Post-Pneumonic Poor Air Entry
Hello, a 2 years old male presented with typical picture of lobar pneumonia, examination showed localized crepitation with no air entry in the right lower lobe and CXR showed right lower zone infiltration, after 10 days with antibiotic treatment, the fever subsided but there is still poor air entry, what could be the possible cause ? 3 months Post Transplant...
My boyfriend was transplanted Feb 24, 2013. He was at about 13% lung function at the time and was listed for about 8 months before transplant. He had one dry run around Thanksgiving of 2012. He was very hesitant about this whole process. In January of 2004 he got a bad case of pneumonia and was put in a medical induced coma. We live in Syracuse NY but he was transported to Boston during this time. he got put on the transplant list there. He was eventually taken off the list because he seemed to bounce back. Occam's Razor be damned
yesterday, 2 cases, the first severe respiratory distress and chest pain. Turns out to be pneumonia with adjacent effusion, and b/l PE's, and a large lung nodule suspicious for Ca, despite a story sounding like COPD and lungs too tight to move any air and an CXR looking more like CHF. The second, suicide attempt, by alcohol and xanies and oxy's (the latter 2 stolen), with aspiration pneumonitis, and tylenol level >100 (despite the PD and paramedics finding no tylenol containing bottles), and then the urine hcg popped up positive, lol. Clinical Policies/Policy Statements
So I hear a lot that you should be well versed with the official policies of the major organizations relevant to your practice. Not necessarily that you have to abide by them but that you need to be familiar with them and know very clearly when you are deviating and why because if you end up in court, it's the policies that will be cited as evidence of your malpractice. So then I have a few questions. Is there anywhere to get a compilation of the current ACEP policies or do you have to pull them off the website one-by-one? Fev's up but switching meds??
<P>Hi! It's been a while since I have posted anything but I always read these forums. I was wondering, my daughter's pfts were the best they have ever been at fev 112%. I was floored!! She had just been on prednisone for 5 days but still! Anyways, she keeps having episodes of needing to be on steroids so the pulmo thinks we need to change her meds from advair to symbicort. What I dont understand is why if her pfts were so good? Why would she keep having these "episodes" if her asthma is improving??
