Novel Strategies for Optimization of the Pre-transplant Donor Lung
AbstractPurpose of ReviewWhile the number of lung transplants continues to increase, there remains an imbalance between potential lung transplant candidates and acceptable donor lungs resulting in excess mortality. We sought to describe the current state of donor acceptance practices and highlight novel approaches that have the potential to result in an increase in acceptable lungs available for transplant.Recent FindingsIncreased utilization of lungs from donors with positive nucleic acid amplification tests for hepatitis C, donation after circulatory death, lungs conditioned for transplant at Centralized Lung Evaluation ...

The Current Status of Organ Recovery Surgeon Remuneration in the United States
AbstractPurpose of ReviewRemuneration for surgeons who procure organs for transplantation from deceased donors is highly variable throughout the country and not standardized by the Centers for Medicare& Medicaid Services (CMS). CMS does not grant formal relative value units (RVUs) for organ recoveries but some transplant centers award “non-standardized” RVUs in an ad hoc fashion.Recent FindingsA REDCap survey was sent out by the Business Practice Committee of the American Society of Transplant Surgeons to understand the practices of individual transplant centers regarding payment for transplant-related procedures. ...

Case report: A successful clinical experience of transplantation of liver and kidney from a donor with myelodysplastic syndromes
This report provides support for the careful use of marginal donors. With effective treatment and full peer assessment, livers and kidneys from donors with a past history of MDS may be safely transplanted. (Source: Frontiers in Immunology)

Donation After Circulatory Death Heart Transplantation: A Narrative Review
Heart transplantation is the definitive treatment for refractory, end-stage heart failure. The number of patients awaiting transplantation far exceeds available organs. In an effort to expand the donor pool, donation after circulatory death (DCD) heart transplantation has garnered renewed interest. Unlike, donation after brain death, DCD donors do not meet criteria for brain death and are dependent on life-sustaining therapies. Procurement can include a direct strategy or normothermic regional perfusion, whereby there is restoration of perfusion to the organ prior to explantation. Normothermic Regional Perfusion to Expand Liver Transplant From cDCD
Controlled donation after circulatory death (cDCD) donors, who are declared dead following cardiorespiratory arrest, are a growing source of organs. DCD is the best field for donor expansion to help to alleviate the organ shortage. Already in 2009, the American Society of Transplant Surgeons recommended focusing efforts on increasing the number of organs from cDCD. However, in the US, only 11.4% of liver recipients who undergo transplant from a deceased donor receive a cDCD liver. Other sources of donor expansion, such as living donors, account for less than 10% of liver donation in US and even less in Europe. (Source: JAMA Surgery)

Lung Transplant Graft Outcomes from Organs Recovered in Independent versus Hospital-Based Donor Care Units
Purpose: In the United States, deceased organ donors may be transferred to one of two types of donor care units (DCUs) for organ recovery: independent or hospital-based. Donation and transplantation outcomes have not been compared between these types, which may differ in staffing, resources, or management processes. We hypothesized that lung transplant graft survival would not differ between donors in independent vs hospital-based DCUs. (Source: The Journal of Heart and Lung Transplantation)

Trend in Lung Donation from Circulatory Death Donors in European Countries
Purpose: Organ shortage is a major issue for solid organ transplantations, particularly for lung (LTx), leading to reduction in recipient viability and candidate mortality. Most organs come from Brain Death Donors (DBD), however in countries where a legal framework exists, donation from Circulatory Death Donors (DCD) increases number of available organs and contribute to addressing organ shortage. Early Outcomes and Trends in Use of Donation After Circulatory Death (DCD) Donors for Multi-Organ Heart Transplantation (HT) in US
Purpose: DCD-donors have become an important source of organs for HT, but there is limited data regarding their use for multi-organ HT. We evaluated the trend and early outcomes of DCD-donor use in multi-organ HT. (Source: The Journal of Heart and Lung Transplantation)

Case report: Organ donation after euthanasia for psychiatric suffering: some of the practical and ethical lessons Martijn taught us
In conclusion, the article advocates for a nuanced approach, respecting patients' altruistic wishes while at the same time addressing challenges associated with ODE in psychiatric suffering. Where possible, and within the current medical, ethical and legal boundaries, the importance of facilitating organ donation without unnecessarily prolonging the suffering of competent psychiatric patients seeking euthanasia is emphasized. The topic calls, for example, for further qualitative research to understand the stakeholders' perspectives to determine the perceived possibilities on the one hand and boundaries on the other.PMID:38...

Normothermic regional perfusion procurement for abdominal organ donors: techniques and troubleshooting
Curr Opin Organ Transplant. 2024 Mar 12. doi: 10.1097/MOT.0000000000001140. Online ahead of print.ABSTRACTPURPOSE OF REVIEW: Normothermic regional perfusion (NRP) is a novel procurement technique for donation after circulatory death (DCD) in the United States. It was pioneered by cardiothoracic surgery programs and is now being applied to abdominal-only organ donors by abdominal transplant programs. Multiple technical approaches can be used for abdominal-only NRP DCD donors and this review describes these techniques.RECENT FINDINGS: NRP has been associated with higher utilization of organs, particularly liver and heart gra...
