Pulmonary Embolism in a Liver Donor: A Case Report and Review of the Literature
Exp Clin Transplant. 2023 Jul 27. doi: 10.6002/ect.2023.0098. Online ahead of print.ABSTRACTLiver transplant is an important treatment option for end-stage liver disease, and living related donation is an option to shorten or eliminate the waiting period for the patients, especially when shortage of organs is of concern. It is crucial to provide optimal safety for the donors and to thoroughly examine them preope-ratively in order to decrease perioperative and postoperative complications. Here, we report the case of a living donor who had undergone a left liver lobectomy and on postoperative day 2 presented with a radiologi...

Ethical and legal issues associated with organ donation and transplantation
Organ donation and transplantation has transformed the lives of many around the world. Despite its place as a treatment option for numerous forms of end-stage organ disease, the need for careful ethical and legal consideration is paramount. In this article we discuss three rules: the Dead Donor Rule, which underpins the use of organs for donation from deceased individuals; the ‘Rule of Consent’ which includes consideration of both deceased and living donors and the recipients of organs; and finally the ‘Fair Transplant Rule’, which offers protection from the challenging issues of exploitation and organ allocation f...

Organ retrieval and preservation
The success of organ transplantation can be attributed to many factors but ultimately depends upon retrieval and preservation techniques to maintain the quality of an organ. Thoracic and abdominal organs from deceased donors are retrieved during a multi-organ procedure. The organs are flushed and cooled in  situ with preservation solution followed by dissection and removal. With high-risk donors, such as donation after circulatory death, rapid in-situ cooling is essential to minimize the injury. Live kidney donation is becoming increasingly popular and the disincentives have been significantly reduce d by the introduction...

A critique of whole body gestational donation
AbstractIn her controversial paper, Anna Smajdor proposes that brain-dead people could be used as gestation units for prospective parents unable or unwilling to undertake the act themselves —what she terms whole body gestational donation (WBGD). She explores the ethical issues of such an idea and, comparing it with traditional organ donation, asserts that such deceased surrogacy could be a way of outsourcing pregnancy’s harms to a populace unable to be affected by them. She argues that if the prospect is unacceptable, this may reveal some underlying problems with traditional cadaveric organ donation. Smajdor’s analys...

Imaging Evaluation of the Living Liver Donor: A Systems-Based Approach
Radiol Clin North Am. 2023 Sep;61(5):771-784. doi: 10.1016/j.rcl.2023.03.002. Epub 2023 May 5.ABSTRACTLiver transplant is the definitive treatment of end-stage liver disease and early hepatocellular carcinoma. The number of liver transplant surgeries done is highly affected by the number and availability of deceased donor organs. Living donor liver transplantation has emerged as an alternative source of donors, increasing the availability of organs for transplant. Many factors must be considered when choosing living donor candidates to maintain a high level of donor safety and organ survival. To that end, potential donors ...
