Evolution of Keratoconus: From Diagnosis to Therapeutics.
Authors: Imbornoni LM, McGhee CNJ, Belin MW Abstract This review describes the evolution of the diagnosis and treatment of keratoconus from the earliest written description to present day. The first description was provided in 1736 by Benedict Duddell who described the prominent corneas of a fourteen-year-old boy. Throughout the 19th century, a variety of surgical procedures were proposed to manage the disease, such as surgically repositioning the pupil away from the cone, iris incarceration to produce a slit-like pupil, cauterization of the cone to produce a scar, and full thickness elliptical excision of the cone...

The Use of Optical Coherence Tomography for the Detection of Early Diabetic Retinopathy.
Authors: Somfai GM, Gerding H, DeBuc DC Abstract Diabetic retinopathy (DR) is one of the leading causes of vision loss globally with a severe burden on all societies due to its high treatment and rehabilitation costs. The early diagnosis of DR may provide preventive steps (including retinal laser therapy and tight carbohydrate, blood pressure, and cholesterol control) that could in turn help to avoid progression of the pathology with the resultant vision loss. Optical coherence tomography (OCT) enables the in vivo structural imaging of the retina, providing both qualitative (structure) and quantitative (thickness) ...

12-Year Outcomes of Microkeratome-Assisted Anterior Lamellar Therapeutic Keratoplasty (ALTK) for Disorders of the Anterior Part of the Corneal Stroma - A Comparative Review of Adult and Children.
CONCLUSIONS: This study shows that anterior lamellar keratoplasty in children retains good visual function when combined with adequate amblyopic therapy. However, the rate of complications is higher in juveniles and requires more intensive interdisciplinary follow-up. [Capsulorhexis Real-Life - Experienced Surgeon versus Femto Laser Assisted Cataract Surgery (FLACS) as Reported in Literature].
CONCLUSIONS: The overlap rate of manual surgery was comparable to FLACS; whether the slightly 'rounder' capsulotomy is clinically relevant remains unclear. The risk of an anterior capsular tear is very low in the hands of an experienced surgeon and even lower than with FLACS. Need for Quantitative Measurement Methods for Posterior Uveitis: Comparison of Dual FA/ICGA Angiography, EDI-OCT Choroidal Thickness and SUN Vitreous Haze Evaluation in Stromal Choroiditis.
Authors: Fabro F, Herbort CP Abstract BACKGROUND/PURPOSE: Quantitative methods for posterior uveitis are necessary for precise appraisal and follow-up of inflammation in practice and in clinical trials. The aim of this study was to assess fluorescein angiography (FA), indocanine green angiography (ICGA), and enhanced depth imaging optical coherence tomography choroidal thickness (EDI-OCT CT) in two stromal choroiditis entities, birdshot retinochoroiditis (BRC), and Vogt-Koyanagi-Harada disease (VKH), as well as to determine (1) disease patterns, (2) respective response to therapy, and (3) their potential utility in...

[Predictive Value of OCT-Controlled Early Response of Macular Edema Secondary to Retinal Vein Occlusion After Intravitreal Bevacizumab Therapy].
CONCLUSION: An OCT examination, 24 hours after the first bevacizumab injection, has a prognostic value and can identify the patients that can tolerate longer intervals between injections, this results in fewer bevacizumab injections during the course of the disease. This way an individualization of the treatment, opposed to a fixed therapy pattern, can be implemented. Also, patients with poor prognosis can be identified early and treatment can be altered or intensified accordingly. Nevertheless, the results plea for a nation-wide survey to be able to representatively formulate demand-orientated approaches for improvement concerning the ophthalmic surgery training in Germany. [Therapeutic Concepts for Treatment of Patients with Non-infectious Uveitis Biologic Disease Modifying Antirheumatic Drugs].
Authors: Walscheid K, Pleyer U, Heiligenhaus A Abstract Biologic disease modifying antirheumatic drugs (bDMARDs) can be highly efficient in the treatment of various non-infectious uveitis entities. Currently, the TNF-α-inhibitor Adalimumab is the only in-label therapeutic option, whereas, all other bDMARDs need to be given as an off-label therapy. bDMARDs are indicated in diseases refractory to conventional synthetic DMARD therapy and/or systemic steroids, or in patients in whom treatment with those is not possible due to side effects. Therapeutic mechanisms currently employed are cytokine-specific (interfero...

Authors: Mann C, Anders F, Liu H, Brockhaus K, Liu A, Grus FH, Pfeiffer N, Thanos S, Prokosch V Abstract INTRODUCTION: Glaucoma is characterised by progressive loss of retinal ganglion cells and axons. Experimental research has concentrated on understanding the pathophysiological mechanisms involved in glaucomatous damage. It is still a matter of debate whether neurons or capillaries are primarily damaged by elevated intraocular pressure (IOP). The aim of this study was to detect IOP-induced vascular changes in the vessels of the optic nerve head and the main vessels of the retina in vivo. METHODS: Experimental...

[Ciliopathies].
Authors: Gerth-Kahlert C, Koller S Abstract Ciliopathies are disorders caused by ciliary dysfunction and can affect an organ system or tissues. Isolated or syndromic retinal dystrophies are the most common ocular manifestation of ciliopathies. The photoreceptor connecting cilium plays a leading role in these ciliopathy-related retinal dystrophies. Dysfunctional photoreceptor cilia cause the most severe type of retinal dystrophy: Leber's congenital amaurosis (LCA). The most common syndromic ciliopathies with an ocular manifestation are Bardet-Biedl syndrome (BBS) and Usher syndrome. Molecular-genetic analysis reveal...

Authors: Wolfrum U, Nagel-Wolfrum K Abstract The human Usher syndrome (USH) is a complex, rare disease manifesting in its most common form of inherited deaf-blindness. Due to the heterogeneous manifestation of the clinical symptoms, three clinical types (USH1-3) are distinguished according to the severity of the disease pattern. For a correct diagnosis, in addition to the auditory tests in early newborn screening, ophthalmological examinations and molecular genetic analysis are important. Ten known USH genes encode proteins, which are from heterogeneous protein families, interact in functional protein networks. In ...

Authors: Steffen H Abstract Non-organic visual loss is a frequent challenge in ophthalmic practice. In children most patients are prepubertal females. The most frequent manifestation in this age-group is a bilateral unexplained decrease of visual acuity. In adulthood patients often indicate a degree of visual loss after an accident or a disease - that could not be explained by the eye condition alone. Non-organic visual loss may be frustrating for both, patients and physicians.
