“Mouths full of gold.” Private practice in Britain’s National Health System
By SAURABH JHA, MD When Aneurin Bevan was asked how he convinced doctors to come on board the National Health Service (NHS) he allegedly replied, “I stuffed their mouths full of gold.” Bevan recognized that to conscript doctors to the largest socialist experiment in healthcare in the world he had to appeal not so much to their morals, but pockets. There is much piety about the NHS. It is the envy of the world, though oddly Saudi oil barons still favor Cleveland Clinic and Texas Heart Institute over quaint little hospitals in rural Scotland. The NHS featured in Britain’s 2012 Olympic parade along with Mr. Bean and the...

Impeachment for " Impulsive, Ignorant Incompetence " ?
In Sunday ’s episode of “Reality-Show Presidency,” we found out what happens when the president and the chairman of the Senate Foreign Relations Committee“stop being polite… and start getting real.” After President Trump blasted him in a series of tweets early Sunday, Sen. Bob Corker (R-TN) shot back: Senator Corker is hardly the only highly placed Republican to express grave doubts about Trump ’s stability and competence. Daniel Drezner has assembled a list—now at 115 items and counting—of news stories in which the president’s own aides or political allies talk about him as if he ’s a “toddler.”...

More Questions Keep Coming About The Security Of The myHR. Maybe A Little More Disclosure Would Help Allay Concerns.
This appeared last week:Are organisations in denial about My Health Record ’s safety?Antony Scholefield6 October 2017 TECH TALKIf you believe the RACGP, the AMA and the Australian Digital Health Agency, the illegal sale of Medicare numbers doesn’t expose any security dangers with My Health Record. All three organisations have fronted a Senate inquiry into how Medicare n umbers ended up for sale on the ‘dark web’ for a bargain rate of about $25 each in July. The prevailing theory is that whoever stole the Medicare numbers exploited legitimate access to the Health Professional Online Services (HPOS) website, and senato...

VisualDx + Derm Expert, a Deep Learning App to Help Diagnose Skin Conditions
When a person develops an unsightly skin condition, the first person to assess it is typically not a dermatologist. A lack of expertise in dermatology too often leads to misdiagnoses. General practice physicians, for example, get very little training on skin conditions. Now, thanks to a new feature inside the latest Apple iOS for iPhones and iPads, any physician can have access to an impressive image recognition platform that identifies skin conditions and recommends further actions. We spoke with Dr. Art Papier, CEO of VisualDx, about the VisualDx + Derm Expert app that the company is making available. The new app is cert...

Patients need more time with their physicians; Physicians need more time with their patients
This article comes from Great Britain and their wonderful NHS.  It is slightly less bad in the USA.  Electronic health records have worsened the problem because charting takes much longer. Many experts believe that inadequate time leads to diagnostic errors. Others (as the article suggests) note that shared decision making takes time, and too often we do not have adequate time to have those discussions. We waste resources trying to cram too many patients into the physicians day.  We cannot say this often enough.  This is not a doctor issue; this is a patient issue.  Patients suffer when we shorten their visits. &nbs...
