Definite Plaque Echolucency is Associated with a Higher Risk of Ipsilateral Ischaemic Stroke during Early Follow up in the Asymptomatic Carotid Surgery Trial-1 (ACST-1)
Introduction: Several carotid plaque characteristics, including a thin fibrous cap, lipid necrotic core and intraplaque haemorrhage, have been suggested as potential markers to select patients at high risk for future stroke. On ultrasound, these “high risk” characteristics appear echolucent. The Asymptomatic Carotid Surgery Trial-1 (ACST-1) is the largest randomised controlled trial comparing carotid endarterectomy (CEA) with deferral of CEA in patients with severe asymptomatic carotid artery stenosis.

Effect of Therapeutic Ultrasound on Brain Angiogenesis Following Intracerebral Hemorrhage in Rats
Publication date: Available online 8 August 2015 Source:Microvascular Research Author(s): Hong-Mei Mu, Li-Yong Wang Intracerebral hemorrhage (ICH) can produce severe neurological deficits in stroke survivors. However, few effective approaches are available to improve the recovery from ICH. Given that therapeutic ultrasound exposure can enhance on angiogenesis in peripheral tissues, the present study was designed to examine the effects of therapeutic ultrasound exposure on the brain angiogenesis following ICH. To this end, we applied once daily therapeutic ultrasound treatment to rats for 7 consecutive days after intrac...

Recurrent ischemic strokes and headaches originating from Lambl's excrescence (P2.273)
CONCLUSIONS: The association between recurrent strokes and late-age onset migraine with aura should raise clinical suspicion of a cardiogenic valvular etiology. Early TEE use is necessary to diagnose cardiac valvular strands like LE. LE associated with migraine headaches should prompt a treatment with anti-thrombotic agent. While anticoagulants and cardiothoracic surgery are indicated for recurrent strokes, a combined antiplatelet therapy was effective in our patient.Disclosure: Dr. Dumitrascu has nothing to disclose. A Pilot Randomized Controlled Trial of Early versus Routine Caffeine in Extremely Premature Infants
Conclusion This pilot study demonstrated the feasibility of conducting such a trial in extremely preterm neonates. We found that early caffeine administration was associated with improved hemodynamics. Larger studies are needed to determine whether early caffeine reduces intubation, intraventricular hemorrhage, and related long-term outcomes.[...]Thieme Medical Publishers 333 Seventh Avenue, New York, NY 10001, USA.Article in Thieme eJournals:Table of contents  |  Abstract  |  Full text

Relationship Between Cerebral Blood Flow and Blood Pressure in Long-Term Heart Transplant Recipients Cerebral Blood Flow
Heart transplant recipients are at an increased risk for cerebral hemorrhage and ischemic stroke; yet, the exact mechanism for this derangement remains unclear. We hypothesized that alterations in cerebrovascular regulation is principally involved. To test this hypothesis, we studied cerebral pressure-flow dynamics in 8 clinically stable male heart transplant recipients (62±8 years of age and 9±7 years post transplant, mean±SD), 9 male age-matched controls (63±8 years), and 10 male donor controls (27±5 years). To increase blood pressure variability and improve assessment of the pressure-f...

Intraplaque Hemorrhage, Fibrous Cap Status, and Microembolic Signals in Symptomatic Patients With Mild to Moderate Carotid Artery Stenosis: The Plaque At RISK Study Brief Reports
Conclusions— In patients with a symptomatic mild to moderate carotid artery stenosis, IPH and FC status are not associated with MES. This suggests that MRI and transcranial Doppler provide different information on plaque vulnerability. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT01709045.

3D Ultrasonic Tissue Characterization for Asymptomatic Carotid Atherosclerosis
Carotid plaque histology has confirmed that intraplaque hemorrhage and enlarging lipid-cores are important determinants of plaque rupture and stroke. Noninvasive characterization of these features may identify individuals at risk for stroke. Two-dimensional (2D) longitudinal-sectional ultrasound images have been used to predict plaque histology, but a single slice provides limited information. Tissue-characterization from 3D ultrasound imaging may offer more comprehensive information. We developed a clinical 3D ultrasound protocol to quantify plaque composition (hemorrhage, lipid, calcium and fibromuscular tissue), measure...

E-006 Familial Incidence of the Congenital Torcular Dural Arteriovenous Shunt
Purpose There are several hereditary disorders that are well known to be associated with arteriovenous malformations (AVMs) involving the central nervous system including Sturge-Weber, Klippel-Trenaunay-Weber, and Parkes-Weber syndromes as well as hereditary haemorrhagic telangiectasia and hereditary neurocutaneous angiomatosis. Furthermore, familial incidence of AVMs in the absence of congenital hereditary disorders also has been reported. Herein, we describe the occurrence of the giant torcular herophili (TH) dural arteriovenous fistula (AVF) in two pediatric cousins treated with endovascular embolization. Case report C...

Rapidly Fatal Internal Carotid Artery Mycotic Aneurysm Rupture in a Rheumatoid Patient Taking a TNF-α Inhibitor: Case Report and Literature Review
Conclusions Although fungal mycotic aneurysms of the ICA are rare, their incidence may increase with the expanded use of immunosuppressive medications. Patients with rheumatoid arthritis who take potent immunosuppression regimens may be prime candidates for mycotic aneurysms because they often have two favoring conditions: atherosclerosis and immunosuppression. These ICA aneurysms carry a high mortality rate, so early diagnosis and aggressive therapy, potentially by endovascular trapping/vessel occlusion coupled with long-term antifungal therapy, is essential.[...]Georg Thieme Verlag KG Stuttgart · New YorkArticle in Th...
