Do you list differentials in your ED notes?
Polling the hive-mind here. The culture at my residency program is make a list of your differentials, something to the effect of "DDx: ACS, pulmonary embolism, thoracic aortic dissection, pneumothorax, pneumonia..." I rotated in at a community hospital recently and got chided for doing that. I was told to not make a list because we didn't necessarily work up everything fully in that list (i.e. thought about PE but didn't do a CT pulmonary angiogram) because it exposes you to medicolegal... I'm all for documenting RELEVANT things on our records, but I've seen some bizarre things being written down at times: 1) What number OR the patient was brought to 2) What fruit the patient is allergic to 3) "Patient's consent was obtained in holding prior to OR" 4) What size IV was started 5) "A full H&P was performed prior to OR" 6) "Patient has prior history of pneumonia." How does pneumonia precipitate acute chest syndrome?
Pathoma says that its because pneumonia causes vasodilation of pulmonary circulation which causes decreased blood flow, increased RBC transit time, then says that causes RBC dehydration, acidemia, and deoxygenation. How does increased transit time of RBC in the pulmonary circulation cause deoxygenation? Shouldn't it be the opposite? Thanks for any help! (Source: Student Doctor Network)

DNR without DNI?
Do you guys typically offer to make patients DNR without also making them DNI? I'm not talking about patients who have conditions that are likely reversible such as pneumonia. Mainly asking about patients with many comorbidities who would be unlikely to be extubated if they were ever intubated or have advanced dementia/COPD/CHF. I've always felt that CPR and intubation cross a certain line and if they are unlikely to bring the patient back to a reasonable baseline, they should not be offered... DNR without DNI? fellowship 7 years out due to health issues???
I've posted previously about considering leaving EM due to a back injury - I had a very successful disc replacement surgery 4.5 years ago, and that in of itself is a non-issue, save for my inability to get LTD at a rate that makes any sense. what IS an issue, however, is that I have a lung problem leading to recurrent and increasingly severe bouts of pneumonia. may or may not be related to newly diagnosed connective tissue disorder. yeah, never going to qualify for ANY individual LTD even... fellowship 7 years out due to health issues??? zythromax long term?
Hi. No diagnosis yet for my 8 yr old daughter. Just had more blood work done today. Waiting on results of PCD panel. Depending on results, will see CF doc in WI or head to indy or cincy. So, daughter was sick recently. Xray showed probable partial lung collapse and possible pneumonia again. Trying to get sputum sample from her. Pulm wants her on zythromax MWF all winter. I don't know what dose but pharmacy double checking before dispensing b/c high dose. I guess this is used as preventative for inflammation. Anyone heard of this? i HAVE A 14 YEAR OLD SON THAT HAS HAD HEALTH PROBLEMS HIS WHOLE LIFE. HE HAD RSV WHEN HE WAS A FEW WEEKS OLD, FAILURE TO THRIVE AT A COUPLE MONTHS OLD AND PNEUMONIA EVERY WINTER FROM AGE ONE UNTIL ABOUT 11. Could it be CF in my 14 yo?
My son is 14 years old, weighs 78 lbs and is 4'10". He has had a cough for 2 years now and asthma among other things have been ruled out. He has had pneumonia twice and bronchitis once in this time. So confused....no diagnosis...stop looking?
Looking for confirmation my 8 year old daughter doesn't have cf. Wondering what is going on. Has environmental allergies (as shown on skin tests 2x).
