Gestational Age at Delivery and Neonatal Outcomes among Infants with Gastroschisis in the Children's Hospitals Neonatal Consortium (CHNC)
Conclusion: Premature infants with gastroschisis had more neonatal complications including respiratory interventions, longer NICU stay, longer time to full enteral feeds, and higher need for tube feeds at discharge as compared with those delivered at term. Differences were greatest for those <35 weeks GA. Maternal over-the-counter analgesics use during pregnancy and adverse perinatal outcomes: cohort study of 151 141 singleton pregnancies
Conclusions Over-the-counter (non-prescription) analgesics consumption during pregnancy was associated with a substantially higher risk for adverse perinatal health outcomes in the offspring. The use of paracetamol in combination with other non-steroidal anti-inflammatory drugs conferred the highest risk. The increased risks of adverse neonatal outcomes associated with non-prescribed, over-the-counter, analgesics use during pregnancy indicate that healthcare guidance for pregnant women regarding analgesic use need urgent updating. Avoidance of routine endotracheal intubation and general anesthesia for primary closure of gastroschisis: a systematic review and meta-analysis
ConclusionThere are few comparative studies examining the impact of performing primary bedside closure of gastroschisis without GA. A meta-analysis of the available data found no statistically significant difference when forgoing intubation and GA. Response to: Meconium-stained amniotic fluid as a predictor of poor outcomes
We read the article by Koehler, at al, titled Meconium-stained amniotic fluid as a predictor of poor outcomes[1] published in the Journal of Pediatric Surgery. The authors did a retrospective study enrolling 171 patients with gastroschisis from their data base between the years of 2000-2014. They mainly studied the effects of meconium stained amniotic fluid (MSAF) on these patients. Sixty-four patients of 135 (47.4%) that they included have MSAF. Relationship between volume and outcome for gastroschisis: A systematic review
Gastroschisis is a congenital anomaly with an incidence of 2.7 to 4.9 cases per 10,000 live births [1 –4]. It is characterized by a full-thickness defect in the abdominal wall, usually located to the right of the umbilical cord leading to extrusion of intestines or other organs into the amniotic fluid [5, 6]. Gastroschisis can be categorized into simple and complex cases. As complex cases present with intestinal atresia, stenosis, perforation or volvulus, they are associated with higher mortaliy and morbidity compared to simple cases [7, 8]. Evaluating the Risk of Peri-Umbilical Hernia after Sutured or Sutureless Gastroschisis Closure
Gastroschisis management protocols have undergone considerable study in the past decade, with increasing emphasis on the potential benefits of a sutureless approach to closure.
