71-Year-Old Man With Progressive Dyspnea on Exertion
A 71-year-old man presented to the emergency department complaining of 5 months of slowly progressive exertional dyspnea, worsened in the past 3 days. He was a lifelong nonsmoker who had been generally healthy until 6 months before presentation, when nonsevere COVID-19 infection developed. One month later, he suffered a fall with cervical spinal trauma, and thereafter he experienced persistent neck pain. Parsonage-Turner Syndrome and Hereditary Brachial Plexus Neuropathy
Parsonage-Turner syndrome and hereditary brachial plexus neuropathy (HBPN) present with indistinguishable attacks of rapid-onset severe shoulder and arm pain, disabling weakness, and early muscle atrophy. Their combined incidence ranges from 3 to 100 in 100,000 persons per year. Dominant mutations of SEPT9 are the only known mutations responsible for HBPN. Parsonage and Turner termed the disorder “brachial neuralgic amyotrophy,” highlighting neuropathic pain and muscle atrophy. Does the Expanded Controlled Evidence Now Mandate the Routine Use of Machine Perfusion in Kidney Transplantation?
The quality of donor kidneys may be improved by taking therapeutic measures that begin before transplant. A recent large-scale prospective trial from the United States by Malinoski et  al1 reported that machine perfusion (MP) was superior to targeted donor hypothermia in reducing delayed graft function (DGF) after kidney transplant from a brain-dead donor (DBD). Delayed graft function was defined as need for dialysis during the first 7 days after transplant. However, the trial f ound no measurable effect on 1-year graft survival. Blood Volume Explains the Relationship of Lean Body Mass With the Cardiovascular System in Men but Not in Women
Lean body mass (LBM) and the functional capacity of the cardiovascular (CV) system, including the strongest prognostic factors for mortality, were recently discovered to constitute a robust relationship in humans.1-3 Essentially any fundamental phenotypic attribute of the main pumping chamber (ie, the left ventricle) determining the capacity to deliver and consume oxygen is associated with LBM independently of adiposity and established CV risk factors.1-3 Thus far, these findings apply to European American women and Hans Chinese women and men, the latter being characterized by low (female-like) LBM.
