Age-specific causes of upper gastrointestinal bleeding in children
World J Gastroenterol. 2023 Dec 21;29(47):6095-6110. doi: 10.3748/wjg.v29.i47.6095.ABSTRACTThe etiology of upper gastrointestinal bleeding (UGIB) varies by age, from newborns to adolescents, with some of the causes overlapping between age groups. While particular causes such as vitamin K deficiency and cow's milk protein allergy are limited to specific age groups, occurring only in neonates and infants, others such as erosive esophagitis and gastritis may be identified at all ages. Furthermore, the incidence of UGIB is variable throughout the world and in different hospital settings. In North America and Europe, most UGIBs...

Numerous gastric nodules in a leukaemia patient
Clinical presentation An 81-year-old white man presented with 2 weeks of odynophagia and dysphagia to solids and liquids limiting his oral intake and leading to 5 lb weight loss. He has a history of chronic myelomonocytic leukaemia with recent transition to myelofibrosis, uncontrolled gastro-oesophageal reflux disease (GORD) and chronic obstructive pulmonary disease on home oxygen. He denied fever, chills, nausea, vomiting, early satiety, abdominal pain, melena or haematemesis. The physical examination was unremarkable, and he had no oral thrush. His laboratory tests showed pancytopenia (absolute neutrophils count 62 cells...

Refractory Gastroesophageal Reflux Disease: Diagnosis and Management
J Neurogastroenterol Motil. 2024 Jan 30;30(1):17-28. doi: 10.5056/jnm23145.ABSTRACTGastroesophageal reflux disease (GERD) is common, with increasing worldwide disease prevalence and high economic burden. A significant number of patients will remain symptomatic following an empiric proton pump inhibitor (PPI) trial. Persistent symptoms despite PPI therapy are often mislabeled as refractory GERD. For patients with no prior GERD evidence (unproven GERD), testing is performed off antisecretory therapy to identify objective evidence of pathologic reflux using criteria outlined by the Lyon consensus. In proven GERD, differentiat...

PBPK modeling to predict the pharmacokinetics of pantoprazole in different CYP2C19 genotypes
In this study, we aimed to establish the physiologically based pharmacokinetic (PBPK) model to predict the pharmacokinetics of pantoprazole in populations with various CYP2C19 metabolic activities. A comprehensive investigation of previous reports and drug databases was conducted to collect the clinical pharmacogenomic data, physicochemical data, and disposition properties of pantoprazole, and the collected data were used for model establishment. The model was evaluated by comparing the predicted plasma concentration-time profiles and/or pharmacokinetic parameters (AUC and Cmax) with the clinical observation results. The p...

National trends in utilization and safety of gastric bypass, sleeve gastrectomy and conversion surgery in patients with GERD
ConclusionsThis data suggests that a strategy of performing a primary  SG and subsequent SG-RYGB conversion for those with recalcitrant GERD symptoms is not riskier than a primary RYGB. Thus, it may be reasonable to perform SG in patients who are well informed of the risk of worsening GERD requiring additional surgical interventions. However, the impact of such stage d approach (SG followed by conversion to RYGB) on long-term outcomes remains unknown.Graphical abstract (Source: Surgical Endoscopy)
