How far We Go for Surgery: Distance to Pediatric Surgical Care in Indiana
Despite increasing numbers of pediatric surgery training programs, access to pediatric surgical care remains limited in non-academic and rural settings. We aimed to characterize demographic and patient factors associated with increased distance to selected pediatric surgical procedures in Indiana. (Source: Journal of Pediatric Surgery)

Comparing healthcare needs in extremely low birth weight infants with NEC and spontaneous intestinal perforation
Necrotizing enterocolitis (NEC) and spontaneous intestinal perforation (SIP) affect 6-8% of extremely low birth weight (ELBW) infants. SIP has lower mortality than NEC, but with similar short-term morbidity in length of stay, growth failure, and supplemental oxygen requirements. Comparative long-term neurodevelopmental outcomes have not been clarified. (Source: Journal of Pediatric Surgery)

PIM Kinase inhibition sensitizes neuroblastoma to doxorubicin
Chemoresistance contributes to relapse in high-risk neuroblastoma. Cancer cells acquire resistance through multiple mechanisms, including drug efflux pumps. In neuroblastoma, multidrug resistance-associated protein-1 (MRP1/ABCC1) efflux pump expression correlates with worse outcomes. These pumps are regulated by PIM kinases, a family of serine-threonine kinases, overexpressed in neuroblastoma. We hypothesized PIM kinase inhibition would sensitize neuroblastoma cells by modulating MRP1. (Source: Journal of Pediatric Surgery)

Letter to the Editor in Response to: Complete Resection of Necrotic Bowel Improves Survival in NEC
We read with great interest the article by Pardy C et al entitled “Complete Resection of Necrotic Bowel Improves Survival in NEC Without Compromising Enteral Autonomy (J Pediatr Surg. 2024 Feb;59(2):206-210) [1]. We congratulate the authors for their pioneering work showing that survival increased when complete resection of necrotic bowels was performed in patie nts with surgical necrotising enterocolitis (NEC). They performed an ostomy in 36 of the 44 babies with surgical NEC, completely resected the necrotic intestines in 27 patients, and left i- situ the necrotic intestines in 17 patients in their study. Primary Liver Transplant in Biliary Atresia: The Case for and Against
The role of liver transplantation as a primary procedure in biliary atresia has been argued over for at least 40 years, indeed since the coming of age of safe liver transplantation during the 1980s. Yet, it is not a common option in most series (usually ≤5%) and typically reserved for those with late presentations (arguably>100 days) with established cirrhosis. This review presents the pros and cons of primary liver transplant. The pros are based upon the observation that at best a Kasai portoenterostomy (KPE) is simply palliative in most, and at worse has no effect whatsoever on restoration of bile flow and is therefore ...

History of the Section on Surgery, The American Academy of Pediatrics: The Third 25  Years (1998–2023)
The Section on Surgery of the American Academy of Pediatrics celebrates its 75th anniversary in 2023. To continue the tradition begun 25 years ago with analyses of the first two 25 years timespans of the organization, this examination focuses on the most recent quarter century. To frame this review, we begin with the seminal beginnings with the work of the section's founder, Herbert E Coe and reflect upon the twenty charter members of the organization. Then we explore the important accomplishments and challenges of the organization to the current time. (Source: Journal of Pediatric Surgery)

Utility of Indocyanine Green for Sentinel Lymph Node Biopsy in Pediatric Sarcoma and Melanoma
Indocyanine green (ICG) is a fluorescent dye with increasing use for adult sentinel lymph node biopsy (SLNB). The utility of ICG in pediatric oncology remains understudied. We aim to describe our experience using ICG for SLNB in pediatrics versus standard blue dye. (Source: Journal of Pediatric Surgery)
