Rheumatology and long term steroids
I did a month of rheumatology in residency and I don’t remember this being a thing. I have noticed that two local rheumatologists in different cities near me that right after they diagnose someone with autoimmune disease, even things without a firm diagnosis (like undifferentiated connective tissue issues) They will start plaquenil or methotrexate, which is fine, but they also put patients on oral steroids for 6-8 weeks!! At 10-20mg of prednisone per day. Is that now the standard of... Read more (Source: Student Doctor Network)

Rheum: Starting prednisone on suspected conditions as initial treatment question
Hello, this is my first post. I'm from Chile and I'm in my first year of internship. Once a Rheumatologist told us to not starting prednisone in rheumatoid arthritis suspicion (we don't have access to DMARDs as general practitioners) because at time of referral there will be no objective clinical or laboratory signs of active disease so it would be more difficult to do the differential, so his suggestion was to start only NSAIDs and early referral (although it takes a pair of months here,... Bronchoscopy Question
My 17 month old son has a CRMS dx (df508 + a "varying consequence" mutation and negative sweat). We switched clinics about 4 months ago. Our new clinic felt that my son's health history and culture history warranted a bronchoscopy. Last Wednesday he had the bronch as well as an endoscopy and an Infant PFT. Normally they have you return 2 weeks post procedure but because of the holiday it looks like we may not be getting in until sometime next month. I've asked the nurse to talk to our pulm about getting us results over the phone at least - we'll see if it happens - but in the meantime I have a couple questions...

Orkambi Journey with 19 y/o Allan Diagnosed at 4 months
I really wasn't sure how to do this. I am the father of Allan. He is a bit shy to post his own thread and definitely not a YouTube video. So here I come. Hope its Ok to start my own thread. You may post in here and ask questions if you wish, if you have CF, a parent of a child with CF or just curious about CF. I am an open book. I can only put in this thread of what my son tells me. He knows this is being done and I do have his consent. So just a tad bit of background on Allan. He is 19 y/o and he was diagnosed at 4 months old by a Doctor here in Texas. His Normal FEV1 is usually around 85% on a good day. Had a b...

ABPA and Xolair
Hi folks, Does anyone have issues with Allergic bronchopulmonary aspergillosis (ABPA) ? Do you do anything for it, any luck with treatment? I just got off Prednisone after being on it for the past 20 months. At the start of 2014 I had a CF-related decline in PFT that I was unable to really bounce back from after 3 weeks of IV antibiotics. They noticed my IGE level was over 6000 (normal is around 100 I believe), and had been for several months. They tried prednisone and voriconizole (one of the only anti-fungal medications, I've been told) but my IGE only dropped to around 1500-1000. Next the docs sent me over to alle...

so frustrated...major drop in lung function so fast
Has this happened to anyone else? I was always quite healthy with CF..the occasional exacerbation, but oral antibiotics took care of it. FEV always around 60-70. Then, two months ago, I got pretty sick. Fevers, the whole bit. The fevers and chills went away after two weeks of IV antibiotics, but now I still feel like crap (lung wise)! I never really did feel better. My lung function went down to 34% and is staying there. I was put on prednisone for 16 days. That is done now, and it did nothing. I started symbicort at the time I started prednisone too, and I'm still on that. I went in today and the lung function only went u...

severe asthma
Anyone else here have severe large airway chronic asthma? Its really kicking my butt right now to the point where I can barely take any inhaled meds since I am so hyperreactive. Any suggestions? IgE is normal, neutophils quite high. FEV 58%, but O2 hanging out between 92-95, drops to 88/89 during exertion, around 90/91 for sleep. Two years ago w/only mild asthma I ran a 10K with this same FEV. Now stairs wind me badly. Please help! Prednisone helps, but only a high dose and dont want to be on it for years. Docs are suggesting low dose methyltrexate. Any experience with this, anyone? (Source: Cystic Fibrosis A...

Going to the doctor for a sweat test rcmndtn.
Im going to the doctor this week to try and get a recommendation for a cf sweat test. Im worried they may think Im crazy, but I'm so frustrated that I dont know what else to do. Since a child I have been getting sinus infections and bronchitis frequently. My doctor diagnosed me with asthma and put me on antibiotics and that worked for a while. I'm now 22 years old (female) and have started going downhill since 16. I know cystic fibrosis is usually diagnosed as a child but i was not tested at birth and I have read that some milder mutations do not.become severe until later in life. Anyway at 16 I started having severe pain ...

coughing up blood at gym
So last thursday I was working out at my gym. I usually do 2x a day. Cardio and weight training. I fell in love with it. I was doing leg lifts and I noticed a wheeze that was really watery. I knew what was coming because it happened 4 years ago. Blood! I coughed it out and it was all over my hand. (I hope nobody saw) but I nonchalantly walked to bathroom trying not to panic. It went on for about 5 minutes and I left, I continued to go to the e.r to just get checked out. Everything was fine. Doctor started an extension on bactrim (which I was already taking), then 60mg prednisone, azithomyocin, vit k, etc haven't coughed up...
