Commentary Regarding: Using Consumer Wearable Devices to Profile Postoperative Complications After Pediatric Appendectomy
This past week, I had the dubious honor of being a patient in the emergency room. Hoping to avoid (or at least defer) seeking medical care, I was finally convinced that my symptoms were not solely attributable to the fatigue of being on call for a week when I looked at my Apple Watch. My resting heart rate, normally in the low 40 s, rose to the 50 s, then 60 s, then to 72. I thought back to my internship, when often the first sign that a patient was becoming septic was an increase in respiratory or cardiac rate. (Source: Journal of Surgical Research)

Association Between Nil Per Os Status and Intubated Patients Undergoing Surgery
Patient outcomes heavily rely on nutritional support. However, holding enteric feeds prior to surgical operations in critically ill patients is still a common practice in intensive critical units. Our objective is to describe the relationship between duration of nil per os (NPO) and respiratory outcomes in intubated, critically ill patients requiring operative intervention. Implementation Requires Evaluation of Adoption: Lessons From a Multimodal Pain Regimen Order Set
Multimodal pain regimen (MMPR) protocols are the standard of care per the 2020 Trauma Quality Improvement Program guidelines. MMPR implementation methodology in trauma services has not been reported. The primary objective of this study was to evaluate the adoption of an MMPR order set at a level 1 trauma center and to describe its implementation. We hypothesized that order set utilization would be about 50%, and barriers to adoption would be related to personal biases. (Source: Journal of Surgical Research)

Surgical Necrotizing Enterocolitis – Can We Predict the Need for Gastrostomy Tube Placement?
Necrotizing enterocolitis (NEC) is a significant cause of morbidity and mortality among extremely premature infants. Approximately 50% of cases progress to surgery, frequently resulting in resection of necrotic bowel and ostomy creation. Premature neonates are at risk for bronchopulmonary dysplasia and feeding failure; surgery in these patients is higher risk. We evaluated the incidence of gastrostomy tube (GT) placement after ostomy reversal in surgical NEC to define a subset of patients who would benefit from concurrent ostomy reversal and GT placement. Attitudes on Artificial Intelligence use in Pediatric Care From Parents of Hospitalized Children
Artificial intelligence (AI) may benefit pediatric healthcare, but it also raises ethical and pragmatic questions. Parental support is important for the advancement of AI in pediatric medicine. However, there is little literature describing parental attitudes toward AI in pediatric healthcare, and existing studies do not represent parents of hospitalized children well. Effectiveness of Mobile Applications for Trauma Care in Combat Casualty Simulations Throughout the Continuum
The U.S. Military uses handwritten documentation throughout the continuum of combat casualty care to document from point-of-injury, during transport and at facilities that provide damage control resuscitation and surgery. Proven impractical due to lack of durability and legibility in arduous tactical environments, we hypothesized that mobile applications would increase accuracy and completeness of documentation in combat casualty simulations. Bilateral Exchange: Enteral Nutrition Clinical Decision Making in Pediatric Surgery Patients
Evidence-based medicine guides clinical decision-making; however, promoting enteral nutrition has historically followed a dogmatic approach in which patients graduate from clear liquids to full liquids to a regular diet after return of bowel function. Enhanced recovery after surgery has demonstrated that early enteral nutrition initiation is associated with shorter hospital stays. We aimed to understand postoperative pediatric nutrition practices in Kenya and the United States. (Source: Journal of Surgical Research)

Early Versus Late Feeding After Percutaneous Endoscopic Gastrostomy Placement in Trauma and Burn
Timing to resume feeds after percutaneous endoscopic gastrostomy (PEG) placement continues to vary among US trauma surgeons. The purpose of this study was to assess differences in meeting nutritional therapy goals and adverse outcomes with early versus late enteral feeding after PEG placement. Timing of Acquisition of Methicillin-Resistant Staphylococcus aureus Nasal Carriage: Can we Limit Repeat Screening?
Broad-spectrum empiric antibiotics are routinely administered to hospitalized patients with potential infections. These antibiotics provide protection; however, they come with their own negative effects. The utility of Methicillin-resistant Staphylococcus aureus (MRSA) polymerase chain reaction (PCR) nasal screening to steward anti-MRSA empiric antibiotics in hospitalized patients is established. With this current study, we look to determine the optimal frequency of MRSA nasal testing to help limit unnecessary testing consistent with the efforts of Choosing Wisely.
