[Vision Loss in Association with Silicone Oil].
Authors: Tode J, Roider J Abstract Silicone oil tamponades have been a key feature of vitreoretinal surgery for more than 40 years. The use of these durable vitreous substitutes has not only been limited to tractive retinal detachments, inflammatory chorioretinopathies or other complicated vitreoretinal diseases, but also to macular holes and rhegmatogenous retinal detachment. During the last 12 years, an increasing number of cases of vision loss during or after the removal of silicone oil have been reported in the literature. In up to one third of patients with uncomplicated macula-attached retinal detachment with...

[Botulinum Toxin - New Developments in Ophthalmology].
Authors: Wabbels B Abstract Botulinum toxin is recognised as the gold standard for the treatment of essential blepharospasm and hemifacial spasm, which is similar in effect in synkinesis after facial nerve palsy. The injection intervals can be adjusted according to the patients' needs and be shortened for up to six weeks in cases of eyelid cramping. Newer indications for the use of botulinum toxin in ophthalmology include eyelid retraction in Graves' disease, induction of protective ptosis and treatment of crocodile tears syndrome after facial nerve palsy. In future, botulinum toxin may be used in depression (facia...

[Genome Editing Tools and their Application in Experimental Ophthalmology].
Authors: Yanik M, Wende W, Stieger K Abstract New genome editing tools in molecular biology are revolutionising precise genome surgery and have greatly influenced experimental ophthalmology too. Aside from the commonly used nuclease-based platforms, such as the zinc-finger nucleases (ZFN) and transcription activator-like effector nucleases (TALEN), CRISPR/Cas systems, clustered regularly interspaced short palindromic repeats (CRISPR) and CRISPR-associated (Cas) genes, perform very efficiently in site-specific DNA cleavage within living cells. DNA double strand breaks (DSB) are repaired through two different conserv...

[Development of Endothelial Cell Density after Penetrating Keratoplasty in Patients with Fuchs Dystrophy or Keratoconus - Comparison of Excimer Laser and Mechanical Trephination].
Conclusion During the first 24 months after PKP, a significantly higher ECD and a significantly lower percentage of ECL per year was observed after EXC trephination for the entire group of patients. For the different diagnostic groups KC, FD and OD, no significant difference in ECD or ECL loss was noticed over a range of follow-up intervals. This may most likely be attributed to the small number of patients in the three subgroups. PMID: 28086248 [PubMed - as supplied by publisher] (Source: Klinische Monatsblatter fur Augenheilkunde)

[New Developments in Cataract Surgery].
Authors: Dick HB, Schultz T Abstract Recent technological innovations in cataract surgery have made the procedure even more precise and safe and the odds of having a highly satisfied patient even higher. These innovations include visualisation systems - such as intraoperative aberrometry - which are particularly helpful when it comes to implanting toric IOLs, where even a slight rotation or misalignment can significantly reduce the postoperative visual quality. Another way to ensure the exact positioning of a toric IOL is to create a mark by making an intrastromal incision using the femtosecond laser. The latter te...

[Current Status of Electronic "Continuous" Eye Pressure Measurements].
[Current Status of Electronic "Continuous" Eye Pressure Measurements]. Klin Monbl Augenheilkd. 2017 Jan 13;: Authors: Faschinger C Abstract Intraocular pressure (IOP) is generally measured at individual points and usually only once, but rarely more often, during office hours and with various different instruments. This is probably inadequate for diagnosis, assessment of progression, and therapeutic decision making, since the IOP is not a static measurement, but rather one that is subject to greater or lesser fluctuations over shorter or longer periods. This has prompted the desire for more re...

[Music and Glaucoma].
Authors: Plange N Abstract Music may have multiple influences on the human organism. A possible therapeutic effect for patients with glaucoma has been postulated, aside from the known impact of music on the cardiovascular system, psychogenic effects and a short-term improvement in mental performance (Mozart effect). The higher level of mental stress in patients with glaucoma and type-A personality behaviour may be related to higher intraocular pressure in patients with glaucoma. Relaxing music may have a positive impact in these patients, related to a reduction in intraocular pressure or its fluctuations. However, ...

[Patient Positioning after Intraocular Surgery].
Authors: Prahs PM, Märker D, Helbig H Abstract Gas tamponades are widely used during intraocular surgery. The high surface tension at the intraocular liquid to gas interface is used in retinal detachment and macular surgery, as well as in lamellar corneal transplant surgery. The patient's postoperative posture determines the position of the gas bubble inside the eye. The article describes the development, physical laws and methods for postoperative positioning after intraocular surgery. Authors: Rüfer F Abstract Traumatic changes in the iridocorneal angle, e.g. in ball sports, can lead to secondary glaucoma. High intensity resistance exercise or yoga exercises - such as the headstand - can increase IOP, and deterioration in the visual field and acute narrow angle glaucoma attacks have been described in some case reports. Glaucoma therapy of professional athletes with steroids, carbonic anhydrase inhibitors and beta-blockers can result in positive doping tests. In manifest glaucoma, moderate aerobic exercise is presumably of use because of the IOP lowering effect, improved retinal perfusion an...

Refractive Lenticule Implantation for Correction of Ametropia: Case Reports and Literature Review.
Authors: Lazaridis A, Messerschmidt-Roth A, Sekundo W, Schulze S Abstract The ReLEx® technique allows correction of refractive errors through the creation and extraction of refractive stromal lenticules. Contrary to excimer laser corneal refractive procedures, where the stromal tissue is photoablated, the extracted lenticules obtained with ReLEx® can be preserved. Recent studies and case reports have described autologous re-implantation and allogeneic implantation of refractive lenticules into femtosecond-laser created stromal pockets in order to reverse the refractive outcome of a myopic corneal refractive...

[Imaging Blood Flow and Pulsation of Retinal Vessels with Full-Field Swept-Source OCT].
Authors: Spahr H, Hillmann D, Hain C, Pfäffle C, Sudkamp H, Franke G, Koch P, Hüttmann G Abstract Optical coherence tomography (OCT) uses interference to image the retina with high axial resolution. In the last 25 years, new technologies have permitted a steady increase in imaging speed, which made it possible to enlarge the imaged field and to avoid motion artefacts. The speed and precision of retinal imaging is now limited by photodamage of the retina caused by the focused OCT beam and by the speed of the scanning mechanics. Full-field swept-source (FF-SS)-OCT decreases irradiance on the retina and disp...

This article presents the basic technological principles of retinal prostheses, using an epiretinal implantable visual prosthesis as example. An implantable visual prosthesis typically consists of extraocular and intraocular sections. The extraocular section is responsible for detecting the image. The information is derived from this image that is needed for appropriate electrical stimulation of the retinal nerve cells. Together with the necessary energy, these data are transmitted through a magnetic connection to the intraocular section. To ensure reliable transmission, the data are encoded. It is therefore essential to select the appropriate scan modalities to detect all morphological changes in different diseases, not only at the VRI, but also in all layers of the retina and in the foveal and parafoveal areas. These can be raster scans, radial scans or "en face" scans. Morphological changes at the VRI and especially in the outer retinal layers are good prognostic factors in high resolution OCT for the success of surgery for vitreomacular interface disord...

The frequency of congenital colour vision deficiencies has been investigated in rail and navy staff since the 1870s. Various test methods have been developed. Wool tests, flor contrast tests and colour plate tests have been used. A published colour plate test, based on Stilling's pseudo-isochromatic plates in combination with a flor contrast test, has been a common screening method for colour vision testing in German-speaking countries. This test is intended to detect congenital ...
