Early Clinical Outcomes of Hybrid Arch Frozen Elephant Trunk Repair with the Thoraflex Hybrid Graft.
CONCLUSIONS: Hybrid aortic arch and FET repair with the Thoraflex hybrid graft appears to be associated with good clinical outcomes, despite being early in the learning curve with this graft. Further investigation with this device is warranted to establish its role within the variations of hybrid arch repair. PMID: 30291835 [PubMed - as supplied by publisher]

What is the specificity of the aortic dissection detection risk score in a low prevalence population?
CONCLUSIONS: Our study confirms that in North America the prevalence of AAS in those undergoing advanced imaging is low. The ADD-RS in this population has a low specificity. A lack of defined inclusion criteria and a low specificity limits the application of this rule in practice. This article is protected by copyright. All rights reserved. BMI and Mortality in UK Biobank: Revised Estimates Using Mendelian Randomization.
CONCLUSIONS: Results support a causal role of higher BMI in increasing the risk of all-cause mortality and mortality from several specific causes. PMID: 30358150 [PubMed - in process]

Image-based screening.
Abstract Screening is a special issue in medical questions concerning disease prevention. Preconditions for screening are clearly defined by the World Health Organization. High prevalence, effectiveness of therapy, availability of accepted test procedure and consensus concerning the economic concerns are necessary for successful implementation of a screening program. Preventive diagnostic studies can only be understood if one is familiar with the statistical terms sensitivity, specificity, prevalence, incidence and bias (especially overdiagnosis and lead time bias). Aspects of radiation protection are especially...

Impact of aortic aneurysms in trans-catheter aortic valve replacement: A single center experience.
CONCLUSIONS: In our patient cohort, 9.5% of patients who underwent TAVR had concomitant aortic aneurysms. Patients with aortic aneurysms had similar incidence of vascular complications as well as in-hospital and 6-month MACE compared to those without. PMID: 30595280 [PubMed - in process]

Clampless aortic punch system for making a large-diameter access route without side-biting clamp: a preliminary study
This study aimed to present the results of our preliminary study on the clampless aortic punch system (APS). The swine aorta was used as experimental specimen (diameter 16 –20 mm). A 10-mm collagen-impregnated knitted Dacron graft was anastomosed to the aorta, and the APS was inserted into it. After piercing the aorta with the inner fish hook of the APS, the aortic wall was scooped out by an outer round cutter. Three different-angled cutters (0°, 15°, and 30°) w ere tested three times. The diameter of the punched-out lesion ranged from 6 to 9 mm (median 8 mm). Macroscopically, no major vessel injuries were seen 15...

Curcumin, the golden spice in treating cardiovascular diseases.
Abstract Cardiovascular diseases (CVDs) cause the largest mortality worldwide, and much attention has been focused to unravel the mechanisms and optimize the treatment regimens. Curcumin is an important bioactive component of turmeric that has been widely applied as traditional medicine to prevent and treat various diseases in some countries. Recent studies have demonstrated its potent activities in modulating multiple signaling pathways associated with cellular growth, proliferation, survival, inflammation and oxidative stress. The cardiovascular protective properties of curcumin in CVDs have been fully illustrat...

Tracheostomy After Thoracoabdominal Aortic Aneurysm Repair: Risk Factors and Outcomes
ConclusionsPatients who undergo tracheostomy after TAAA repair have a high risk of early and late mortality, as well as prolonged hospitalization. Strategies for improving survival outcomes in tracheostomy patients warrant investigation.

A Rare Cause of Stroke Four Weeks After Ascending Aortic Aneurysm Repair
A 71-YEAR-OLD, 97 kg, 178 cm man with a history of a slowly expanding 5.4 cm ascending aortic aneurysm, bicuspid aortic valve disease without stenosis, hypertension, hyperlipidemia, type II diabetes mellitus, and hemodynamically insignificant coronary artery disease underwent ascending aortic replacement using a woven synthetic graft. The patient's postoperative course was complicated by persistent sternal drainage that required exploration, evacuation of pericardial fluid, and antibiotic treatment, but he otherwise made an uneventful recovery.

Characteristic Cerebrovascular Findings Associated with ACTA2 Gene Mutations.
Abstract A specific mutation (Arg179) of the ACTA2 gene has previously been described to cause a syndrome of multisystemic smooth muscle dysfunction with an extremely characteristic cerebrovascular appearance.1 Accurate neuroimaging diagnosis of this entity is important as this syndrome predisposes to complications such as early-onset ischemic stroke and ascending thoracic aortic aneurysm.2,3 The following case demonstrates a previously undescribed ACTA2 mutation (Met46) with an identical cerebrovascular imaging appearance to that of Arg179 mutations, but a less severe overall phenotype. Open Descending and Thoracoabdominal Aortic Repairs in Patients Younger than 50 Years Old
ConclusionsPatients under 50 years old with D/TAAA have low surgical risks and open repairs can be performed with excellent short-term and durable long-term results. Open surgical repairs should be considered initially in younger patients requiring D/TAAA repairs. HTAD warrants closer postoperative surveillance.

Early Outcomes after Thoracoabdominal Aortic Aneurysm Repair with Hypothermic Circulatory Arrest
ConclusionsOur extended experience with this technique confirms its safety and effectiveness when used on a routine basis. The rates of spinal cord injury and permanent renal failure are among the lowest reported in the literature. Particularly favorable outcomes were observed in younger patients and those undergoing elective operations.
