Facial Nerve Palsy After Chimeric Antigen Receptor T-Cell Therapy
A man in his 60s with relapsed/refractory IgG lambda multiple myeloma underwent chimeric antigen receptor T (CART) cell therapy. Twenty-five days after infusion, he presented with new right-sided facial numbness. Physical examination showed right-sided facial paralysis (Figure  1). A magnetic resonance image of the brain with and without contrast enhancement showed new increased enhancement of the bilateral facial nerves (Figure 2). PAX8-Positive (Gynecologic-Type) Clear Cell Carcinoma Metastatic to the Kidney
A woman in her late 60s presented with a 3.2-cm mass involving both the liver and the right kidney. Histopathologic evaluation showed the presence of a tumor with clear cytoplasm (Figure  1A) that was diffusely positive for the paired box 8 (PAX8) transcription factor, which is frequently used to establish renal histogenesis (Figure 1B). Although the presence of a PAX8-positive tumor with clear cytoplasm suggested a clear cell renal cell carcinoma that was primary to the kidney, t his tumor had some unusual morphologic features. 5-Lead Electrocardiogram Monitoring to Detect Perioperative Myocardial Infarction, Noncardiac Surgery Patients
Perioperative myocardial infarction (MI) can have a mortality rate upward of 25%. The 5-lead electrocardiogram (ECG) is one of the standard basic anesthesia monitors.1 The V5 lead is the most sensitive lead of the ECG for detecting MI.2 The diagnosis of MI usually requires an elevation in the cardiac troponin concentrations above the 99th percentile with a rising or falling pattern accompanied by clinical evidence of acute myocardial ischemia. Sleepwalking Into a Risky Path: Expanding the Concerns for Parasomnias in the Elderly
Parasomnias are abnormal behaviors occurring during sleep. Obesity: Central Role in Heart Failure With Preserved Ejection Fraction?
The prevalence and incidence of heart failure with preserved ejection fraction (HFpEF) have steadily risen during the past 10 years, especially in the United States. About half of heart failure (HF) diagnoses are HFpEF, with a prevalence of 1% each year.1 In addition, it remains an important public health issue as HF care is projected to cost $69.8 billion by 2030.1 Mortality in HFpEF also remains substantial, with a recent estimate of 33.3% 1-year mortality in patients hospitalized for acute decompensated HFpEF.
