A 74 year old attorney presents for Right Shoulder replacement. He was a civil trial lawyer but retired 6 years ago. HIs main concern is "stroke" from the anesthesia as he was informed by the surgeon his position will be beach chair for 2.5 hours. PMH: HTN CAD One Coronary stent placed 2 years ago; recent stress test from 6/2011 is fine. Taking Advice From The Wrong People
If your car broke down would you call a mechanic or someone that is interested in being a mechanic that has no actual experience? If you were to hire a personal trainer, would you hire someone that was obese? Obviously not. But, even crazier, most of you trust advice about getting into dental school from someone that has not successfully applied and earned a seat in dental school. How backwards is that? OB case
30 yo female morbidly obese (BMI of 48) 35weeks pregnant with urosepsis. Currently hypotensive 80-90/40-50 in ICU on 10mcg norepinephrine and beginning to contract every 2 min. What to do when you suspect disease in people you dont know?
I saw a lady yesterday working at the local department store, with the classical symptoms of Cushing's dz (buffalo hump, red moon face, central obesity, thin arms and legs,etc). Obviously, I don't know for sure if she has Cushing's or I would have told her. I mean, I am just a medical student and Im 90% likely wrong about this. Still, I felt guilty and not sure what to do in this situation. I feel that we have a responsibility with the knowledge we have learned but at the same time, don't... Lobectomy Fluid Management
Seeking comments and advice from those of you much wiser and more experienced than than I am. (All of you) Question about intraoperative fluid management in this theoretical case: Morbidly obese 67 yo F 5'2" 137kg with COPD and history of delayed emergence presents for right VATS upper lobectomy. Case goes for 8hrs induction to emergence (no conversion to thoracotomy). Difficult DLT placement takes 20 minutes of tweaking to sit right. Ultimately complicated by ~1200ml blood loss, lowest...
