Eulogy For My Father
I thought I would get used to it. Maybe feel some callousness to it. Walking into the building and being directed to the viewing hall. Walking to the front of the room and seeing a loved one lying in a casket. I have seen a cousin, grandparents, uncles and friends within the last five years or so pass away. These past few months I have had to come to grips that I will again face death. My dad is dying! Twenty-four years ago my father suffered a near fatal stroke. The years following have been dealt with his heath declining and new issues he's faced including COPD and congestive heart failure. He has had a portable defibr...

Treatment: Special Conditions: Co-existing Heart Disease: Atrial Fibrillation
AF is the most common sustained rhythm disorder of the heart affecting approximately 6 million Americans. 1 AF occurs in 0.4% of the general population, 4% of the hospital population, and in 40% of patients with congestive heart failure (HF)2-5. A variety of clinical conditions predispose to the development of AF, the commonest being hypertension (see section ii below). AF is a major cause of serious morbidity such as cerebrovascular embolism (‘stroke’), causing a 5 fold increase in the risk of stroke (including over 10% of all strokes in the elderly).

Cause-Specific Risk of Hospital Admission Related to Extreme Heat in Older Adults
ImportanceHeat exposure is known to have a complex set of physiological effects on multiple organ systems, but current understanding of the health effects is mostly based on studies investigating a small number of prespecified health outcomes such as cardiovascular and respiratory diseases.ObjectivesTo identify possible causes of hospital admissions during extreme heat events and to estimate their risks using historical data.Design, Setting, and PopulationMatched analysis of time series data describing daily hospital admissions of Medicare enrollees (23.7 million fee-for-service beneficiaries [aged ≥65 years] per year; 8...

The New Strategy in Infective Endocarditis: Early Surgery Based on Early Diagnosis: Are We Too Late, When Early Surgery is Best?
Abstract Patients with infective endocarditis (IE) have a poor prognosis due to complications including congestive heart failure, paravalvular abscess formation, central and peripheral embolism, and hemorrhagic stroke.(1) Patients with IE and congestive heart failure (CHF) have a mortality as high as 50%.(1) The poor prognosis seems not be improved during recent years.(2) In Sweden 7.603 patients with IE received a close follow-up with assessment of the 30-day mortality and 5-year follow-up including autopsy in all fatal events sampled from 1997 to 2007.(2) During the observation period no change of mortality was ...

The value of CHADS score in predicting new-onset atrial fibrillation in Chinese patients with acute myocardial infarction
Previous studies have shown that new-onset atrial fibrillation (AF) in the setting of acute myocardial infarction (AMI) was associated with increased in-hospital and long term mortality [1,2]. Therefore, it was crucial to recognize the patients with AMI who were on the increased risk for developing new-onset AF. CHADS2 score (congestive heart failure, hypertension, age≥75years, diabetes and previous stroke or transient ischemic attack) was a very useful and convenient scoring system, which was widely used for the stroke risk stratification in the patients with non-valvular AF [3].

Dabigatran etexilate for secondary stroke prevention: the first year experience from a multicenter short-term registry.
DISCUSSION: Our pilot data indicate that dabigatran appears to be safe for secondary stroke prevention during the first year of implementation of this therapy. However, high cost may limit the long-term treatment of AF patients with dabigatran, leading to early discontinuation. PMID: 24790645 [PubMed - as supplied by publisher]

Obstructive Sleep Apnea and Risk of Cardiovascular Events and All-Cause Mortality: A Decade-Long Historical Cohort Study
The objective of this study was to evaluate the relationship between OSA-related variables and risk of CV events. Methods and Findings A historical cohort study was conducted using clinical database and health administrative data. Adults referred for suspected OSA who underwent diagnostic polysomnography at the sleep laboratory at St Michael's Hospital (Toronto, Canada) between 1994 and 2010 were followed through provincial health administrative data (Ontario, Canada) until May 2011 to examine the occurrence of a composite outcome (myocardial infarction, stroke, congestive heart failure, revascularization procedures, or de...

Prognostic role of aldosterone in patients with acute coronary syndrome: short and medium term follow-up
The objective of this study was to determine the prognostic role of aldosterone in patients with myocardial infarction. MethodsThe study population included 96 consecutive patients admitted to our department for ST-elevated and non-ST-elevated myocardial infarction from June 2009 to March 2012. Plasma aldosterone levels were measured at admission to hospital in all patients. A 2-year prospective follow-up was performed, and fatal events and non-fatal events, such as reinfarction, congestive heart failure and arrhythmias, were recorded. ResultsAldosterone levels at admission were associated with incidence of congestive hear...

Reimbursement systems and quality of hospital care: An empirical analysis for Italy
Abstract: There is an ongoing debate about the effect of different reimbursement systems on hospital performance and quality of care. The present paper aims at contributing to this literature by analysing the impact of different hospital payment schemes on patients’ outcomes in Italy. The Italian National Health Service is, indeed, a particularly interesting case since it has been subject to a considerable decentralization process with wider responsibilities devolved to regional governments. Therefore, great variability exists in the way tariffs are used, as Regions have settled them in accordance with the characteristics...

Short-term and long-term mortality associated with ventricular arrhythmia in patients hospitalized with acute coronary syndrome: findings from the Gulf RACE registry-2
Objectives: Ventricular arrhythmia (VA) in the setting of acute coronary syndrome (ACS) carries an ominous prognosis; however, long-term prognosis associated with VA in ACS in the Middle East is unknown. Accordingly, we sought to assess the incidence, in-hospital outcomes, and 1-year mortality of in-hospital VA in patients with ACS. Methods and results: The Second Gulf Registry of Acute Coronary Events (Gulf RACE-2) is a multinational observational study of patients with ACS, which enrolled 7930 patients. Of these, 333 (4.2%) developed VA during hospitalization. Patients with VA were significantly older (mean age 58.3 vs. ...
