Psychiatrist treating obesity?
Given that some of the antiobesity medications (Contrave, stimulants, even phentermine-topiramate) as well as nonpharmacological options such as therapy (lifestyle modification, behavioral activation, motivational interviewing) are within the scope of practice for psychiatry, I was wondering how common it is for psychiatrists to be treating obesity outside of antipsychotic-induced weight gain? There are many more medications that we may not use (orlistat, GLP-1 agonists although this is... Read more (Source: Student Doctor Network)

How will you escape the pit (the ER)?
A few years into practice and already starting to wonder if EM is sustainable. I’ve considered plenty of other options. The ones I keep coming back to are wound care, obesity medicine, and maybe something like the VA or teaching if a med school will hire me. In the current world though, I’m not sure what the best exit strategy is. For those of you who plan to leave the pit, how are you getting out? (Source: Student Doctor Network)

Switching research areas post-tenure
I'll be going up for tenure in a year or two (contemplating an early tenure application but will be meeting with may department head at the end of this month for feedback). My primary focus in research up to this point has been in mental health disparities and the intersection of mental health and SES, but I've had a long-standing interest in eating disorders that I never pursued because my undergrad didn't have any faculty who studied them, and they tend to be a more competitive area of... Is starting a weight loss clinic while in residency possible?
During medical school I became interested in weight loss and got heavily involved in research. I've started working through the requirements for the American Board of Obesity Medicine CME pathway and should be done in a couple weeks. Part of the requirement to state that you are ABOM certified (even though it's not a recognized ABMS specialty) is to have completed a residency program and be board certified within that specialty. I'd have the information, and feel like I know more of it... Read more (Source: Student Doctor Network)

Blood glucose level to cancel cataract surgery
Thoughts on this? What would be your BG range and cut-offs? Recently I cancelled one whose BG was 400-500 mg/dL. Pt claimed to be compliant with all her diabetic medications. Talked to her PCP and found out she was lying and that she actually ran out of her humalog a month ago. No recent A1c.. given the hx I can assume her BG in the 400-500's range for the past month. Not the healthiest patient either, with CHF exacerbation in the last 3 months, known CAD, morbidly obese. Known history of... Read more (Source: Student Doctor Network)

Bupropion contraindications
Hello all, I am wondering, how strict are you with the FDA contraindications of bupropion and how would you handle the following situation? I tend to draw the line at not prescribing to people with an active restrictive or purging type eating disorder, as well as known seizure history. Now, what if you had a patient on bupropion with active anorexia and a history of seizure, whom you inherited from a colleague. You took them off bupropion and they are doing poorly. There are other options... Read more (Source: Student Doctor Network)

OB epidural tip
On another forum I saw a thread that mentioned unsuccessful attempts to place an epidural in OB due to being off center. When you get an extremely obese patient where you are unable to palpate or see any landmarks I always found it useful to ask the patient to point to the center of their back. They would unerringly point to the midline. Hope this helps someone at 4 a.m. (Source: Student Doctor Network)

Does the topic of research in a post bac RA position influence topic of the PhD research significantly?
Hi All, I'm coming up on I found a research coordinator position that I'm interested in. It deals with a population that I find interesting, but maybe would not want to make a career out of: people with eating disorders, specifically bulimia. It's in a great area, deals with some techniques I find interesting (neuroimaging), and is in an AMC. It's a position that I think, possibly naively, may actually be in the cards for me being that my undergrad research has been in psychobiology and so... Read more (Source: Student Doctor Network)
