Accidental exposures to peanut in a large cohort of Canadian children with peanut allergy
Conclusion Despite increased awareness, accidental exposures continue to occur, mainly at home, and most are managed inappropriately by both health care professionals and caregivers. Consequently, more education is required on the importance of strict allergen avoidance and the need for prompt and correct management of anaphylaxis.

Sensitization pattern of crustacean allergic individuals can indicate allergy to molluscs
This study investigated the sensitization pattern of crustacean allergic patients according to tolerance to molluscs. Thirty‐one patients with anaphylaxis to crustaceans (14 with mollusc allergy and 17 with mollusc tolerance) were studied using skin prick tests (SPTs), specific IgEs (sIgE) and SDS‐PAGE immunoblotting. IgE‐reactive shrimp proteins were identified by proteomic analyses. Patients with mollusc allergy presented more frequently SPTs positive to molluscs and higher sIgE titres in response to both molluscs and crustaceans. Shrimp‐sIgE and rPen a1‐sIgE values of 1.57 kUA/L and 4.38 kUA/L respectively, sh...

School personnel's self‐efficacy in managing food allergy and anaphylaxis
ConclusionsThe study supports the use of self‐efficacy scale to identify specific areas where teachers’ confidence in their ability to care for food‐allergic students is especially weak. This would empower the development of training programs specifically tailored to the needs of teachers and caregivers. This article is protected by copyright. All rights reserved.

Penicillin skin testing in hospitalized patients with β-lactam allergies: Effect on antibiotic selection and cost.
CONCLUSION: After the implementation of a PST protocol, we observed a decrease in non-BLA use in patients with previously documented β-lactam allergy. PST is a safe and cost-effective procedure to serve as a negative predictor test for penicillin hypersensitivity mediated by IgE. PMID: 27211057 [PubMed - as supplied by publisher]

Use of a Penicillin Allergy Screening Algorithm and Penicillin Skin Testing for Transitioning Hospitalized Patients to First-Line Antibiotic Therapy
Conclusions The use of the PAHA and subsequent PST is a safe, effective multidisciplinary intervention that facilitates the transition to β-lactam antibiotics. Our approach is unique in that it prioritizes patients based on the use of second-line antibiotics, and then applies an algorithm to determine eligibility for PST.

An In-Depth Characterization of a Large Cohort of Adults Patients with Eosinophilic Esophagitis.
CONCLUSION: Our adult cohort of manually verified, biopsy-proven EoE showed increased risk of pollen food allergy syndrome, anaphylaxis, and comorbid autoimmune and psychiatric conditions compared with the UPHS population. There was also an increased prevalence of impaction and stricture/fibrosis requiring endoscopic intervention compared with the pediatric population. PMID: 30223114 [PubMed - as supplied by publisher]

IgE-Mediated Food Allergy
AbstractFood allergies are defined as adverse immune responses to food proteins that result in typical clinical symptoms involving the dermatologic, respiratory, gastrointestinal, cardiovascular, and/or neurologic systems. IgE-mediated food-allergic disease differs from non-IgE-mediated disease because the pathophysiology results from activation of the immune system, causing a T helper 2 response which results in IgE binding to Fcε receptors on effector cells like mast cells and basophils. The activation of these cells causes release of histamine and other preformed mediators, and rapid symptom onset, in contrast with non...

A Retrospective Critical Analysis and Risk Stratification of Penicillin Allergy Delabeling in a UK Specialist Regional Allergy Service
ConclusionsSystematic clinical characterization and risk stratification has an important role in Pen-A delabeling. These data provide proof of concept for a guideline-based selection of patients labeled with Pen-A for a direct penicillin challenge. Patients in the “low risk” group seem suitable for this intervention, although a rigorous prospective evaluation is needed in a multicenter study.

Investigation into the α-Gal syndrome: Characteristics of 261 children and adults reporting red meat allergy
ConclusionsThe α-Gal syndrome is a regionally common form of food allergy that: has a characteristic but not universal delay in symptom onset; includes gastrointestinal symptoms; can develop at any time in life; and, is equally common in otherwise non-atopic individuals.
