Cardiovascular efficacy and safety of dipeptidyl peptidase-4 inhibitors: A meta-analysis of cardiovascular outcome trials
CONCLUSION: DPP-4 inhibitors do not seem to confer any significant cardiovascular benefit for patients with T2DM, while they do not seem to be associated with a significant risk for any major cardiac arrhythmias, except for atrial flutter. Therefore, this drug class should not be the treatment of choice for patients with established cardiovascular disease or multiple risk factors, except for those cases when newer antidiabetics (glucagon-like peptide-1 receptor agonists and sodium-glucose co-transporter-2 inhibitors) are not tolerated, contraindicated or not affordable for the patient.PMID:34754403 | PMC:PMC8554356 | DOI:1...

Five-year-outcome of new-onset perioperative atrial fibrillation after left atrial appendage amputation concomitant with cardiac surgery
ConclusionPOAF is associated with a higher rate of the combined endpoint of all-cause mortality, stroke and rehospitalization. The composite endpoint in patients with LAA-amputation concomitant with OPCAB surgery developing new-onset POAF in a 5-year follow-up was not increased compared to a control cohort maintaining sinus rhythm.Graphical abstractFive-year outcome of patients with POAF and LAA-amputation;95% CI, 95% confidence interval,CPR, cardiopulmonary resuscitation,ECLS, extracorporeal life support,HR, hazard ratio,IABP, intra-aortic balloon pump,LAA, left atrial appendage,OPCAB, off-pump coronary artery bypass graf...

Epidemiology of arrhythmias and sudden cardiac death in Asia.
Abstract Cardiac arrhythmias are cardiac rhythm disorders that comprise an important epidemiological and public health problem. Cardiac arrhythmias are significantly associated with increased risks of cardiovascular complications and sudden death, consequently leading to decreased quality of life, disability, high mortality, and healthcare expense. Atrial fibrillation (AF) is the most common sustained arrhythmia, and has been further increasing with the aging of society. Although the prevalence is relatively lower in Asians than in Westerners, the prognostic impacts on stroke and mortality in Asians are comparable...

Clinical Spectrum of Isolated Left Ventricular Noncompaction Thromboembolic Events, Malignant Left Ventricular Arrhythmias, and Refractory Heart Failure
A 17-year-old boy presented with cardioembolic stroke. Echocardiography and cardiac magnetic resonance imaging (MRI) (A and B: arrows indicate left ventricular trabecular meshwork with deep intertrabecular spaces; Online Videos 1, 2, 3, and 4) fitted the diagnostic criteria for isolated myocardial noncompaction. MRI showed late gadolinium enhancement (LGE) of the apex with transmural distribution (C and D, arrows) and of the interventricular septum with intramyocardial distribution (C, arrowhead). An implantable cardioverter-defibrillator was implanted after sustained ventricular tachycardia. Electroanatomic voltage mappin...

Optimal Blood Pressure in Patients With Atrial Fibrillation (from the AFFIRM Trial)
Many medications used to treat atrial fibrillation (AF) also reduce blood pressure (BP). The relation between BP and mortality is unclear in patients with AF. We performed a post hoc analysis of 3,947 participants from the Atrial Fibrillation Follow-Up Investigation of Rhythm Management trial. Systolic blood pressure (SBP) and diastolic blood pressure (DBP) at baseline and follow-up were categorized by 10-mm Hg increments. The end points were all-cause mortality (ACM) and secondary outcome (combination of ACM, ventricular tachycardia and/or fibrillation, pulseless electrical activity, significant bradycardia, stroke, major...

Perioperative risk of major non-cardiac surgery in patients with severe aortic stenosis: a reappraisal in contemporary practice
Conclusion Severe aortic stenosis is associated with increased risk of MACE. In contemporary practice, perioperative mortality of patients with SAS is lower than previously reported and the difference from controls did not reach statistical significance. Emergency surgery is the strongest predictor of post-operative death. These results have implications for perioperative risk assessment and management strategies in patients with SAS.

Interruption of Pacing following Non‐sustained Ventricular Tachycardia in an AAI Programmed Implantable Cardioverter Defibrillator
Conclusions: Unintended consequences can occur when complex VT detection parameters interact with specific pacing modes. At times, non‐conventional programming can avoid these interactions while still achieving effective AAI(R) pacing. This article is protected by copyright. All rights reserved

A decade has passed since the previous survey of the epidemiological and clinical characteristics of Japanese HCM patients. The Aichi Hypertrophic Cardiomyopathy (AHC) Registry is based on a prospective multicenter observational study of HCM patients. The clinical characteristics of 42 ambulant HCM patients followed up for up to 5...

Dabigatran for left ventricular thrombus.
Abstract Male patient in dilated phase of hypertrophic cardiomyopathy had multiple hospitalizations during the past 2 years either due to congestive heart failure, stroke, scar epilepsy, or atrial fibrillation and ventricular tachycardia. Medication included evidence based therapy for heart failure, cordarone and warfarin. Anticoagulation had to be discontinued due to marked fluctuations in INR. Transthoracic Echocardiography (TTE) revealed a mobile mass in the left ventricle. He was treated with Dabigatran 110mg twice a day for 4 months without any bleeding or embolic episode and complete resolution of thrombus. ...

Ventricular Tachycardia Ablation in the Presence of Left Ventricular Thrombus: Safety and Efficacy
Conclusion: Initial data suggest ablation of VT in the presence of intracavitary thrombus is feasible, is associated with a similar success rate to historical studies in patients without thrombus, and has an acceptable risk of complications given the high risk nature of patients with ES. Further data is needed; however, the presence of a laminated thrombus should not necessarily preclude ablation in patients who have failed medical therapy for VT in whom ablation is otherwise indicated. This article is protected by copyright. Gender Discrepancy in Diabetic Patients Hospitalized With Heart Failure: Does Age Matter?
Conclusions: In DHF, female gender is characterized by having a high prevalence of metabolic syndrome components. Also, females are more likely to have better Left ventricular ejection fraction but less likely to receive cardiovascular evidence based medications. There is no significant difference in the overall hospital mortality between both genders, however, in the younger age; males have a significantly higher mortality.
