Tangier disease in Israel; Complicated by mobile lipomatous mass on mitral valve and acute stroke
Background and Aims: Tangier disease (TD) is a rare genetic disease which causes very low or even deficient HDL in plasma and accumulation of cholesterol in different tissues throughout the body. The disease is related to early onset of atherosclerotic disease and other clinical sequelae, previous articles described increased incidence of stroke in tangier patients. Valvular involvement in Tangier's disease was never described before .

Guideline-Directed Low-Density Lipoprotein Cholesterol Management After Acute Ischemic Stroke: Findings from a National Health Care Service
Patients with ischemic stroke are at high risk for future cardiovascular events and should be treated intensively with lipid-modifying agents. Combination lipid-lowering therapies are often needed to achieve updated guideline-directed treatment goals. However, real-world data on intensification of lipid-lowering therapies and attainment of low-density lipoprotein cholesterol (LDL-C) targets early after ischemic stroke are limited. We extracted data from the largest healthcare provider in Israel on patients hospitalized with acute ischemic stroke between January 2020 and February 2022.

Preventing post-stroke dementia. The MARCH Trial. Protocol and statistical analysis plan of a randomized clinical trial testing the safety and efficacy of Maraviroc in post-stroke cognitive impairment
CONCLUSIONS: The results of this work could lead to a novel, readily available, therapeutic avenue to reduce PSCI, and possibly other pathologies. This study will test safety and effectiveness of Maraviroc in limiting cognitive deterioration and/or post stroke cognitive impairment in patients with cerebral small vessel disease.SCHEDULE: First-patient first-visit was May 2021. Recruitment to complete in 2023, follow-up to complete in 2024.PMID:36082248 | PMC:PMC9446318 | DOI:10.1177/23969873221098857

Novel bleeding prediction model in atrial fibrillation patients on new oral anticoagulants
Conclusions We present a novel and simple risk score for prediction of major bleeding in patients with non-valvular AF treated with NOACs. Validation in additional cohorts is warranted.

Association of Polycythemia with Outcomes of Acute Coronary Syndrome
Conclusions: Although younger and with fewer comorbidities, polycythemic ACS patients are at increased risk for long-term all-cause mortality. Further study of this association is warranted to understand the causes and possibly to improve the outcomes of these patients.Cardiology

Blood pressure variability at midlife is associated with all-cause, coronary heart disease and stroke long term mortality
Conclusion: In this cohort of tenured male workers, BPV taken over 5 years was clearly associated with 18-year all-cause, CHD and stroke mortality.

Low serum potassium levels are associated with the risk of atrial fibrillation.
CONCLUSION: We found that low serum potassium levels of less than 3.5 mmol/l are associted with increased risk of AF. PMID: 32723154 [PubMed - as supplied by publisher]

Assessing Patent Ductus Arteriosus (PDA) Significance on Cardiac Output by Whole-Body Bio-impedance
AbstractWe evaluated the effectiveness of a whole-body bioimpedance device (NICaS ®, NI Medical, Petach Tikva, Israel) to predict the presence of a hemodynamically significant patent ductus arteriosus (PDA) in premature infants. A total of 36 infants less than 35 week’s gestation age and birth weights of less than 1750 g were included in the study. Using the NICaS® device, w e obtained whole-body bioimpedance measurements of stroke volume index (SI), cardiac output index (CI) and total peripheral resistance index. A total of 61 measurements were taken together with echocardiograph imaging. The study population was div...

Use of oral anticoagulation therapy in the first 3 months after the diagnosis of atrial fibrillation in Israel: a population ‐based study
ConclusionIn this real world, population ‐based cohort of patients with newly diagnosed AF, we found a lower than expected rate of OAC prescription within 3 months of diagnosis but an encouraging increase in OAC purchasing over time. Use of NOACs has risen exponentially in only a few years, accounting for a greater pool of AF patients be ing prescribed an OAC.This article is protected by copyright. All rights reserved.

Comparison of Stroke Risk Stratification Scores for Atrial Fibrillation
Several stroke risk stratification scores have been developed to guide clinical decision making in patients with non valvular atrial fibrillation (AF). The aim of this study was to compare the performance of the CHADS2, CHA2DS2-VASc and R2CHADS2 risk scores to predict stroke. This retrospective cohort study was based on electronic medical records from Clalit Health Services (CHS), the largest payer provider healthcare organization in Israel. Data of CHS members with AF diagnosis between 2004- 2015 were extracted.

XANTUS-EL: A real-world, prospective, observational study of patients treated with rivaroxaban for stroke prevention in atrial fibrillation in Eastern Europe, Middle East, Africa and Latin America
ConclusionsXANTUS-EL confirmed low stroke and major bleeding rates in patients with NVAF from EEMEA and Latin America. The population was younger but with more heart failure and hypertension than XANTUS; stroke/SE rate was similar but major bleeding lower.

Temporal-Trends in Sex-Related Differences and Outcomes in Patients Undergoing Transcatheter Aortic Valve Implantation (From the Israeli TAVI Multicentre Registry)
We evaluated temporal trends in sex-related differences in patients undergoing trans-catheter aortic valve implantation (TAVI) for severe symptomatic aortic stenosis (AS) from a multicentre TAVI registry during the years 2008-2016. Our final cohort included 1,159 males and 1,370 females, with a median follow-up of 2.3 (IQR 1.2, 4.4) years. For temporal trends analysis, the entire population was divided according to period of procedure: 2008-2012 and 2013-2016. During the 2008-2012 period, the rates of in-hospital aortic paravalvular leak, myocardial infarction, pacemaker implantation, and stroke were higher among males tha...
