[Cost Comparison of Licensed Intravitreal Therapies for Insufficiently Anti-VEGF Responding Fovea Involving Diabetic Macular Edema in Germany].
CONCLUSIONS: In summary, the short-term cost-cost comparison demonstrates that steroid implants can provide significant cost savings versus in-label anti-VEGF treatment for center-involving diabetic macular edema. Single application of the long-lasting Fluocinolone acetonide implant is the most cost-efficient in-label treatment option. Botulinum Toxin for the Treatment of Cyclic Strabismus in Children: Three Case Reports.
Authors: Wipf M, Bok-Beaube C, Palmowski-Wolfe A Abstract Cyclic strabismus is a rare disease of unknown origin. If untreated, it leads to manifest strabismus with the risk of amblyopia in children. Treatment is generally surgical. Here we report on three children in whom cyclic esotropia was successfully treated with one bimedial injection of 5 IU Botox®. All patients remained orthotropic with good stereo functions at the last follow-ups at 16, 11, and 8 months. Botulinum toxin offered a minimally invasive treatment option in these patients. [Personalized Ophthalmology - Induced Pluripotent Stem Cells for In Vitro Modelling of Retinal Degenerative Diseases].
Authors: Brandl C, Weber BHF Abstract Today, the search for therapeutic options to treat retinal degeneration often relies on an in-depth understanding of the underlying pathological events. Alternatively, it is conceivable to search, in an undirected screening approach, for chemical compounds affecting disease outcome. For both approaches, there is an urgent need for in vitro and, ideally, in vivo disease models that adequately reflect the site of pathology. Currently available animal models possess limitations as they often develop only defined aspects of disease. Primary cell cultures, derived from the posterior...

Macular Edema in Childhood Uveitis.
CONCLUSION: Early diagnosis and strict control of inflammatory activity have led to a dramatic reduction in the incidence of vision-threatening secondary complications. In the majority of cases, it has also been possible to resolve cystoid macular edema, which, if insufficiently controlled by systemic therapy, usually responds well to intravitreal dexamethasone implants. [Significance of Helicobacter pylori Diagnostic in Glaucoma].
Authors: Joachim SC Abstract Glaucoma is one of the most common causes for blindness worldwide. Even if some risk factors for this disease are known, like increased intraocular pressure or increased age, its pathomechanism is still not fully understood. Multiple studies recently analyzed the role of a Helicobacter pylori infection in the glaucoma pathogenesis. Several studies showed a correlation, while others did not come to this conclusion. In summary, it is very likely that a Helicobacter pylori infection coincides with an increased risk for a primary open-angle or normal-tension glaucoma. Still, larger studies,...

[Significance of Endothelin-1 in Glaucoma - a Short Overview].
Authors: Gugleta K Abstract This review article is focused on the various facets of possible endothelin's role in glaucoma; involvement of endothelin in other ocular, in particular vascular, diseases is not specifically discussed. Endothelin is an ubiquitous molecule that occurs in practically all ocular tissues. Its primary physiological function is regulation of the blood vessel diameter and hence regulation of the blood supply in tissues. It is secreted locally, and exerts its effect also predominantly locally. This limits the value of venous blood sampling for estimation of the endothelin function in a particul...

[The Keyrole of Multiomics in the Predictive, Preventive and Personalised Medical Approach towards Glaucoma Management].
Authors: Golubnitschaja O Abstract Glaucoma is the second leading cause of blindness, with altogether about 70 million patients registered worldwide. These facts prompt us to reconsider currently applied concepts in overall glaucoma management. Innovative strategies of predictive, preventive and personalised medicine (PPPM) are expected to considerably improve disease prevention and personalised treatment. The comprehensive PPPM measures include the application of individualised patient profiles, "phenotyping", molecular characterisation of the pre-lesions and disease stages, innovative screening programs...

[Risk Factors and Therapeutic Goals for Primary and Secondary Cardiovascular Prevention: Significance for the Glaucoma Patient].
Authors: Hacke C, Erb C, Weisser B Abstract Hypertension, diabetes mellitus and dyslipidaemia are among the most important cardiovascular risk factors. In addition, there is an association with the development and progression of glaucoma. During the last years, there has been an intense discussion concerning optimal treatment goals for these risk factors. Recently, the American Heart Association has defined a blood pressure greater than 130/80 mmHg as hypertension and the European guidelines will follow this definition. These lower blood pressure limits increase the risk for a too strict blood pressure treat...

[Despite Challenges and Pitfalls: How Ophthalmology Benefits from the Use of Next-Generation Sequencing].
Authors: Bolz HJ Abstract Within a few years, high-throughput sequencing (next-generation sequencing, NGS) has become a routine method in genetic diagnostics and has largely replaced conventional Sanger sequencing. The complexity of NGS data requires sound bioinformatic analysis: pinpointing the disease-causing variants may be difficult, and erroneous interpretations must be avoided. When looking at the group of retinal dystrophies as an example of eye disorders with extensive genetic heterogeneity, one can clearly say that NGS-based diagnostics yield important information for most patients and physicians, and that...

[Dacryoendoscopic Surgery-Factors Influencing Postoperative Results].
CONCLUSION: Dacryoendoscopy with its minimally invasive approach is suitable as a first-step procedure in lacrimal surgery. The absence of a scar and the quick recovery are vitally important for the patients. Medically important is that the topographic anatomy is preserved by using dacryoendoscopy, so other surgical techniques can be performed later without limitations, if necessary. Patients with incomplete obstructions and a localization of stenosis, being pre- to intrasaccal, profit from this treatment method in particular. [Atropine for the Prevention of Progression in Myopia - Data, Side Effects, Practical Guidelines].
Authors: Schittkowski MP, Sturm V Abstract The prevalence of myopia has increased worldwide in recent decades. In East Asia's metropolises ≥ 80% of young adults are affected. This dramatic increase is mainly caused by changes in lifestyle and behaviour. Atropine has been used for more than 100 years to arrest myopia progression. It has become an evidence-based treatment regimen in the last decade, although the exact mechanism of the effect of treatment is still unknown. Atropine eye drops can slow myopia progression by an average of - 0.54 dioptres (D)/year in Asian children and - 0.35&thins...

[Effect of the Age-Related Corneal Elasticity on Applanation Tonometry].
Authors: Sperlich K, Reiß S, Bohn S, Stolz H, Guthoff RF, Jünemann A, Stachs O Abstract Elevated intraocular pressure (IOP) is accepted to be one important criterion for glaucoma and is usually measured by applanation or rebound tonometry. The individual uncertainty due to central cornea thickness (CCT) is thereby corrected, while the error induced by age-related elastic modulus (EM) change of the cornea is ignored. To investigate its influence on IOP measurement, we derive a model including also the elastic modulus. Our approach is based on known equations from experimental physics and several assumptio...

Conclusion A switch in anti-VEGF therapy is not causally related to better VA outcome. In the statistical evaluation of non-randomised, retrospective case series, special attention must be given to potential confounding variables. [Real-life Data on the Treatment of Diabetic Macular Oedema in Germany].
Conclusions Results of real life treatment in clinical routine are found to fall behind controlled, prospective trials, as found to the findings in other anti-VEGF indications: mean gain in VA is lower than in prospective trials, and the initial gain cannot be fully maintained over a prolonged time period. Similarly, the number of injections received is markedly lower than in controlled trials. This can partially be explained by differences in study populations and a negative selection bias in longer term results. However, additional barriers that hamper timely treatment tailored to disease activity requirements must be id...
