUSION: Postoperative fluid management using SVV may be especially useful after LDLT. Monitoring the circulating blood volume using a 10% SVV index is useful for avoiding lung edema after LT. PMID: 26897956 [PubMed - in process]

Gut microbiota: Gut bacteria affect post-ischaemic inflammation in stroke by modulating intestinal T cells
Nature Reviews Gastroenterology & Hepatology 13, 250 (2016). doi:10.1038/nrgastro.2016.64 Author: Charlotte Ridler Acute brain injury in stroke is modulated by commensal gut bacteria, according to new research. Benakis et al. found that antibiotic treatment in mouse models of stroke substantially reduced infarct volume in these animals compared with control animals. This protective effect was attributed

Ischemic Colitis Associated with Rhabdomyolysis and Heat Stroke after an Intense Exercise in Young Adult.
Abstract Ischemic colitis primarily affects the elderly with underlying disease, but it rarely occurs in young adults with risk factors, such as coagulopathy or vascular disorder. Moreover, it is extremely rare in the very young without risk factors. This paper presents a patient with ischemic colitis associated with heat stroke and rhabdomyolysis after intense exercise under high-temperature conditions. A 20-year-old man presented with mental deterioration after a vigorous soccer game for more than 30 minutes in sweltering weather. He also presented with hematochezia with abdominal pain. The laboratory tests reve...

The Correlation Between Manometric and Videofluoroscopic Measurements of the Swallowing Function in Brainstem Stroke Patients With Dysphagia
The objective of the current study was to examine the correlation between high-resolution manometric and videofluoroscopic measurements of the swallowing function. Background:In such cases in which manometric analysis is not feasible, it would be ideal if information regarding pressure during swallowing could be inferred from a videofluoroscopic swallowing study (VFSS) evaluation alone. Study:The swallowing function was examined using simultaneous VFSS and high-resolution manometry (HRM) in 24 brainstem stroke survivors with dysphagia. The pharyngeal constriction ratio (PCR) and upper esophageal sphincter (UES) maximum ope...
