Do you list differentials in your ED notes?
Polling the hive-mind here. The culture at my residency program is make a list of your differentials, something to the effect of "DDx: ACS, pulmonary embolism, thoracic aortic dissection, pneumothorax, pneumonia..."
I rotated in at a community hospital recently and got chided for doing that. I was told to not make a list because we didn't necessarily work up everything fully in that list (i.e. thought about PE but didn't do a CT pulmonary angiogram) because it exposes you to medicolegal...
