Upper airway microbiome transplantation for patients with chronic rhinosinusitis
ConclusionsNasal microbiome transplants obtained from healthy individuals and administered as nasal lavages to patients with CRSsNP are feasible. The patients reported significant and lasting reduction of symptoms and these findings were associated with a lasting increase in abundance and diversity of the local bacterial flora. The observations, which need to be confirmed by randomized controlled trials, may constitute a new treatment avenue for these difficult to treat patients where antibiotics only provides short lasting symptom control.Trial registration number: NCT03122795.This article is protected by copyright. Association between Staphylococcal Enterotoxin (SE)-specific IgE and specific IgE for house dust mites (HDM) in Brazilian patients with Chronic Rhinosinusitis with Nasal Polyps (CRSwNP).
SE are a group of superantigens formerly associated with several respiratory diseases, including asthma, rhinitis and CRSwNP. Superantigens trigger the production of high specific IgE levels and are usually related to high levels of total IgE (tIgE) and eosinophilic inflammation. We aim to evaluate the correlation between the positivity of SE-specific IgE for A; B; C; TSST-1; tIgE and specific IgE for D. pteronyssinus (DP); D. farinae (DF) and B. tropicalis (BT) in CRSwNP Brazilian patients.36 CSRwNP patients, defined by 2012-EPOS (European Position Paper on Rhinosinusitis) criteria, submitted to nasal endoscopy and CT...

What Are Common Gastrointestinal Polyposes in Children?
Discussion Bloody stools or blood in the diaper is a relatively common complaint in general pediatrics. For most patients it is often a minor concern. Commonly it is a transient problem (e.g. rectal fissure caused by constipation or trauma) or often not blood (e.g. urate crystals in the diaper, food, menses, etc.). Real blood does occur with an identifiable cause such as long distance running or heavy exercise, or milk protein allergy/sensitivity that improves with a milk-restricted diet. Many more serious causes are associated with heavier or more recalcitrant bleeding, increased defecation, abdominal pain, poor eating an...

Relationship between socioeconomic status, exposure to airborne pollutants, and chronic rhinosinusitis disease severity
ConclusionLower SES predicted higher exposure to air pollution and increased disease severity in patients with CRS as demonstrated by the increased need for steroid treatment.

AERD Associated Nasal Polyposis: Efficacy of Postoperative Antileukotriene Therapy in Comparison with Aspirin Desensitization. A Retrospective Study
Conclusion: Postoperative treatment with leukotriene-receptor-antagonists and aspirin desensitization both significantly reduce nasal polyp recurrence. MT has a positive effect on subjective sinonasal outcomes and patients ’ quality of life over time.Int Arch Allergy Immunol

Association of asthma control with allergic rhinitis severity in adult patients
Conclusions: Allergic rhinitis is common comorbidity of asthma that interrelated to asthma control. Identifiable allergens could be detected AR in asthma by SPT. Therefore, reduction of allergens exposure could become preventive approach for asthma patients with AR.

Loss-of-Function in SMAD4 Might Not Be Critical for Human Natural Killer Cell Responsiveness to TGF- β
This study was carried out in accordance with approval of the Melbourne Health and Walter and Eliza Hall Institute of Medical Research's Human Research Ethics Committee (approval number: 2013.081). All subjects gave written informed consent for participation and publication. Results and Discussion TGF-β signaling in NK cells is associated with: phosphorylation in SMAD2 and 3, inhibition of IL-15-induced metabolism/proliferation, simultaneous downregulation of CD44, CD49e, and Eomes, and upregulation of CD16 and CD49a expression (7, 10). SMAD family member 4 (SMAD4) belongs to the SMAD family of transcript...

Chronic otitis media with effusion in chronic sinusitis with polyps.
Abstract The relationship between otitis media with effusion (OME) and chronic rhinosinusitis with nasal polyposis (CRSwNP) remains unclear. We conducted a cross-sectional study of 80 consecutively presenting patients-42 males and 38 females, aged 15 to 76 years (median: 48)-who were diagnosed with CRSwNP. Our aim was to ascertain the prevalence of OME in CRSwNP patients, to determine whether the severity of CRSwNP affected OME, and to identify risk factors for OME in CRSwNP patients. The severity of CRSwNP was assessed on the basis of nasal symptoms, endoscopic nasal examinations, and Lund-Mackay staging scores. ...

The association of air pollutants and allergic and nonallergic rhinitis in chronic rhinosinusitis
ConclusionOur results suggest that small inhalant pollutants may contribute to nonallergic symptomatology in patients with and without nasal polyps. Regardless of allergy status, BC may play a role in CRS symptom severity.

Relationships between upper airway diseases and lower airway obstruction in the adult Korean general adult population
Conclusions: Smoking may be a confounder of the relationships between nasal polyps and lower airway obstruction in a general population samples. However, considering the heterogeneity of nasal polyps, phenotype investigation is warranted.

Denatonium ‐induced sinonasal bacterial killing may play a role in chronic rhinosinusitis outcomes
ConclusionBacterial killing after stimulation with low levels of denatonium correlates with number of prior FESS and postoperative SNOT‐22 scores in CRSwNP. Some symptoms of CRS in patients with hyperresponsiveness to low levels of denatonium may be due to increased airway immune activity or inherent disease severity.

Remodeling changes of the upper airway with chronic rhinosinusitis
ConclusionRemodeling features are present in CRS. Tissue eosinophilia and evidence of eosinophil activation is closely associated with remodeling features of CRS, associated mucosal damage and clinical symptoms.

Clinical Practice Guideline (Update): Adult Sinusitis
Objective This update of a 2007 guideline from the American Academy of Otolaryngology—Head and Neck Surgery Foundation provides evidence-based recommendations to manage adult rhinosinusitis, defined as symptomatic inflammation of the paranasal sinuses and nasal cavity. Changes from the prior guideline include a consumer added to the update group, evidence from 42 new systematic reviews, enhanced information on patient education and counseling, a new algorithm to clarify action statement relationships, expanded opportunities for watchful waiting (without antibiotic therapy) as initial therapy of acute bacterial rhinos...
