[Minimally invasive Therapy of lacrimal Drainage System Stenosis].
Authors: Avgitidou G, Koch KR, Heindl LM Abstract Epiphora is one of the most common symptoms for clinical ophthalmological consultation. This symptom is mostly caused by stenosis of the lacrimal drainage system. Over a century, external dacryocystostomy according to Toti was the treatment of choice. Today, new very encouraging minimally invasive techniques for recanalisation of the lacrimal drainage system, such as microdrilldacryoplasty or laserdacryoplasty, as well as bypass-based techniques, such as transcanalicular laser-assisted dacryocystorhinostomy, become more important in the treatment of dacryostenosis. ...

This article is a review of diagnostic and therapeutic possibilities in common epibulbar malignant tumors, such as basal cell carcinoma, conjunctival lymphoma, squamous cell carcinoma and conjunctival melanoma. Most importantly, for every tumor patient there is a detailed anamnesis, split lamp examination and photo documentation. Further regular controls after therapy are required due to the risk of recurrences.Basal cell carcinoma is the most common tumor of the periocular skin, divided into three subtypes (nodular, superficial and morphea). The treatment consists of total excision with a sufficient safety margin. The most...

[Healthcare Research into Anti-VEGF Therapy: Selection and Methodological Precautions].
Authors: Ziemssen F, Stahl A, Dimopoulos S Abstract Health care research has emerged as an approach to assess and improve quality of care and patient outcomes in the real world. It also has the potential to reduce healthcare costs by providing evidence to guide healthcare decisions.Randomised controlled trials (RCTs) theoretically offer the ideal study design to support treatment decisions. In RCTs, randomisation (formal chance) determines treatment allocation, which prevents selection bias from distorting the parameters of anti-VEGF treatment effects. Despite this advantage, only a minority of patients qualify for...

[Do We Perform Glaucoma Surgery too Late?]
This study reveals that almost ¾ of patients are referred rather late for glaucoma surgery. Criteria for adequate timing of glaucoma surgery are given for non-glaucoma specialists in particular. They apply for all types of glaucoma surgery, and the pros and cons are discussed in detail. Trabeculectomy is still regarded as the gold standard of glaucoma surgery. Because trabeculectomy may not be an ideal glaucoma procedure, there is hope that the modern less-invasive glaucoma procedures (MIGS) will be able to extend indications for glaucoma surgery. This may also lower the hurdle of referral for glaucoma surgery for b...

[Causes, Diagnosis and Therapy of Negative Dysphotopsia].
Authors: Wenzel M, Langenbucher A, Eppig T Abstract Negative Dysphotopsia (ND) is a dark, sometimes rounded bar or shadow in the temporal visual field after uncomplicated cataract surgery that may lead to great patient discomfort. R. Olson called it "the number one troublesome complaint after uneventful cataract surgery". The origin of this phenomena remains uncertain and is discussed conversely in literature. We have found evidence that it may be caused by a slight difference in magnification caused by the new IOL compared to the old crystalline lens. This may lead to a minimal shift of the object space ...

It is based on the preoperative anterior chamber depth (ACD) and axial length (L). Material and Methods Differences between IOL constant triplets can be judged based on their statistical measurement uncertainty. Authors: Kirchhof B Abstract Robert Machemer offers a surgical approach to age-related macular degeneration with his retinal rotation. There is already considerable experimental and clinical knowledge available in Europe and the US on transplantation techniques for age-related macular degeneration. On average, initial visual acuity can be preserved. When photodynamic therapy was standard for exudative AMD, transplantation was superior. Photodynamic therapy could not stop, but was instead able to slow down visual loss. Currently, VEGF-blocker therapy has priority, because the visual acuity can be improved. However, ...

[Retinal Vascular Occlusion: Modern Therapeutic Approaches].
Authors: Pettenkofer M, Feltgen N, Feucht N, Lohmann CP, Maier MM Abstract Retinal artery occlusion (RAO) should be evaluated as an emergency and can be seen as an ocular analogue to the cerebral apoplex. Both have the same arteriosclerotic risk factors, which are also responsible for cardiac, circulatory and cerebrovascular diseases. That is why an intensive interdisciplinary clarification is necessary to recognize possible comorbidities in time and, if necessary, to treat them. The current therapeutic possibilities of an acute RAO are very limited in their efficiency regarding visual improvement. Methods for syst...

Authors: Blatsios G, Haas G, Zehetner C, Bechrakis NE Abstract Significant advancements have been made in the last decades in the treatment of uveal melanoma. Radiation therapy can achieve, depending on the method, local tumour control in up to 98% of cases. Surgical modalities have been implemented in the ocular therapy for uveal melanoma either to treat/prevent radiation complications, such as so-called toxic tumour syndrome, or to primarily treat the tumour with or without adjunctive radiotherapy. These include endoresection and transscleral resection for melanomas of the choroid/ciliary body or irido-trabeculo-...

Further development of imaging techniques makes fundus autofluorescence (FAF) imaging a safe, non-invasive, easy-to-perform and reproducible diagnostic tool. FAF uses the autofluorescent properties of tissues, in particular the retinal pigment epithelium (RPE) and its fluorophores. FAF images display phenomena of normal aging as well as disease-related changes of the fundus, but also can be used for monitoring retinal diseases and therapy. After a short introduc...

Furthermore, the functional success was never comparable to intravitreal drug therapy, and the manoeuvres are no longer used in clinical routine. The procedures, the surgical theory and the criticism on vitrectomy, radial optic neurotomy (RON), retinal endovascular fibrinolysis (REVL) and arteriovenous dissection (AVD) will be discussed in this paper. One of the most important risk factors for developing keratoconus is chronic eye rubbing which, apart from other known risk factors such as atopy or Down's syndrome, is the only modifiable factor.
