Mitral valve prolapse.
This article reviews the pathophysiology, genetics, clinical features, diagnostic imaging, complications, long term outcomes and indications for intervention in mitral valve prolapse. Expert commentary: Several key dilemmas in the management of mitral valve prolapse remain. Factors which influence progression of mitral regurgitation are unclear and therefore we have no therapeutic targets to prevent progression. Evidence-based methods to reduce the risk of sudden death, stroke and endocarditis have not been identified. In symptomatic patients with severe mitral regurgitation valve surgery is recommended. In asymptomatic pa...

Hemodynamic and clinical performance of Solo stentless bioprosthetic aortic valves
ConclusionsSolo stentless aortic valves are safe to implant, with promising clinical outcomes in short- and medium-term assessment. Moreover, they show an excellent hemodynamic performance: low transvalvular gradients, large effective orifice areas and low incidence of patient-prosthesis mismatch.

Role of Echocardiography in Assessment of Cardioembolic Sources: a Strong Diagnostic Resource in Patients with Ischemic Stroke
AbstractPurpose of ReviewThis review will discuss the most frequent sources of cardiac embolism and the role of echocardiography in these different clinical settings, and, in addition, provide suggestions about the choice between transthoracic (TTE) and transesophageal echocardiography (TEE).Recent FindingsStroke is the third leading cause of death in industrial countries, and 15 –40% of all ischemic strokes are due to cardioembolism. TTE and TEE are cornerstones in the detection of cardioembolic sources and provide fundamental information about the embolic risk and most suitable treatment of these patients, improving lo...

Patterns of incidence rates of cardiac complications in patients with congenital heart disease
ConclusionsCardiac complications are frequent in congenital heart disease. Apart from perioperative stroke and complete heart block, incidence rates are low in childhood but the incidence increases during adult life. These data underscore the need of life-long follow up and may help for better allocation of resources maintaining follow-up.

Early vs Late Surgery for Patients With Endocarditis and Neurological Injury: A Systematic Review and Meta-analysis
ConclusionsAvailable observational data support delaying surgery by 7-14 days if possible in IE complicated by ischemic stroke and> 21 days in hemorrhagic stroke to decrease perioperative mortality and neurological exacerbation rates.

Systemic atrioventricular valve replacement by mechanical prosthesis in children: Evolution in practice and predictors of long-term outcome
ConclusionsEarly mortality after SAVR decreased during the second period of our study. Modification of the operative technique was the main cause of better early outcomes. SAVR replacement remains a challenging procedure with high mortality but limited mortality related to anticoagulation.

Workup and Management of Native and Prosthetic Valve Endocarditis
AbstractInfective endocarditis (IE) is associated with significant serious adverse outcomes including death. IE usually presents with diverse clinical picture and syndromic diagnoses including heart failure, stroke, and peripheral embolization. Given variable, vague, and syndromic presentations, the diagnosis of IE may be delayed for days to weeks. Maintaining a high index of suspicion among clinicians is the key to early recognition of the disease and prompt initiation of antimicrobial therapy to prevent IE-associated mortality and morbidity. Blood culture and echocardiography remain essential tools in the diagnosis of in...

More to the picture of the psychological impact of endocarditis and thoracic aortic pathology.
Abstract Over the years there has been substantial advanced in the diagnosis and surgical management of complex thoracic aortic disease and complex endocarditis. As these therapies are being offered to a growing segment of patients-and more and more patients are felt to potentially benefit from such therapies, the long-term consequences of these interventions is sometimes poorly understood. While traditional medical complications, such as stroke, renal failure, respiratory failure, and even death are often the focus of outcomes studies, little is known on the impact of these diseases and therapies on mental health...

Intracranial haemorrhage in infective endocarditis.
CONCLUSIONS: ICH is a common complication of left-sided IE. The impact on prognosis is dependent on mechanism (haemorrhage of undetermined aetiology). We observed a higher mortality rate in patients who had conservative treatment when cardiac surgery was indicated compared with in those who underwent cardiac surgery. Minimally Invasive Mitral Valve Repair Through Right Minithoracotomy - 11-Year Single Institute Experience.
CONCLUSIONS: MIMVR can be performed safely with low levels of mortality and morbidity, and provides sufficient repair durability. A learning curve exists in terms of repair durability, especially for anterior mitral leaflet repair. PMID: 29657253 [PubMed - as supplied by publisher]
