Eating disorders in adults: what can we even do
I have no pithy comments for this week's conversation starter. Just frustration and frequent helplessness. I do CL and so of course see adult eating disorder patients in the hospital who are extremely ill. The adult hospital is not equipped to take care of them. I have seen some very bad outcomes. These diseases have a very high mortality rate, but I wish I knew what the mortality rate would be if they actually got quality care (inpatient OR outpatient). I'm sure it would still be high but... "I'm Glad My Mom Died" by Jennette McCurdy
Anyone else read/listen to this? It's a memoir by a former actress who was on an extremely successful live action tween show (ICarly) from 2007-2011. She talks a lot about how emotionally and sexually abusive her mother was to her and her siblings (as well as doing things like chasing her dad around the kitchen with a knife on a frequent basis), and her mom teaching her how to be anorexic at the age of 11, which later became severe bulimia (multiple binge-purge sessions a day, etc). It's... Read more (Source: Student Doctor Network)

Opening for Child Fellowship in my same city - Conflicted
Hey everyone, I'm a PGY3 and there is an opening for a child psych spot at a local university, and I am conflicted. I do enjoy my child outpatient days, particularly teenagers struggling with trauma, eating disorders, depression and anxiety; but I also find it exhausting. And I am not a big fan of autism. I also find parents exhausting, and that every patient visit takes forever due to overbearing parents. I haven't had any inpatient child experience either. I guess the pros are that it... Obesity is associated with central µ-opioid receptor (MOR) downregulation
Mesolimbic opioid-dopamine interaction is disrupted in obesity but recovered by weight loss following bariatric surgery - Translational Psychiatry Obesity is a growing burden to health and the economy worldwide. Obesity is associated with central µ-opioid receptor (MOR) downregulation and disruption of the interaction between MOR and dopamine D2 receptor (D2R) system in the ventral striatum. DO Schools To Avoid/Go For?
Not applying this year but I only have had 1 MD interview from VCU (which has specific release dates for acceptances), on 6 holds (Drexel, George Washington, Penn State, West Virginia, UCSD (in-state for me), SUNY Upstate) and 5 MD rejections (4 out of state schools and Georgetown). The physician I'm shadowing is an MD but recommended DO Schools to apply to in case I don't get in this year (she's a family medicine doctor who also focuses on obesity medicine and prefers improving lifestyles... Bariatric surgery
Can a person who is not obese pay cash and get weight loss surgery? Or would the surgeon refuse it unless they are BMI 40 or some cutoff like that? (Source: Student Doctor Network)

sarcoma case drain site
I have a case of a pT1(4cm)N0MO Grade 2 sarcoma s/p resection by a good surg onc. Margins 4mm in two areas (deep and anterior), 1 mm "superficial" margin that surg onc feels is artifactually close (he took some skin). This was a superficial sarcoma in an obese leg. Just barely touched deep fascia but did not invade into muscle though surg onc took some muscle. Tumor involved saphenous vein and it had to be ligated. Patient already has some minor distal edema. I took a cartoon of a... Read more (Source: Student Doctor Network)

Outpatient tonsils question
The 2019 AAO-HNFS guidelines for tonsillectomy state that pediatric patients with severe OSA should be admitted for post-op monitoring and more specifically that obese patients with BMI >95% should undergo sleep studies to check for severe OSA. However, per their own admission, compliance with these guidelines is quite low, roughly in the 40% range. There are a number of reasons for non-compliance including lack of sleep study availability, cost, disagreement with the validity of the... Read more (Source: Student Doctor Network)
