Continuation of lithium after a diagnosis of chronic kidney disease
ConclusionAfter an initial diagnosis of CKD, patients who are selected by their physicians to continue lithium treatment may not necessarily have an increased risk of developing end‐stage CKD. Shifting to an anticonvulsant per se may not be associated with an advantage; however, this requires further investigation.

Hypothyroidism as a Predictor of Surgical Outcomes in the Elderly
Conclusions It is recommended to post-pone elective surgery in elderly patients with hypothyroidism until an euthyroid state is achieved. If patients need urgent or emergent surgery, it is recommended to proceed with surgery only if they have mild or moderate hypothyroidism. Replacement therapy should be started preoperatively and there should be growing attention to the possible occurrence of minor post-operative complications in hypothyroid patients. As suggested by the American Thyroid Association (ATA), the treatment in elderly patients should be initiated at low doses with slow titration based on serum TSH evaluation...

Integrative Analysis of DiseaseLand Omics Database for Disease Signatures and Treatments: A Bipolar Case Study
We presented here an integrative analysis framework of the Janssen DiseaseLand transcriptomics database, which currently includes 1,885 human and 1,460 animal (mouse or rat) studies from public and Janssen internal sources, and is constantly growing. By applying standardized re-processing to raw datasets, removing outlying samples and problematic datasets, and comparing multiple meta-analysis approaches, we were able to generate a unified gene expression signature for a disease. We demonstrated our approach on a meta-analysis of 30 gene expression datasets from human brain tissues in bipolar. Bipolar is a serious mental i...

Trauma hazards in children: An update for the busy clinician
Abstract Trauma and injury continue to be common in children and remain an important cause of mortality and morbidity. Legislation mandating the use of helmets for all cyclists appears to have been effective in reducing the incidence and severity of head and facial injuries, with no clear evidence of a reduction in cycling usage or activity. Straddle injuries, whilst uncommon and generally minor, require careful clinical assessment as they may be associated with urethral trauma. Quad bikes remain highly dangerous with continuing reports of deaths in child riders due to their inherent lack of stability: a ban on their use b...

Level of magnesium in patients with depression treated with lithium -- pilotage research.
CONCLUSION: Significantly higher plasma magnesium levels were observed among city than village inhabitants, there was also a relationship between type of education and magnesium level in blood plasma of the patients in the study. Further studies including larger groups of patients should be performed to enable a final conclusion. PMID: 23540223 [PubMed - in process]

The Cord Blood Insulin and Mitochondrial DNA Content Related Methylome
In conclusion, in our investigation of epigenome-wide methylation, we identified several previously described as well as novel genes and pathways on multiple levels in relation to both, cord blood insulin and mtDNA content. Possible important targets of insulin as well as mitochondrial (dys-) function during embryogenesis point in the direction of neuronal development, histone modification, CYP-metabolism, and biological aging. What Causes Hypercalcemia?
Discussion Calcium homeostasis is regulated by mechanisms involving the absorption from the gastrointestinal tract, bone deposition and resorption, and renal excretion. To review Vitamin D homeostasis click here. Serum calcium is found in three forms: free (47%), protein bound (43%) and diffusable calcium complexes (10%). The protein binding proteins are albumin (80%) and globulin (20%). The free calcium is the most important biologically. Acidosis also increases free calcium and alkalosis decreases free calcium. Problems associated with hypercalcemia include nausea, emesis, abdominal pain, constipation, polyuria, dehydra...

Portable power supply options for positive airway pressure devices.
CONCLUSIONS: There are several options to power PAP devices with direct current when alternating current is not available. Knowledgeable primary care physicians especially in rural and remote areas can help OSA patients improve PAP compliance in order to mitigate morbidity and long-term complications of OSA. Perstimulus asystole during electroconvulsive therapy: Clinical case and critical literature review.
CONCLUSIONS: The occurrence of perstimulus asystole is not considered as a serious complication of ECT and therefore as a contra-indication to any future sessions. On the contrary, most authors are campaigning for the continuation of shocks with the possibility of adding prophylactic intravenous atropine. Cardiac arrest reminds us that ECT requires a special attention to its cardiovascular effect, which emphasizes the role of interdisciplinarity between anaesthesiologists and psychiatrists. Lithium exposure during pregnancy: outcomes for women who attended a specialist antenatal clinic.
DISCUSSION: Pregnant women with severe mood disorders treated with lithium are a vulnerable, high-risk obstetric population who would benefit from preconception counseling, regular antenatal care in a tertiary center, delivery with neonatal pediatric support and experienced psychiatric management. Lithium Toxicity in Older Adults: a Systematic Review of Case Reports
ConclusionIn published case reports, the lithium dose was higher than recommended for older adults. Polypharmacy and comorbidities appeared to be important precipitating factors of lithium toxicity. Regular monitoring of serum lithium concentrations and appropriate caution should be exercised, even when serum lithium concentrations are within reported therapeutic levels.

Genome-Wide Identification of Host Genes Required for Toxicity of Bacterial Cytolethal Distending Toxin in a Yeast Model
Conclusion: The genome-wide screen in the yeast deletion library allowed us to identify a large number of host genes required for AaCdtB cytotoxicity. Further investigation could lead to more insights into the mechanisms of CdtB intoxication. Introduction Bacterial pathogens often possess several virulence factors to facilitate colonization and survival in hosts. Cytolethal distending toxin (CDT) is a genotoxin produced by many Gram-negative pathogens, such as Escherichia coli, Aggregatibacter actinomycetemcomitans, Haemophilus ducreyi, Shigella dysenteriae, Helicobacter hepaticus, and Campylobacter species (Gargi ...
