Super High Dose Beta-blocker Administration for the Patients with Dilated Cardiomyopathy
First case is a 25-year-old female with dilated cardiomyopathy. Left ventricular end-diastolic diameter (LVEDD) was 74  mm and ejection fraction (EF) was 29%. Carvedilol was already uptitrated to 20 mg twice daily. Right heart catheterization (RHC) revealed very low cardiac output (C.O. 2.82 l/min), cardiac index (C.I. 1.47 l/min/m2), stroke volume (SV 29 ml) and rapid heart rate (HR 96/min) despite maximu m dosage of carvedilol in Japan. Carvedilol was uptitrated to 50 mg twice daily and additional 10 mg bisoprolol twice daily was started under administration of low-dose dobutamine.

The heart in m.3243A > G carriers.
CONCLUSIONS: Cardiac involvement in m.3243A>G carriers has been only rarely systematically studied, which is perhaps why the incidence of cardiac diseases in MELAS is lower than would be expected. Myocardial abnormalities are much more frequent than arrhythmias or conduction defects. All symptomatic and asymptomatic m.3243A>G carriers should be systematically investigated for cardiac disease. Is resynchronization therapy really efficient in laminopathy?
ConclusionCardiac resynchronization therapy seems to be less efficient in LMNA patients compared to usual implanted patients with other type of advanced cardiomyopathy. An impaired right ventricular stroke function seems to be the only predictive factor leading to poor response to CRT.

Mortality risk in dilated cardiomyopathy: the accuracy of heart failure prognostic models and dilated cardiomyopathy ‐tailored prognostic model
ConclusionsAn analysis of various HF prognostic models found them to be suboptimal for DCM patients. A self ‐developed DCM prognostic model showed improved performance over the nine other models studied. However, further validation of the prognostic model in different DCM populations is required.

Modification of ventriculo –arterial coupling by spironolactone in nonischemic dilated cardiomyopathy
ConclusionsIn NIDCM, adding MRA to conventional anti ‐failure therapy markedly improved LV ejection fraction and reduced peripheral vascular resistance, due to both improved LV contractility and especially to enhanced V–A coupling, as Ea decreased to normal. Total circulatory performance was a sensitive indicator of both LV pump performance and th e arterial loading conditions.
