Endocrine Surgery
Early in my training I was taught that the most important localizing process in parathyroid surgery was to localize a surgeon who knew how to effectively perform a parathyroidectomy. This advice was readily expanded to most of the other endocrine operations as well. While certainly a tongue-in-cheek pearl of wisdom, it turns out to be true. Primary Hyperparathyroidism
Primary hyperparathyroidism (PHPT) is caused by the overproduction of parathyroid hormone by 1 or more parathyroid glands resulting in hypercalcemia and its downstream clinical consequences. The definitive management of PHPT is surgery, which is termed parathyroid exploration. Approaches to successful surgery include bilateral exploration or focused parathyroidectomy with intraoperative parathyroid hormone monitoring, which in experienced hands are associated with a low risk of complications.
