Transplant tourism - a missed opportunity
Curr Opin Organ Transplant. 2023 Oct 4. doi: 10.1097/MOT.0000000000001108. Online ahead of print.ABSTRACTPURPOSE OF REVIEW: To explore the resurgence of transplant tourism (TT) despite the recent reiteration of the Declaration of Istanbul (DoI) in 2018. As demand grows exponentially and supply remains static, novel approaches to bridging the gap should be explored.RECENT FINDINGS: TT is estimated to comprise up to 10% of transplants worldwide. Prosecuting patients seeking organs through TT has been unsuccessful. Extra jurisdictional prosecution of brokers, vendors and institutions participating in illicit TT has been diffi...

Mice grown with rat brains to help study cross-species organ donation
Creating mice with partial rat brains helps scientists better understand whether synthesising embryos from human and pig cells to grow organs such as kidneys could accidentally lead to pigs with human-like brains (Source: New Scientist - Health)

How do we maximize organs available for transplantation —the role of normothermic machine perfusion (NMP)
We present a timely review of this fascinating topic by Dr Griffin and colleagues from Newcastle University. They outline some of the novel therapeutics currently being added to perfusion platforms during NMP experimentally in an attempt to improve organ function and post-transplant outcomes. (Source: QJM)

Acute complication posttransplant: primary allograft dysfunction
Curr Opin Organ Transplant. 2023 Oct 1;28(5):376-383. doi: 10.1097/MOT.0000000000001091. Epub 2023 Aug 9.ABSTRACTPURPOSE OF REVIEW: Heart transplant is the gold standard treatment for patients with end-stage heart failure, improving both quality of life and survival. Despite advances in donor and recipient management, primary graft dysfunction (PGD) remains the most common cause of morbidity and mortality in the early posttransplant period. This review summarizes recent discoveries in the underlying pathophysiology, risk prediction and management of PGD.RECENT FINDINGS: The incidence of PGD appears to be rising and it is n...

Increased-Risk Versus Standard-Risk Donation in Lung Transplantation: A United Network of Organ Sharing Analysis
Donors with characteristics that increase risk of HBV, HCV, and HIV transmission are deemed increased-risk donors (IRD) per Public Health Service guidelines. Compared to organs from standard-risk donors (SRDs), IRD organs are more frequently declined. We sought to investigate the outcomes of IRD lung transplant recipients following the 2013 guideline change. (Source: The Journal of Thoracic and Cardiovascular Surgery)

Hypothermic Machine Perfusion Preservation: a Proof of Concept for Uterus Transplantation.
To describe the feasibility of Hypothermic Machine Perfusion (HPM) in Uterus Transplantation (UT) to potentially improve the preservation of the uterus to enhance graft preservation in the DBD context.UT is a new surgical approach to treat absolute uterus infertility, it can be performed after living donation (LD) or after Donation after Brainstem Death (DBD). In DBD context, uterus is typically the last organ removed after other vital organs with the exception of the Baylor team, which removes the uterus first. (Source: Fertility and Sterility)

Kidney transplant in the next decade: Strategies, challenges and vision of the future
The objectives, thus, to optimize this situation in the next decade are fundamentally focused on minimizing the rate of kidney graft loss, improving patient survival, increasing the rate of organ procurement and its distribution, promoting research and training in health professionals and the development of scientific registries providing clinical and reliable information that allow us to optimize our clinical practice in the field of KT. With this perspective, this review will deep into: (1) strategies to avoid chronic dysfunction and graft loss in the medium and long term; (2) to prolong patient survival; (3) strategies ...
