Gender is independently associated with red blood cell and platelet transfusion in patients undergoing coronary artery bypass grafting: data from the Netherlands Heart Registration
Coronary artery bypass grafting (CABG) is the most common cardiac operation worldwide. It is associated with perioperative blood loss and a high rate of blood products use (1, 2). Perioperative RBC transfusion is associated with increased risk of postoperative morbidity and mortality after CABG. (3-5)In the United States, the reported transfusion rates after CABG present a wide variability, ranging from 7.8% to 92.8% for red blood cells (RBC) with a mean of 56%. An intracardiac shunt in a patient undergoing left ventricular assist device implantation
Upon the initiation of a left ventricular assist device (LVAD), the left side of the heart is decompressed, in some instances establishing a right-to-left interatrial pressure gradient. This pressure gradient is further compounded in the setting of pulmonary hypertension and/ or right ventricular dysfunction. Therefore, to minimize the risk of a right-to-left intracardiac shunt, hypoxemia and paradoxical embolization, interatrial communications are often closed in patients undergoing implantation of a durable LVAD. Beating Heart Transplants —Overview and Implications for Anesthesiologists
As the demand for heart allografts for transplantation continues to rise, ex vivo organ perfusion strategies are playing an increasingly important role in the preservation of organs from donation after circulatory death and extended-criteria donors. One such method uses the Organ Care System (TransMedics, Andover, MA). Traditionally, this technique of preservation requires 2 periods of warm ischemia and subsequent cardioplegic arrest. In a novel surgical technique pioneered at the authors ’ institution, heart allograft implantation no longer requires a second cardioplegic arrest. How Would I Treat My Own Thoracoabdominal Aortic Aneurysm: Perioperative Considerations From the Anesthesiologist Perspective
A thoracoabdominal aortic aneurysm (TAAA) can be potentially life-threatening due to its associated risk of rupture. Thoracoabdominal aortic aneurysm repair, performed as endovascular repair and/or open surgery, is the recommended therapy of choice. Hemodynamic instability, severe blood loss, and spinal cord or cerebral ischemia are some potential hazards the perioperative team has to face during these procedures. Therefore, preoperative risk assessment and intraoperative anesthesia management addressing these potential hazards are essential to improving patients ’ outcomes. Extracorporeal Membrane Oxygenation for Myocardial Infarction-Related Cardiogenic Shock: The Devil is in the Details
I read with great interest a recent article in the Journal of Cardiothoracic and Vascular Anesthesia.1 The authors provided insightful commentary about a paper on extracorporeal life support (ECLS) in cardiogenic shock due to myocardial infarction. The study reported no difference in survival between patients supported by ECLS versus those who were not.1,2 The authors emphasized that a low incidence of left ventricular (LV) unloading was observed in the intervention arm at 5.8% (11 of 191 patients), and discussed the importance of the maneuver.
