Pain mechanisms for the practicing rheumatologist
Best Pract Res Clin Rheumatol. 2024 Mar 22:101942. doi: 10.1016/j.berh.2024.101942. Online ahead of print.ABSTRACTPain in rheumatic diseases transcends the traditional nociceptive paradigm, incorporating complex interactions between nociceptive, neuropathic, and nociplastic mechanisms, as well as significant psychosocial factors. Advances in understanding chronic pain highlight the role of peripheral and central sensitization, and the emergence of nociplastic pain-a result of altered central nervous system processing. This modern perspective acknowledges the influence of mood disorders, environmental stressors, and cogniti...

Suicidal ideation in patients with diabetes and childhood abuse - The mediating role of personality functioning: Results of a German representative population-based study
CONCLUSIONS: Diabetes patients experience increased SID prevalences, especially those with emotional or sexual abuse. In individuals with a history of emotional abuse, impaired personality functioning partly explained SID and should therefore be considered and addressed in this patient group.PMID:38521129 | DOI:10.1016/j.diabres.2024.111635 (Source: Clin Med Res)

Large language models as decision aids in neuro-oncology: a review of shared decision-making applications
J Cancer Res Clin Oncol. 2024 Mar 19;150(3):139. doi: 10.1007/s00432-024-05673-x.ABSTRACTShared decision-making (SDM) is crucial in neuro-oncology, fostering collaborations between patients and healthcare professionals to navigate treatment options. However, the complexity of neuro-oncological conditions and the cognitive and emotional burdens on patients present significant barriers to achieving effective SDM. This discussion explores the potential of large language models (LLMs) such as OpenAI's ChatGPT and Google's Bard to overcome these barriers, offering a means to enhance patient understanding and engagement in their...

Major Adverse Events in Youth-Onset Type 1 and Type 2 Diabetes: The SEARCH and TODAY Studies
CONCLUSIONS: In youth-onset diabetes, end-organ events were infrequent but did occur before 15 years diabetes duration. Rates were higher and had different risk factors in T2D versus T1D.PMID:38493952 | DOI:10.1016/j.diabres.2024.111606 (Source: Clin Med Res)

Associations of overweight and obesity with the risk of cardiovascular disease according to metabolic risk factors among middle-aged Japanese workers: The Aichi Workers' cohort study
CONCLUSIONS: Overweight/obesity was associated with the risk of CVD and its subtypes. About 30% of the risk was explained by hypertension, hyper-LDL-cholesterolemia, and diabetes, of which hypertension accounted for approximately the half of the explained risk. However, overweight/obesity increased the risk of CVD even in those without hypertension. These findings highlight the importance of controlling and preventing overweight/obesity regardless of chronic disease status.PMID:38480065 | DOI:10.1016/j.orcp.2024.02.006 (Source: Clin Med Res)
