Percutaneous Approach to Mechanical Aortic Valve Prosthesis Paravalvular Leak Closure: Lessons Learned
An adverse outcome of cardiac valve replacement, either open surgical replacement or percutaneous replacement, is paravalvular leak (PVL). The incidence of moderate or severe PVL has been reported up to 24% in post-mitral valve replacement, post-transcatheter or surgical aortic valve replacements, and mechanical valve replacements in either position.1-3 A clinically significant PVL can present with hemolytic anemia, symptoms of congestive heart failure, and is associated with increased mortality. Virtual Reality as Anesthetic Alternative for Ablation of Non-inducible Ventricular Arrhythmia
The anesthetic management of patients undergoing ventricular arrythmia ablation can vary from local anesthetic infiltration, moderate sedation, to general anesthesia (GA). The ideal anesthetic strategy for ablation of ventricular arrhythmias (VAs) is variable based on targeted arrhythmia, procedure time, patients age, comorbidities, risk of airway obstruction, and patient ability and/or desire to cooperate. Pulmonary vascular resistance-pulmonary artery compliance relationship and time constant in pulmonary hypertension due to valvular heart disease
Pulmonary vascular resistance and pulmonary artery compliance are inversely related and remain in a fixed relationship during a wide variety of experimental conditions. However, an increase in pulmonary capillary wedge pressure may alter the relationship between resistance and compliance in the pulmonary circulation.1-3 The time constant of the pulmonary circulation (determined by the product of pulmonary vascular resistance and arterial compliance) has been proposed as a more comprehensive index of right ventricle afterload than either variable alone.
