Hormonal treatments for prostate cancer are often given late
Men with advanced prostate cancer are typically treated with drugs that cause testosterone levels to plummet. Testosterone is a hormone that fuels growing prostate tumors, so ideally this type of treatment, which is called androgen deprivation therapy (ADT), or hormonal therapy, will stall the disease in its tracks. For that to happen, ADT has to be administered correctly. But according to a new study, men frequently don’t get ADT at the proper dosing intervals. Too many of them get the treatments later then they should, causing testosterone levels to rise unacceptably. “Rapid increases in testosterone followin...

We Seem To Have The ADHA Continuing To Fail To Live Up To Their Hype!
This appeared a few days ago. 12 August 2020 ADHA pulls brakes on eScripts Pharmacy Practice Management Technology Posted by Francine Crimmins The national rollout of eScripts has experienced a false start, with the Australian Digital Health Agency asking GP software companies to remove the electronic prescribing functionality that was only launched in late July. GPs who were early adopters and already updated their software to issue eScript tokens will still be able to use the function. But for others, the update will remain unavailable, unless your practice is one of the designated “communities of interest”. The ADHA...

What is ME – Myalgic Encephalomyelitis?
Myalgic Encephalomyelitis, also known as Chronic Fatigue Syndrome, is a little-understood, female-biased [1] illness. It is estimated that up to 90% of people with the condition go diagnosed [2] and about 25% of sufferers are severely ill [3]. In recognition of the profound physical and psychological impact this disease has on its victims, August 8th has been designated as a Severe ME Awareness Day. What is Myalgic Encephalomyelitis (ME)? The disease is best known for a state of extreme and unremitting fatigue. A range of additional symptoms may suggest the presence of an infectious disease: myalgia, nausea, cogniti...

How might Covid-19 affect the number of GPs available to see patients in England?
Health Foundation -This research reveals that one in three GPs who singlehandedly manage a GP practice are at high risk of death from Covid-19. It estimates that this could potentially leave 710,043 patients without face-to-face appointments with their GPs if those at high risk take the difficult decision to limit direct patient contact. It argues that CCGs must ensure that they are aware of gaps in face-to-face provision of core general practice services, and must work with practices and primary care networks to find solutions.BriefingHealth Foundation - press release (Source: Health Management Specialist Library)

General practice in the post Covid world: challenges and opportunities for general practice
This report makes the case that throughout the pandemic, general practice has shown it functions well with fewer bureaucratic processes, such as contractual and regulatory compliance activities. While it recognises the need for regulation and other processes to ensure patient safety and quality of care, the report says there also needs to be a shift to a ‘higher trust’ model that gives GPs more time to deliver patient care.ReportRoyal College of General Practitioners - news (Source: Health Management Specialist Library)

General practice in the post Covid world: challenges and opportunities for general practice
This report calls on the four governments of the UK to each produce a comprehensive plan to support GPs in managing the longer-term effects of Covid-19 in the community. The RCGP says the plans should contain: costed proposals for additional funding for general practice; solutions for how the current GP workforce capacity can manage new and pre-existing pressures; commitments to continue the reduction in regulatory burdens and ‘red tape’ which has enabled GPs to spend more time on frontline patient care during the pandemic; a systematic approach for identifying those patients who are likely to require primary care supp...

Misaligned Relationships, Rejection, Guilt, and Betrayal
One of my readers recently asked: How do you stay connected to loved ones who aren’t on a journey of curiosity and growth? I’ll frame this more generally by addressing these question too: How do you maintain relationships with incompatible people who expect you to stay connected? How do you manage shifting relationships while on a journey of growth? How do you deal with feelings of guilt arising from letting go of incompatible people? To answer the first question, my answer is pretty simple. I really don’t. If they reach out to me, I’ll be civil with them, but I don’t see the point o...
