Evaluation of serum levels of homocysteine and C-reactive protein in patients with ischemic stroke
Cell Mol Biol (Noisy-le-grand). 2022 Jul 31;68(7):56-62. doi: 10.14715/cmb/2022.68.7.10.ABSTRACTStroke is the third most common cause of death in the world. Plasma homocysteine is an emerging risk factor for stroke. Also, previous studies have shown that serum levels of C-reactive protein (CRP) are significantly associated with mortality and function in patients with ischemic stroke. In the present study, we examined homocysteine and serum CRP levels in patients with ischemic stroke. This descriptive-analytical study was performed on 100 patients with ischemic stroke. Blood samples were taken in the first 24 hours of a str...

Rehabilitation in Geriatric Patients after Ischemic Stroke - A Comparison of 2 Organisational Systems in Germany Using Claims Data of a Statutory Health Insurance Fund.
Abstract Due to historical aspects in some federal states in Germany rehabilitation of geriatric patients is organized in geriatric departments in hospitals (§ 109 SGB V). In other states rehabilitation of these patients is mainly realized in geriatric rehabilitation facilities outside hospital care after approval by the health insurance (§ 111 SGB V). Thus, it is of interest to compare both types of health care with respect to differences in population characteristics, resource utilization and outcome parameters (i. e., excess costs, rehospitalization, fracture risk and mortality) using a common geriatric indic...

Guideline conform diagnostics for dysphagia : A  representative survey of speech therapists at certified stroke units in Germany.
CONCLUSION: The implementation of a detailed and differentiated anamnesis, standardized screening, and a clinical swallowing examination with testing of different food consistencies suggests a high quality of the dysphagia diagnostics at stroke units in Germany. The increasing availability of technical instruments, especially fiber endoscopy, substantiates this view. PMID: 28382486 [PubMed - as supplied by publisher]

Incidence of myocardial infarction and stroke in acromegaly patients: results from the German Acromegaly Registry
ConclusionsFor acromegaly patients being treated at specialized centres, the incidence of MIs and strokes does not seem to differ from the general population. Certainty regarding such statements requires large, prospective studies however.

Treatment goals in patients with post-stroke upper limb spasticity following injection of botulinum toxin  A : Results of the German-Austrian subgroup of the ULIS-II study.
CONCLUSION: A single injection of BoNT-A in adult patients with post-stroke spasticity of the arm led to a high response rate of approximately 80% in both cohorts. The BoNT-A injections in post-stroke adult patients contributed to an improvement in the daily life of patients and their carers beyond simple reduction of muscle tone or spasticity. Association of serum markers of oxidative stress with myocardial infarction and stroke: pooled results from four large European cohort studies
AbstractOxidative stress contributes to endothelial dysfunction and is involved in the pathogenesis of myocardial infarction (MI) and stroke. However, associations of biomarkers of oxidative stress with MI and stroke have not yet been addressed in large cohort studies. A nested case –control design was applied in four population-based cohort studies from Germany, Czech Republic, Poland and Lithuania. Derivatives of reactive oxygen metabolites (d-ROMs) levels, as a proxy for the reactive oxygen species burden, and total thiol levels (TTL), as a proxy for the reductive capacity , were measured in baseline serum samples of ...

Podcast: Electromechanical-assisted training for improving arm function and disability after stroke
A common consequence for people who have a stroke is a reduction in their arm function. Various approaches are available to try to help and, in September 2018, the Cochrane Review for one of these, electromechanical and robot ‐assisted arm training, was updated by a team of researchers from Germany. We asked lead author, Jan Mehrholz from Dresden Medical School, to tell us about the latest findings in this podcast." More than two ‐thirds of people who have had a stroke have difficulties with reduced arm function. This can limit their ability to perform everyday activities and electromechanical and robot‐assisted arm ...

Peptidase neurolysin functions to preserve the brain after ischemic stroke in male mice
This study is the first to document functional significance of Nln in pathophysiology of stroke and provide evidence that Nln is an endogenous mechanism functioning to preserve the brain from ischemic injury. This article is protected by copyright. All rights reserved.

Simulation-Based Training of the Rapid Evaluation and Management of Acute Stroke (STREAM) —A Prospective Single-Arm Multicenter Trial
Introduction: Acute stroke care delivered by interdisciplinary teams is time-sensitive. Simulation-based team training is a promising tool to improve team performance in medical operations. It has the potential to improve process times, team communication, patient safety, and staff satisfaction. We aim to assess whether a multi-level approach consisting of a stringent workflow revision based on peer-to-peer review and 2–3 one-day in situ simulation trainings can improve acute stroke care processing times in high volume neurocenters within a 6 months period. Methods and Analysis: The trial is being carried out in a pre-tes...

Quality of care of acute ischemic stroke in Germany 2018.
Abstract In this overview the current quality of acute in-hospital care of stroke patients in Germany in 2018 is described based on standardized and evidence-based quality indicators. For this purpose the reports of the regional quality assurance projects for stroke care, which collaborated within the German-speaking Stroke Registers Study Group (ADSR) were analyzed. Overall, more than 280,000 acute admissions of stroke patients were documented in the included quality assurance projects. The results regarding the defined 16 quality indicators comprising diagnostics, acute treatment, rehabilitation and secondary pr...

A review of mobile stroke units
AbstractPurpose of reviewMobile stroke units (MSUs) for prehospital treatment and management of patients with acute stroke have been developed more than a decade ago and is currently spreading worldwide. This review discusses the history of MSU and current operations and research.Recent findingsMultiple studies have shown that MSU can significantly reduce treatment time with a tenfold increase of patients treated within the first 60  min of symptom onset. Recent preliminary results from the Berlin Prehospital or Usual Delivery of Acute Stroke Care trial (B-PROUD) showed a positive shift in modified Rankin Scale (mRS) scor...

Modeling the Optimal Transportation for Acute Stroke Treatment
ConclusionOur study showed a  considerable impact of the diurnal variations in traffic rate and direction of travel on optimal stroke transportation. Stroke systems of care should take real time traffic information into account.

Mobile Stroke Units: Current and Future Impact on Stroke Care
Semin Neurol DOI: 10.1055/s-0040-1722724Ischemic stroke is a leading cause of death and major disability that impacts societies across the world. Earlier thrombolysis of blocked arteries with intravenous tissue plasminogen activator (tPA) and/or endovascular clot extraction is associated with better clinical outcomes. Mobile stroke units (MSU) can deliver faster tPA treatment and rapidly transport stroke patients to centers with endovascular capabilities. Initial MSU trials in Germany indicated more rapid tPA treatment times using MSUs compared with standard emergency room treatment, a higher proportion of patients treated...

Regional Deprivation, Stroke Incidence, and Stroke Care-An Analysis of Billing and Quality Assurance Data From the German State of Rhineland-Palatinate
CONCLUSION: These findings document a relationship between regional deprivation and the occurrence of acute ischemic stroke. Poorer GIMD 2010 scores were associated with worse care outcomes in a number of variables, but the absolute differences were small.PMID:33663687 | DOI:10.3238/arztebl.m2021.0149
