Prenatal, perinatal and neonatal risk factors for perinatal arterial ischaemic stroke: a systematic review and meta ‐analysis
The aim of the present study was to perform a meta‐analysis of published data to determine the significance of clinical factors and exposures to the risk of perinatal arterial ischaemic stroke (PAIS) and provide guidance for clinical diagnosis and treatment. A comprehensive literature search of the PubMed, Embase, MEDLINE and Cochrane Library databases for relevant observational studies (cohort/case−control) from March 1984 to March 2016 was undertaken. Two review authors independently examined the full text records to determine which studies met the inclusion criteria and evaluated risk factors for PAIS. Risk ratios, ...

Ambient Temperature and Stillbirth: A Multi-Center Retrospective Cohort Study
Conclusions: Extremes of local ambient temperature may have chronic and acute effects on stillbirth risk, even in temperate zones. Temperature-related effects on pregnancy outcomes merit additional investigation. https://doi.org/10.1289/EHP945 Received: 09 August 2016 Revised: 06 December 2016 Accepted: 22 December 2016 Published: 22 June 2017 Address correspondence to P. Treating periodontal disease for preventing adverse birth outcomes in pregnant women.
CONCLUSIONS: It is not clear if periodontal treatment during pregnancy has an impact on preterm birth (low-quality evidence). There is low-quality evidence that periodontal treatment may reduce low birth weight (< 2500 g), however, our confidence in the effect estimate is limited. There is insufficient evidence to determine which periodontal treatment is better in preventing adverse obstetric outcomes. Future research should aim to report periodontal outcomes alongside obstetric outcomes. PMID: 28605006 [PubMed - as supplied by publisher]

Drinking Water Salinity and Raised Blood Pressure: Evidence from a Cohort Study in Coastal Bangladesh
Conclusions: DWS is an important source of daily sodium intake in salinity-affected areas and is a risk factor for hypertension. Considering the likely increasing trend in coastal salinity, prompt action is required. Because MAR showed variable effects, alternative technologies for providing reliable, safe, low-sodium fresh water should be developed alongside improvements in MAR and evaluated in “real-life” salinity-affected settings. https://doi.org/10.1289/EHP659 Received: 01 February 2016 Revised: 14 June 2016 Accepted: 31 August 2016 Published: 30 May 2017 Address correspondence to Dr. Pauline Scheelbeek, ...

Severe Maternal Morbidity in a Large Cohort of Women With Acute Severe Intrapartum Hypertension
(Am J Obstet Gynecol. 2016;215(1):91.e1–91.e7) Adverse effects of hypertensive disorders of pregnancy such as stroke, eclampsia, hemolysis, elevated liver enzymes, low platelet count syndrome, renal failure, and disseminated intravascular coagulation are commonly associated with high rates of maternal morbidity and mortality in the United States. The present study aimed to analyze existing management patterns in a large group of women with severe intrapartum hypertension, to determine the maternal morbidity associated with severe intrapartum hypertension and compare it to that in women without severe hypertension.

Risk Factors for Pregnancy-Associated Stroke in Women with Preeclampsia: a Case-Control Study (S15.007)
Conclusions:Chronic hypertension, underlying prothrombotic conditions and infections increase PAS risk in women with PEC. These women may warrant closer postpartum monitoring, due to high PAS-associated mortality in this population.Disclosure: Dr. Miller has nothing to disclose. Dr. Gatollari has nothing to disclose. Posterior reversible encephalopathy syndrome in a postpartum hemorrhagic woman without hypertension: A case report
Rationale: Posterior reversible encephalopathy syndrome (PRES), which diagnosis is based on clinical symptoms and radiological features, is a neurotoxic disease characterized by a set of clinical manifestations, such as seizure, headache, visual, and/or consciousness disturbance. It is the first case of PRES followed by postpartum hemorrhage (PPH) without underlying disease. Patient concerns: A 37-year-old healthy woman had PPH after caesarean section. Six days after delivery, headache occurred suddenly, followed by episodes of clonus seizure. Diagnoses: Brain computed tomography showed ischemic stroke. However, magnetic...

Committee Opinion No. 692: Emergent Therapy for Acute-Onset, Severe Hypertension During Pregnancy and the Postpartum Period.
Authors: Abstract Acute-onset, severe systolic hypertension; severe diastolic hypertension; or both can occur during the prenatal, intrapartum, or postpartum periods. Pregnant women or women in the postpartum period with acute-onset, severe systolic hypertension; severe diastolic hypertension; or both require urgent antihypertensive therapy. Introducing standardized, evidence-based clinical guidelines for the management of patients with preeclampsia and eclampsia has been demonstrated to reduce the incidence of adverse maternal outcomes. Individuals and institutions should have mechanisms in place to initiate the...

Chapter 35 Neurologic complications in critically ill pregnant patients
Publication date: 2017 Source:Handbook of Clinical Neurology, Volume 141 Author(s): W.L. Wright Neurologic complications in a critically ill pregnant woman are uncommon but some of the complications (such as eclampsia) are unique to pregnancy and the puerperal period. Other neurologic complications (such as seizures in the setting of epilepsy) may worsen during pregnancy. Clinical signs and symptoms such as seizure, headache, weakness, focal neurologic deficits, and decreased level of consciousness require careful consideration of potential causes to ensure prompt treatment measures are instituted to prevent ongoing neuro...

Role of corin in the regulation of blood pressure
Purpose of review: Corin is a transmembrane protease that activates atrial natriuretic peptide (ANP), an important hormone in regulating salt-water balance and blood pressure. This review focuses on the regulation of corin function and potential roles of corin defects in hypertensive, heart, and renal diseases. Recent findings: Proprotein convertase subtilisin/kexin-6 has been identified as a primary enzyme that converts zymogen corin to an active protease. Genetic variants that impair corin intracellular trafficking, cell surface expression, and zymogen activation have been found in patients with hypertension, cardiac hy...

Inherited and Uncommon Causes of Stroke
This article is a practical guide to identifying uncommon causes of stroke and offers guidance for evaluation and management, even when large controlled trials are lacking in these rarer forms of stroke. Recent Findings: Fabry disease causes early-onset stroke, particularly of the vertebrobasilar system; enzyme replacement therapy should be considered in affected patients. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL), often misdiagnosed as multiple sclerosis, causes migraines, early-onset lacunar strokes, and dementia. Moyamoya disease can cause either ischemic or he...
